[Long-term results of gastric ulcers chemotherapy].

AIM: Determination of criteria of choice of gastric ulcer (GU) treatment. MATERIALS AND METHODS: 904 GU patients given conservative drug therapy in 1986-1996 were followed up. RESULTS: Recurrences of GU can be of three types: type 1--rare recurrences (once in 10-11 years), type 2--recurrences occur twice for 10-11 years), type 3--recurrences occur every 1.5 years). CONCLUSION: A simple criterion of assessing effectiveness of conservative treatment of uncomplicated ulcers is proposed. It is duration of remission. If the recurrence occurs within 1.5 years after the treatment, the patient is at risk of frequent recurrence. In such cases surgical treatment is justified.

[The reliability of endoscopic gastric biopsy in the diagnosis of the malignant degeneration of stomach ulcers].

A comparative analysis of results of the pre- and postoperative investigations carried out in 232 patients with gastric ulcerations of different nature has shown a relatively low effectiveness of fibrogastroscopy with biopsy in the recognition of malignant ulcers. The sensitivity of this method was found to be (83 +/- 3)%. The results obtained show the necessity to widen the operative treatment of patients with ulcer disease of the stomach, patients with the so called relative indications included.

[Expanding the indications for the surgical treatment of gastric peptic ulcer].

A group of patients operated upon for ulcer disease of the stomach was selected from 1279 patients with this disease in which the nearest results of the surgical treatment were followed-up retrospectively. Seven patients died. The average period of postoperative treatment in the hospital was (17.7 +/- 1.5) days. Complications developed in 78 patients. A comparison of results of preoperative examinations with the conclusions of the morphologist according to data of studying the operative specimens has established low sensitivity of the current methods of diagnosing the cancerous nature of ulcers--as low as (72 +/- 5)%. The final diagnosis of a malignant ulcer was made in 81 patients, only in 58 of them the malignization was suspected before operation, in 23 patients the tumoral process was a "chance" finding. This fact speaks in favour of widening the indications for operative treatment of noncomplicated gastric ulcers.

[Effectiveness of differential diagnosis of benign and malignant ulcers at endoscopy].

AIM: Assessment of efficacy of differential diagnosis between benign and malignant lesions in gastric mucosa by means of endoscopy. MATERIALS AND METHODS: Morphological verdicts upon investigation of operative specimens were compared to findings at preoperative duodenofibrogastroscopy with spot biopsy in 326 patients operated for gastric ulcer. The number of discrepancies between the endoscopic and morphological diagnoses enabled estimation of endoscopy + biopsy efficiency in identification of the ulcer malignancy. RESULTS: The number of diagnostic errors made on malignant or benign nature of ulcer at endoscopic and biopsy investigations is rather big, i.e. at least 12% for both false negative and false positive hypophyses. The overall diagnostic efficacy of endoscopy in detection of the disease lies within 65-80%. CONCLUSION: Reliability of endoscopic identification including biopsy of malignant nature of gastric ulcers is not very high. Real sensitivity of this method in detection of ulcers undergoing malignant transformation reaches 76-78%.

[The efficacy of the conservative treatment of patients with acute appendicitis on board ships at sea].

Under study was efficiency of conservative treatment of 247 patients with acute appendicitis on ships in the sea depending on the age of patients, clinical manifestations of the disease, volume of medicamentous therapy and time of taking medical advice. It was established that conservative treatment is most effective if started within the first 6 hs from the beginning of the disease and antibiotics were used in combination with spasmolytics, antihistamines and sulfanilamides. Patients treated conservatively had pronounced inclinations to delimiting the inflammatory process in the abdominal cavity.

[Blood reinfusion in penetrating gunshot wounds of the abdomen].

On the basis of literature and their own experience in performing the reinfusion of bacteria-contaminated blood in 20 patients with gunshot penetrating abdominal injuries the authors make a conclusion that in critical cases there are no alternative to application of reinfusion. The article gives clinical examples and contains data concerning the character of lesions. Indications and counter-indications for reinfusion are enclosed.

[Combined cycles--a new form of advanced training for physicians. Initial experience and perspectives].

A new organizational form of postgraduate training of physicians was substantiated. Thus it became possible to combine high educational, methodological and scientific potential of the institutes (faculties) of advanced training of physicians with the possibilities of large hospitals in applied training of physicians. Combined specialized cycles on ophthalmology demonstrated a number of advantages of such training compared to the traditional techniques and also indicated some conditions for raising its effectiveness. Even now it is possible to expand the range of specialties on combined training of physicians and in the future to reorganize training of young interns.
