Rapid rise in FDG uptake in an irradiated human tumour xenograft.

In order to investigate early changes in the glucose metabolism of irradiated tumours, tumour uptake of 2-[18F]fluoro-2-deoxy-d-glucose (18FDG) was studied in human tumour xenografts. Three human tumour lines [ependymoblastoma (NNE), small cell lung cancer (GLS), and glioblastoma (KYG)] showing different radiosensitivities and incidences of radiation-induced apoptosis were subcutaneously transplanted into nude mice, and were irradiated at a single dose of 10 Gy. Then 0.5 mCi of 18FDG was intravenously administered 1 h before sacrifice. The animals were sacrificed at 2, 4 and 6 h following irradiation, and 18FDG accumulation in the tumours was examined. Before irradiation, GLS and KYG tumours showed significantly higher rates of 18FDG accumulation compared with NNE tumours (P <0.004 and P <0.001, respectively). NNE (the most radiosensitive tumour with the highest incidence of radiation-induced apoptosis), however, displayed a 2.3-fold higher rate of 18FDG accumulation at 2 h following irradiation compared with a non-irradiated group (P <0.01), and thereafter showed a plateau up to 6 h. The accumulation did not increase significantly in the other tumours with lower radiosensitivity and much less radiation-induced apoptosis. The rapidity of the increase in 18FDG accumulation in the most radiosensitive tumour line, occurring as early as 2 h following irradiation, suggests that the increase was independent of recovery phenomena following radiation damage.

Aggravation of the nutcracker syndrome during pregnancy.

BACKGROUND: The nutcracker syndrome is a rare condition thought to be caused by compression of the left renal vein between the descending aorta and the superior mesenteric artery. CASE: Gross hematuria appeared in the third trimester of an otherwise normal pregnancy. It continued despite treatment, and a cesarean was performed at 37 weeks' gestation. The hematuria stopped postpartum. With postpartum angiography and three-dimensional computed tomography, the diagnosis of nutcracker syndrome was finally made. CONCLUSION: Pregnancy can aggravate the nutcracker syndrome. This syndrome should be recognized as one of the diseases that causes gross hematuria.

Radiation therapy for stage III non-small cell lung cancer invading chest wall.

The treatment outcome and prognostic factors for Stage III non-small cell lung cancer (NSCLC) invading the chest wall and treated with definitive radiation therapy alone were investigated. From 1976 to 1990, 40 consecutive patients with Stage III NSCLC invading the chest wall underwent radiation therapy alone. There were 31 with Stage IIIA and nine with Stage IIIB. Squamous cell carcinoma was present in 30 patients (75%). All the patients underwent radiation therapy at a total dose of 60-80 Gy at 2 Gy per fraction. Local pain completely disappeared in 11/13 (85%) and decreased in the remainder after treatment. The 5-year survival rate was 8% for all patients, 10% for Stage IIIA and 0% for IIIB (P = 0.02), 11% for lesions invading the parietal pleura and 0% for those invading the ribs or spine (P = 0.2). Good performance status, Stage IIIA, metastases of lymph nodes less than N3 and small tumor size were better prognostic factors by univariate analysis. By multivariate analysis, performance status (P = 0.01) and Stage (P = 0.03) were the important prognostic factors. Five-year local progression-free survival rates were 51% for patients with lesions invading the parietal pleura and 0% for those invading the ribs or spine (P = 0.009). Good pain relief was achieved in patients with lesions invading the ribs or spine by radiation therapy. Tumors invading the parietal pleura were fairly well controlled by radiation therapy alone.

A case of pulmonary ossified adenocarcinoma with marked osteoplastic bone metastasis.

We report the case of a 53-year-old man suffering from a pulmonary adenocarcinoma with ossification and diffuse metastatic osteoplastic lesions throughout the skeletal system. This is a rare condition in lung carcinomas. Radiographs of the chest and bones demonstrated mineral densities in the primary tumor and multiple expansive osseous lesions with a diffuse sclerotic pattern resembling multiple bone metastases from prostatic carcinoma.

Split-course accelerated hyperfractionation radiotherapy for advanced head and neck cancer: influence of split time and overall treatment time on local control.

We analyzed 52 patients with stage III and IV head and neck cancer who were given split-course accelerated hyperfractionated radiotherapy with curative intent, focusing particularly on the influence of split-time on local control. An initial complete response was achieved in 16 patients (31%), and the rate of persistent local control at 3 years was 23%. The cause specific survival rate at 3 years was 29%. Univariate analysis of local control according to the split-time duration and overall treatment time showed that shorter duration (< or = 14 days or < or = 45 days, respectively) had a significantly positive impact on local control (P < 0.05). Multivariate analysis using local control as an endpoint also demonstrated that gender (women showing a better outcome than men) and split-time (< or = 14 days was better than > 14 days) were statistically significant factors for local control. These results suggest that shortening the split-time during radiotherapy might improve local control in accelerated hyperfractionation.

Spatiotemporal dynamics of intracellular calcium in the mouse egg injected with a spermatozoon.

Oscillatory rises in intracellular Ca2+ concentration ([Ca2+]i) are the pivotal signal in the fertilization of mammalian eggs. The spatiotemporal dynamics of [Ca2+]i rises in mouse eggs subjected to intracytoplasmic sperm injection (ICSI) were analysed by Ca2+ imaging and compared with those subjected to in-vitro fertilization (IVF). The first Ca2+ transient occurred 15-30 min after ICSI in most eggs, and was followed by Ca2+ oscillations which lasted for at least 6 h at intervals of approximately 10 min. The pattern of Ca2+ oscillations, an initial relatively larger Ca2+ transient followed by smaller Ca2+ transients, was similar to that at fertilization. Confocal Ca2+ imaging during early Ca2+ transients showed that, in fertilized eggs, [Ca2+]i increased in a wave which started from the sperm attachment site and propagated across the egg cytoplasm. In eggs subjected to ICSI, [Ca2+]i increased gradually and then a Ca2+ spike was generated when [Ca2+]i reached a certain level. The [Ca2+]i rise occurred in the whole egg, associated with neither a wave nor significant heterogeneity between the cortical and central regions. It is suggested that cytosolic factor(s) may leak from the injected spermatozoon, diffuse slowly in the egg cytoplasm, and then cause a synchronous Ca2+ release from intracellular Ca2+ stores.

Evaluation of effect of treatment for invasive bladder cancer by ultrasonography with intra-arterial infusion of carbon dioxide microbubbles.

RATIONALE AND OBJECTIVES: The purpose of this study was to evaluate the diagnostic usefulness of the ultrasonography with intra-arterial infusion of carbon dioxide microbubbles (CO2) for invasive bladder cancer. METHODS: Twelve patients with muscle-invading bladder cancer who were treated by concurrent radiotherapy and intra-arterial infusion of daily low dose of cisplatin using an implanted infusion port were included. A total of 30 studies was performed during the treatment to evaluate the visualization of the tumor and effect of the treatment compared with conventional ultrasonography, computed tomography, or cystoscopy. RESULTS: Satisfactory visualization of the tumor in CO2 ultrasonography was obtained in all patients, in particular in those with flat tumor or prostatic invasion. The enhancement effect of CO2 on the tumor, which was maintained well in the late period of the treatment, made possible evaluation of the therapeutic effect. With respect to the evaluation of local response, disagreement between clinical response and the evaluation of CO2 ultrasonography was observed in two patients with definite differentiation between wall edema and residual tumor after treatment being difficult. CONCLUSIONS: Carbon dioxide ultrasonography is easy to perform in patients treated with arterial infusion therapy using an implanted infusion port and provides practical information in evaluating therapeutic effect.

Effects of fibrin glue on postsurgical adhesions after uterine or ovarian surgery in rabbits.

OBJECTIVE: To evaluate the prophylaxis with fibrin glue on postoperative adhesions following uterine or ovarian surgery in rabbits. STUDY DESIGN: The uterine horns of 10 rabbits were exteriorized and electrocauterized by monopolar coagulation. Fibrin glue was applied to one uterine horn and the other was left untreated as a control (Experiment 1). Multiple ovulation was induced in 12 rabbits with hCG, and wedge resection was performed on both ovaries of each animal. One ovary was treated with fibrin glue and the other was untreated as a control (Experiment 2). Second-look laparotomy was performed to assess adhesion formation 2 weeks after the initial operation. RESULTS: The use of fibrin glue resulted in significant decrease in postoperative adhesions compared with the control side, in the uterine horns and the ovaries (p < 0.02 and p < 0.01, respectively). CONCLUSION: Fibrin glue was useful in preventing postoperative adhesions of the reproductive organs in rabbits.

Applying Association Rule Discovery Algorithm to Multipoint Linkage Analysis.

Knowledge discovery in large databases (KDD) is being performed in several application domains, for example, the analysis of sales data, and is expected to be applied to other domains. We propose a KDD approach to multipoint linkage analysis, which is a way of ordering loci on a chromosome. Strict multipoint linkage analysis based on maximum likelihood estimation is a computationally tough problem. So far various kinds of approximate methods have been implemented. Our method based on the discovery of association between genetic recombinations is so different from others that it is useful to recheck the result of them. In this paper, we describe how to apply the framework of association rule discovery to linkage analysis, and also discuss that filtering input data and interpretation of discovered rules after data mining are practically important as well as data mining process itself.

Effect of Krestin as adjuvant treatment following radical radiotherapy in non-small cell lung cancer patients.

To evaluate the efficacy of Krestin (PSK) as adjuvant treatment after radical radiation therapy (RT) for non-small cell lung cancer (NSCLC), treatment results of 225 patients with NSCLC treated with RT followed by adjuvant administration of PSK between 1976 and 1989 were analyzed. Of these patients, 170 (76%) had squamous cell carcinoma. In the patients with squamous cell carcinoma of the lung, PSK was given only when the tumor showed satisfactory shrinkage (complete or partial response) after completion of RT. The treatment outcomes were compared with those of the responders to RT not receiving PSK. The 5-year survival rates of patients with stages I-II and stage III disease were 39 and 26%, respectively, while the non-administered responder group's were 17 and 8%. These differences are statistically significant. An improvement in the treatment results with combined use of appropriate immuno-modulating drugs is anticipated in the future. When clinical trials of the efficacy of these drugs are conducted, the agents should be given to the patients with satisfactory tumor regression after RT, although they still take much time and cost.

Ectopic lacrimal gland of the orbit.

A case of intraorbital ectopic lacrimal gland was evaluated with various radionuclides. CT and MRI with contrast enhancement showed a soft-tissue mass in the orbit. The lesion was gallium-avid, but 18F-FDG PET demonstrated very faint uptake. On early and delayed 201Tl-Cl images, mildly increased uptake and mild retention were observed. Technetium-99m-(V)-DMSA SPECT showed intense accumulation on both the early and delayed images. Technetium-99m-pertechnetate SPECT, demonstrating marked uptake in the lesion, was useful for the differential diagnosis between intraorbital malignancy and this benign lesion. These scintigraphic variations played an important role in the tissue characterization of this uncommon lesion.

Vitamin D receptor alleles, bone mineral density and turnover in premenopausal Japanese women.

Recent studies have shown that genetic effects on bone mineral density (BMD) and bone turnover are related to allelic variation in the vitamin D receptor (VDR) gene. We examined allelic influences of the VDR gene on bone turnover and density in 202 normal healthy premenopausal Japanese women (age 30.1 +/- 1.2, mean +/- SEM). The VDR effect on BMD and turnover is similar to that observed in Caucasian women; however, there are major differences in allele frequency. The B allele by BsmI restriction fragment length polymorphisms (RFLPs), associated with low BMD and high bone turnover, is found in only 12% of Japanese women (1.4% homozygote BB), compared with 41% of Caucasians (16.7% homozygote BB). In comparing the two most frequent genotypes, Bb heterozygotes (21.5%) and bb homozygotes (77.1%), BMD is 5.3% lower in Bb heterozygotes, and levels of bone formation markers including osteocalcin and bone-specific alkaline phosphatase are 20-32% higher with lower serum calcium (2.30 +/- 0.02 vs 2.35 +/- 0.01 mmol/l) and higher 1,25-dihydroxyvitamin D (95 +/- 4.8 vs. 76 +/- 3.8 pmol/l). Further discrimination of the genotype was achieved using two additional RFLPs (ApaI, A and TaqI, T); the lumbar spine BMD of the common genotype BbAATt was 9.3% (0.94 SD) lower than in the bbaaTT genotype in premenopausal Japanese women. These data confirm that VDR RFLPs affect bone mineral metabolism regardless of racial differences. Moreover, the VDR genotypes based on haplotype analysis should yield useful insights into the potential prevention of osteoporosis.

Prognostic significance of immunohistochemically detected p53 protein expression in stage IIIB squamous cell carcinoma of the uterine cervix treated with radiation therapy alone.

Immunohistochemical studies were performed to investigate the prognostic significance of p53 protein expression in 46 patients with stage IIIB squamous cell carcinoma of the uterine cervix who were treated with radiation therapy alone. Tumor cells showed p53 protein expression in 29 of 46 patients. The 5-year overall actuarial and cause-specific survival rates for the patients with p53-positive tumor were 52.9 and 70.4%, respectively. The corresponding rates for the patients with p53-negative tumor were 58.2 and 69.0%, respectively. There was no significant difference of local control rates and of distant metastases rates between the two groups. In conclusion, no relationship was observed between radiation treatment outcome and the immunohistochemically detected p53 protein expression in the patients with stage IIIB squamous cell carcinoma of the uterine cervix.

Reduction of adhesions with fibrin glue after laparoscopic excision of large ovarian endometriomas.

STUDY OBJECTIVE: To evaluate the effect of fibrin glue on the formation of adhesions after laparoscopic excision of endometriomas. DESIGN: Prospective case series. SETTING: Department of Obstetrics and Gynecology at a university-affiliated hospital. PATIENTS: Thirty women with ovarian cysts. INTERVENTIONS: In 10 women (16 cysts) we performed laparoscopic resection of endometriomas leaving the ovarian capsule open. Twenty patients (25 cysts) underwent laparoscopic removal of endometriomas and fibrin glue approximation of the ovarian capsule, as well as coating of serosal defects with the glue. At second-look laparoscopy 4 to 6 months after the initial surgery, postoperative adhesion formation was assessed according to the revised American Fertility Society classification. MEASUREMENTS AND MAIN RESULTS: The mean (+/- SEM) adnexal adhesion score in patients without fibrin glue was 8.6 +/- 2.2 at initial surgery and increased slightly to 12.6 +/- 2.6 at second-look laparoscopy. In patients treated with fibrin glue, the score decreased significantly from 10.9 +/- 1.4 to 7.8 +/- 1.3 (p <0.02). The decrease was most marked in women with endometriomas 5 cm or more in diameter and for those with a preoperative adhesion score of 8 or higher. CONCLUSION: Fibrin glue reduced postoperative adhesions after laparoscopic resection of endometriomas.

An interaction of cisplatin and radiation in two rat yolk sac tumour cell lines with different radiosensitivities in vitro.

We investigated the interaction of cisplatin and radiation in two rat yolk sac tumour cell lines with different radiosensitivities in vitro. The effects were assessed by clonogenic assay. D0 for the radiosensitivity of the radioresistant variant cell line, NMT-1R was 139 cGy, which was 1.3 times larger than that for the parent radiosensitive cell line, NMT-1 (D0 = 107 cGy). The concentration of cisplatin required to reduce colony formation by 50% at 1 h treatment (ID50 of cisplatin) was 0.25 microgram/ml for NMT-1, whereas that for NMT-1R was 1.0 microgram/ml. Cisplatin potentiation of radiation sensitivity was manifested by the decrease in the slope of the radiation dose-response curve. D0's for NMT-1 and NMT-1R were 83 and 100 cGy in combined treatment with ID50 of cisplatin immediately before radiation. The enhancement ratios of cisplatin were therefore 1.30 for NMT-1 and 1.39 for NMT-1R respectively in D0. No different enhancement ratio for cisplatin was observed in the time course of combination treatment with cisplatin and radiation within the interval of 6 h. There was no clear change in cell cycle distribution within 6 h after treatment with ID50 of cisplatin for both cell lines. In conclusion, cisplatin had a synergistic effect on both cell lines which was independent of the time course and sequence in combination with radiation within 6 h.

A radioresistant variant cell line, NMT-1R, isolated from a radiosensitive rat yolk sac tumour cell line, NMT-1: differences of early radiation-induced morphological changes, especially apoptosis.

A radioresistant variant cell line, NMT-1R, was isolated by repeated radiation exposure of a radiosensitive rat yolk sac tumour cell line, NMT-1, producing alpha-fetoprotein, with the potential for lymphatic metastasis in the inbred Wistar rat. Cultured NMT-1R cells showed more cobblestone-like appearances, although the morphological features were almost the same as radiosensitive NMT-1 cells reported previously. The doubling time of NMT-1R cells was 13.6 h, being shorter than that of NMT-1 cells (16.0 h). For NMT-1R cells, D0 for radiation sensitivity was 165 +/- 3 cGy, 1.7 times as large as for NMT-1 cells. The extrapolation number, n, was 1.48 +/- 0.17 for NMT-1R cells although that for NMT-1 cells was 1.08 +/- 0.15. The surviving fractions at 2 Gy (SF2) were 0.42 for NMT-1R cells and 0.28 for NMT-1 cells. The population of G2-M phase for NMT-1R cells was larger than for NMT-1 cells (32.5 versus 26.8%) in exponentially growing cells. Although a clear G2 delay was observed after irradiation with a dose of 182 cGy for both cell lines, NMT-1R cells had a shorter recovery time from G2 block than NMT-1 cells, G1 arrest was observed in NMT-1 cells. NMT-1 cells showed much higher incidence of early morphological changes, especially apoptosis, after irradiation with a dose > 500 cGy compared with NMT-1R cells.

Radiation therapy for stage I-II non-small cell lung cancer in patients aged 75 years and older.

Between 1976 and 1992, 32 patients aged 75 and older with stage I-II non-small cell lung cancer (NSCLC) were given definitive radiation therapy. These patients did not undergo surgery because of old age, poor cardiac/pulmonary condition, or refusal to give consent. The mean age was 79 years, and 11 patients were over 80 years old. The histologic type was squamous cell carcinoma in 25 patients and adenocarcinoma in 7. The clinical T and N stage was T1N0 in 4 patients, T2N0 in 9, and T2N1 in 19. The total dose of radiation therapy given to each patient exceeded 60 Gy using 10-MV X-rays. The treatment was completed in all 32 patients without treatment-related complications. The 2- and 5-year overall actuarial survival rates wer 40% and 16%, respectively. Eleven intercurrent deaths occurred, including 7 patients who died of heart disease. The 2- and 5-year cause-specific survival rates were 57% and 36% respectively. None of the patients developed severe pneumonitis requiring hospitalization. All but three patients received radiation therapy on an inpatient basis. The mean duration of the hospital stay for initial treatment was 56 days, and mean ratio to total survival period (mean 739 days) was 8%. Although many elderly patients have concurrent medical complications such as heart disease and chronic pulmonary disease, the present study showed that elderly patients with clinical stage I-II NSCLC can expect a realistic probability of long-term survival with definitive radiation therapy.

Results of radiation therapy for hypopharyngeal carcinoma: impact of accelerated hyperfractionation on prognosis.

We reviewed the results of treatment in 52 patients with hypopharyngeal carcinoma who underwent radical radiation therapy using a two-fractionation scheme--conventional fractionation (CF) and accelerated hyperfractionation (AHF)--in order to evaluate the impact of the fractionation scheme on prognosis. The 5-year survival rate for the 52 patients was 19.5%, and a significantly better rate was obtained with AHF (44.4%) than with CF (12.4%). Of 27 patients who showed a complete response (CR), a higher CR rate (61%) was achieved with AHF than with CF (47%). A similar trend was also observed for T3 and T4 tumors (55% with AHF vs. 36% with CF). Multivariate analysis demonstrated that neck node status was a significant factor related to local control, and that local response and the fractionation scheme employed were significant prognostic factors for survival. The recurrence rate and recurrence within the radiation field in the AHF group was lower than in the CF group (27% vs. 50%), although the time to recurrence in the two groups did not differ greatly. Voice preservation was achieved in 20% of the patients. Considering the better survival and local control rate compared with conventional fractionation, accelerated hyperfractionation seems a preferable treatment for hypopharyngeal carcinoma.
