Histological changes in the genital tract in transsexual women following androgen therapy.

Thirty-two young women who desired sexual reassignment were treated with large doses of androgen for a period of 1 year or more, followed by total hysterectomy. Histological examination revealed marked atrophy of cervical epithelium which could mimic dysplasia, and variable degrees of endometrial atrophy. The ovaries showed occasional corpora lutea indicating that even in the face of long term androgen therapy, ovulation may occur.

Primary and adjunctive intra-arterial digital subtraction arteriography of the lower extremities.

Standard contrast arteriography (SCA) and intra-arterial digital subtraction arteriography (DSA) were performed during a 26-month period in 459 cases. The DSA group consisted of 22 aortoiliac studies, 66 crural-pedal arch studies, and 227 combinations. In addition, postoperative DSA was performed in 42 patients to evaluate graft patency, morphology, and inflow and runoff circulations. There were no significant differences in the quality of the preoperative aortoiliac studies performed by either SCA or DSA although, in select cases, one or the other of these techniques resulted in a superior study. Distal crural-pedal arch visualization was enhanced with DSA compared with SCA (85% vs. 65%) but when both were compared with their corresponding intraoperative completion arteriograms, the interpretive error rates resulted in comparable accuracies, false positive and negative rates, and predictive values. The likelihood of achieving graft patency in patients who have unsatisfactory preoperative visualization of the distal circulation by DSA is reasonable (11 of 27 patients) but inferior to the number obtained when there is adequate DSA visualization (40 of 53 patients). We conclude that DSA is a valuable adjunct to preoperative SCA but should not be used as the sole criterion for the assessment of operability for limb salvage. Intraoperative prereconstruction arteriography or direct surgical exploration of the crural arteries in patients with inadequate preoperative visualization will result in graft patency in a significant percentage of cases. Intra-arterial DSA for postoperative evaluation of lower limb bypass adds another dimension to analysis of graft structure and status of the host circulatory beds and also provides a method for accurate interpretation of postoperative data.

Patterns of expression of keratin 19 as detected with monoclonal antibodies in human breast tissues and tumours.

The monoclonal antibodies BA16 and BA17 directed to different epitopes on human keratin 19 have been tested for their reaction with normal breast and with benign and malignant breast lesions and associated tissue. In Western blots of gel-separated extracts of fibroadenomas, malignant tumours or normal mammary epithelial cells, the antibodies reacted with only one component of 40 kd molecular weight. Immunoperoxidase staining of sections of normal breast tissues showed all basal cells and a few luminal cells to be unstained by the antibodies. The distribution of the unstained (keratin 19-) luminal cells in the mammary tree is consistent with that of cells with the proliferative potential to give rise to the growth of terminal ductal lobular units (TDLU) seen at pregnancy. A total of 42 benign and 141 malignant lesions were stained with the antibodies, and a clear difference in staining pattern was seen between the benign and malignant tumours. All but 3 of the benign lesions showed a heterogeneous staining pattern with 5-50% unstained cells. In contrast, the cancer cells in 106/116 invasive primary tumours and in all 21 metastatic lesions examined showed a homogeneously positive reaction with antibodies BA16 and BA17: the malignant cells in 4 cases of Paget's disease also showed homogeneously positive staining with the antibody. In the malignant tumours, the observed homogeneity in expression of keratin 19 was confined to the malignant cells; tumour-associated normal tissue and benign proliferative lesions contained keratin 19-cells. Seven pure in situ tumours were examined and 5 showed the homogeneous pattern of staining characteristic of invasive tumours while 2 contained a high number of keratin 19-cells. A general model is presented to explain the presence of keratin 19-cells in benign proliferation and the dominance of keratin 19-cells in invasive carcinoma.

The experience of a department of medicine with a clinical medical library service.

Five years of experience using the services of a full-time clinical medical librarian (CML) by the Department of Medicine at the Hospital of the Medical College of Pennsylvania have been reviewed. Over the five years there was a marked increase in queries from faculty and house staff members with an accompanying decrease in CML-initiated questions, while requests by students and others remained constant. The largest category of queries was patient-related. There were increases in the use of the CML for patient-related queries, for research, and for oral presentations. The number of requests obtained on rounds or in conferences decreased, while the number obtained by phone, in the library, or elsewhere increased. Requests for computer searches remained constant.

Effects of selenium and vitamin E on incidence of retained placenta.

In a series of experiments over 3 yr, relatively low doses of selenium and vitamin E were injected intramuscularly 3 wk prepartum to pregnant Israeli-Holstein dairy cows in a high-producing herd (9000 kg milk/yr) with a history of retained placenta in 17% of the primiparous and 28% of the multiparous animals, as recorded for the 12 mo preceding the study. Selenium ranged from .035 to .109 ppm in the prepartum diet and from .160 to .200 ppm in the postpartum diet. Doses of selenium ranging from 2.3 to 23.0 mg reduced the incidence of retained placenta in 186 primiparous and in 428 multiparous to 7 and 15%, which was half of that of controls. Low doses of selenium (2.3 to 4.6 mg) tended to be more effective than higher ones. Selenium alone was at least as effective as a combination of selenium and vitamin E. Dosage, timing, mechanism, and interactions of selenium with other factors in reducing the incidence of retained placenta need clarification.

Clinical industrial toxicology. An approach to information retrieval.

Exposure to hazardous chemicals often occurs in the workplace. Workers and health care providers may have little knowledge of the health effects of these substances. An algorithm was developed to outline a logical approach to toxicologic information retrieval. Printed material and information sources such as government agencies, manufacturers, unions, and poison control centers should be consulted as a first step. If additional information is needed, telecommunications systems provide access to online databases. Aspects of databases most likely to provide useful information on toxic exposures are described.

Penile fracture: diagnosis and management.

Corpus cavernosum rupture or fracture of the penis is an unusual injury caused by blunt trauma. There are few cases reported in the literature and controversy regarding initial conservative versus operative management exists. This experience adds 17 cases and suggests that optimal management be determined by demonstrating the presence or absence of a tunica albuginea rent on corpus cavernosography. If no rent is demonstrated conservative management is recommended. If a rent is present operative management is the treatment of choice.

Retrograde amnesia effects of intravenous diazepam in endoscopy patients.

The presence of a retrograde amnesia, when caused by medications given prior to a medical procedure, can pose the medicolegal question of informed consent. This study investigated the retrograde amnesic effects of intravenous diazepam administered prior to gastrointestinal endoscopy. No significant retrograde amnesia was found in case subjects when compared with controls.

Maintenance of Glossina morsitans morsitans on antiserum to procyclic trypanosomes reduces infection rates with homologous and heterologous Trypanosoma congolense stocks.

Three experimental groups of male Glossina morsitans morsitans were infected at their first feed with procyclic forms of different stocks of Trypanosoma congolense and subsequently maintained on a diet containing rabbit antiserum to one of these stocks. Control flies were similarly infected but were then maintained on normal rabbit serum. Dissection of the flies 19-21 days post infection showed a reduction in both immature and mature infection rates in all groups fed on antiserum by comparison with control flies. These results suggest that vaccination with a single procyclic T. congolense strain could reduce transmission of both homologous and heterologous T. congolense strains which might, in certain epidemiological circumstances, provide an alternative or additional control method for African trypanosomiases.

A new method for the cytochemical staining of cells immobilized in agarose.

A quick, simple and inexpensive technique utilizing an isotonic matrix of semi-solid agarose for immobilizing channel catfish (Ictalurus punctatus) peripheral blood leucocytes onto microscope slides before cytochemical staining is described. The technique was demonstrated to be superior to the more conventional means of immobilizing cells for procedures requiring either fixed (Sudan Black B and nonspecific esterase stains) or viable (Nitro-Blue Tetrazolium stain) cells. Although the agarose immobilization technique was developed with channel catfish leucocytes, its utility with other fragile cell populations seems likely.
