The physical growth of Japanese children from birth to 18 years of age. Cross-sectional percentile growth curve for height and weight.

The purpose of this paper is to set forth the standards of weight and height for the Japanese children and to discuss the mathematical methods utilized for the study. In 1980, Japanese Ministry of Health and Welfare and Ministry of Education conducted a nationwide survey of weight and height in a total of some 680,000 children from birth to 18 years of age. These data were utilized for the present study. In order to draw a mathematically designed smooth curve, we divided the subjects into 4 age groups and expressed each percentile curve of the age range in terms of a polydimensional and polynominal function using the least square method. In comparison with the presently available eye-fit cross-sectional percentile growth curve, our growth curve appears to better simulate physical growth of the contemporary Japanese children.

Left ventricular outflow tract obstruction due to mitral annulus calcification in patient with mild concentric left ventricular hypertrophy.

The combination of the systolic anterior motion of the anterior leaflet of the mitral valve and/or the mitral annular calcification in the case of asymmetrical septal hypertrophy has been fully recognized. However, in concentric left ventricular hypertrophy the systolic anterior motion of the anterior mitral valve and the massive posterior submitral calcification have not been commonly reported. We present a case with mild concentric left ventricular hypertrophy and massive posterior submitral calcification which displace the entire mitral ring anteriorly, namely, toward the left ventricular outflow tract. In this case, typical left ventricular outflow obstruction with systolic anterior motion of the anterior mitral valve was seen. Thus, we considered that this rare condition may have contributed to the formation of the systolic anterior motion of the anterior mitral leaflet in this case. We have provided additional information regarding the possible causes of systolic outflow obstruction in hypertrophic obstructive cardiomyopathy.

Photoreceptor outer segments in the aqueous humor in rhegmatogenous retinal detachment.

We treated seven patients (all male, ranging in age from 11 to 31 years) with Schwartz syndrome, a combination of uveitis, high intraocular pressure, and retinal detachment with peripheral tears. The condition was resistant to corticosteroids but responded well to surgery. Puncture of the anterior chamber disclosed many outer segments of the visual cells with few inflammatory cells in the aqueous humor in all seven patients. Possibly the outer segments, flowing from the subretinal space through the peripheral retinal break into the aqueous humor, cover the trabecular meshwork and cause the decrease in the coefficient of outflow. Detection of the outer segments led us to make a definite diagnosis and institute the appropriate treatment.

Immune complex containing herpesvirus antigen in a patient with acute retinal necrosis.

A 21-year-old man with acute retinal necrosis showed a marked increase in the convalescent titer to herpes simplex type 1 virus, especially in the aqueous humor obtained by anterior chamber paracentesis. Using an enzyme-linked immunosorbent assay, we tried to detect the herpesvirus antigen in the circulating immune complex obtained from this patient. The immune complex contained an antigen or antigens that reacted with antiherpes simplex type 1 antibody.

Effects of new neuroleptics, isofloxythepin and zotepine, on post-decapitation convulsions and prolactin secretion in rats.

Oral administration of isofloxythepin (0.5-8.0 mg/kg) inhibited decapitation convulsions in a dose-dependent manner as shown by decreasing the incidence and shortening the convulsion's duration, the effects continuing until 24 hr after administration. Zotepine (8, 16 mg/kg, SC) also decreased the duration but not the incidence of the convulsion. However, the other neuroleptics such as haloperidol, spiperone, perphenazine, trifluoperazine, pimozide and sulpiride failed to affect decapitation convulsions. Reserpine (8 mg/kg, SC) inhibited decapitation convulsions, accompanied by decreasing levels of norepinephrine, dopamine and serotonin in the spinal cord. Isofloxythepin at doses of 4 and 8 mg/kg, PO which completely abolished decapitation convulsions, failed to change norepinephrine and dopamine levels but increased serotonin levels in the spinal cord. Isofloxythepin (0.5-8.0 mg/kg) increased serum prolactin levels in a dose-dependent manner and zotepine (16 mg/kg), haloperidol (1 mg/kg) and reserpine (8 mg/kg) also elevated the levels. The results imply that both isofloxythepin and zotepine, but not the other neuroleptics, inhibit convulsions without decreasing spinal levels of norepinephrine which appears to be the amine most directly involved in the occurrence of decapitation convulsions, although these neuroleptics enhance prolactin secretion by blocking dopamine receptors in the pituitary.

Endomyocardial biopsy findings in pediatric patients with post myocarditic state.

Endomyocardial biopsy was performed on eight children 5 to 12 years old, who were in post myocarditic state. They were evaluated within 2 to 25 months (mean lyr and 1 m) after the onset of the symptoms. Two of the patients developed heart failure and six patients developed other cardiac manifestations such as syncope, palpitation or ECG abnormalities at onset. Definite elevation of viral antibody titer was observed in four patients. Radionuclide angiography was also performed in all eight patients. An abnormal perfusion area was observed in six patients as a focal hypoperfusion area by T1-201 myocardial imaging. Ejection fraction was examined by Tc-99m-HSA gated equilibrium ventriculography. LVEF was reduced in 3 patients and RVEF was reduced in 2 patients. Judging from the histopathological findings, these patients were divided into three categories: chronic or smoldering myocarditis (3 patients); healing or healed myocarditis (4 patients); and post myocarditic hypertrophy (1 patient). Measurement of left ventricular function was obtained by cardiac catheterization and left ventriculography (LVG), which revealed some abnormal findings such as increased left ventricular mass index, increased left ventricular end diastolic volume index (LVEDVI) and reduced left ventricular ejection fraction (LVEF). Therefore, endomyocardial biopsy findings of pediatric patients in a post myocarditic state reveal certain histopathological abnormalities even in the long-term follow-up period in the absence of cardiac dysfunction.

Immunological studies of uveitis. 1. Immune complex containing herpes virus antigens in four patients with acute retinal necrosis syndrome.

Four patients with acute retinal necrosis syndrome were examined. Three of them showed elevated antibody titers to varicella-zoster virus during the convalescent stage, while one of them showed an elevated titer to herpes simplex virus type 1, especially in the intraocular fluid. Also, these patients had an elevated circulating immune complex in the acute phase. Western blotting demonstrated that the immune complexes contained varicella-zoster or herpes simplex type 1 antigen, corresponding to the elevated antibody titer to either virus.

Immunological studies of uveitis. 2. Immune complex containing retinal S antigen in patient with chronic intractable uveitis.

A case of chronic uveitis resistant to steroid treatment was reported where plasma exchange therapy, in contrast, had a remarkable effect on reducing the cystoid macular edema and retinal phlebitis. The removed plasma showed an elevated level of immune complex, and Western blotting demonstrated that this immune complex in the plasma contained retinal S antigen.

Immunological studies of uveitis. 3. Cell-mediated immunity to interphotoreceptor retinoid-binding protein.

Three patients were seen with the same clinical characteristics which were not described previously: diffuse uveitis with bilateral involvement, onset in the second decade of life, no extraocular signs or symptoms, diffuse retinal capillary leakage on fluorescein angiography but without apparent fundus changes, many vitreous cells but no snowbank-like vitreous opacity, responsiveness to steroid, and good prognosis. The peripheral blood lymphocytes of all three patients responded to bovine and human interphotoreceptor retinoid-binding protein (IRBP), but not to bovine retinal S antigen. It was speculated that these patients were human counterparts of IRBP-induced uveoretinitis that was described in experimental animals.

Improvement in the percutaneous absorption of beclomethasone dipropionate by gamma-cyclodextrin complexation.

Inclusion complex formation of beclomethasone dipropionate (BMDP) with gamma-cyclodextrin (gamma-CyD) in water and in solid state was assessed by solubility analysis and X-ray diffractometry. The solid complex of BMDP with gamma-CyD in a molar ratio of 1:2 was prepared and its in-vitro release behaviour was investigated using an ointment release simulator. The release of BMDP from hydrophilic ointment was significantly improved by the gamma-CyD complexation. Permeation and uptake studies indicated that the enhanced release of BMDP from the ointment may be due to the faster dissolution and the lower binding affinity of the complex in the ointment base. The vasoconstrictor activity of BMDP in man was found to be increased by gamma-CyD complexation, suggesting an improvement in the percutaneous absorption of BMDP.

Hepatocellular carcinoma detected by iodized oil. Use of anticancer agents.

Transcatheter arterial embolization (TAE) was performed in 97 patients with hepatocellular carcinoma. Ethiodol (iodized oil) containing an anticancer drug was infused via the hepatic artery followed by Gelfoam particles. The Ethiodol emulsion was selectively retained in the tumor vessels and also remained in the small daughter nodules that could not be detected by angiography or computed tomography (CT) prior to TAE. Ethiodol remained in the tumor for more than 1 year. Following TAE, main tumors and small daughter nodules appeared as areas of markedly high density on CT. In most patients there was a reduction in the tumor size following TAE, and serum alpha-fetoprotein levels were reduced in all patients whose initial levels had exceeded 400 ng/ml. This method is considered to be effective not only for treatment of hepatic tumor but also useful for evaluation of post-TAE changes in the tumor and diagnosis of small daughter nodules, due to the long-term accumulation of Ethiodol in tumor vessels.

Radionuclide imaging for assessment of myocarditis and postmyocarditic state in infant and children. Thallium-201 myocardial imaging and technetium-99m-HSA gated equilibrium ventriculography.

T1-201 myocardial imaging and Tc-99m-HSA gated equilibrium ventriculography were performed in 1 infant and 11 children between 2 months old and 12 years old with myocarditis. The time of first evaluation was between 1 week and 13 months after the onset. Their clinical manifestations were congestive heart failure in 2 patients, pericardial effusion in 2 patients, syncope in 1 patient, convulsions in 1 patient and palpitations in 7 patients. Significant elevation of virus antibody titer was demonstrated in 6 patients. A perfusion defect was observed with T1-201 imaging in 9 of 12 patients with myocarditis. A right ventricular image was observed in 5 patients. A reduced LVEF was present in 4 patients and a reduced RVEF was found in 2 patients. Cardiac catheterization was performed in 8 patients and an endomyocardial biopsy was done in 7. The myocardial specimens revealed postmyocarditic pathological changes upon microscopic evaluation. No coronary artery obstructive or stenotic lesions were observed in these patients. T1-201 myocardial imaging is a useful non-invasive technique for evaluating the cardiac performance and myocardial damage in acute myocarditis and the postmyocarditic state. We conclude that T1-201 uptake of myocardium is dependent upon both regional coronary perfusion and the activity of myocardial cells in patients with myocarditis and in the postmyocarditic state.
