Similar proportion of sporadic cases in cytochrome b558 negative chronic granulomatous disease and Duchenne muscular dystrophy.

We studied 12 Japanese families of cytochrome b558 (b) negative chronic granulomatous disease (13 patients and 23 family members) and 15 controls for cytochrome b content, O2-, and H2O2 production. Cytochrome b content (nmol/10(8) cells), O2- production (nmol/10(7) cells/min), and percentage of H2O2 generating cells (%) were 1) 0.45-1.19, 64.1-174.2, and 85.8-100.0 in controls (mean +/- 2 S.D.), 2) 0.13-0.38, 22.6-50.9, and 20.0-62.4 in healthy carriers, and 3) undetectable, undetectable, and 0 in patients, respectively. These findings indicate that healthy carriers and normal homozygotes are separable by these three parameters, and that 4 of the 12 families studied represent fresh gene mutation. Pooling of data by Segal et al., Ohno et al., and ours yielded the overall incidence of a fresh gene mutation to be 19.4% (7 of 36 cases). The lower fresh mutation rate than predicted from Haldane's formula suggests a higher mutation rate in males than in females, as previously suggested in Duchenne muscular dystrophy.

Regulation of testicular differentiation and testosterone production in the fetal mouse gonad in vitro.

The objective of the present study was to develop a chemically-defined medium in which early stages of testicular differentiation can be investigated in an organ culture system. Mouse gonadal primordia were explanted before and after initiation of morphological sex differentiation, i.e. 11 and 12 day of gestation (d.g.), respectively. We found that a combination of human albumin fraction, insulin (or IGF-I), and sodium pyruvate promoted testicular organization of gonadal explants of 11 d.g., but not those of 12 d.g. Insulin also increased the production of testosterone from testicular explants of 11 d.g., but not those of 12 d.g. For the younger explants, progesterone was more efficient than pregnenolone as a steroid precursor during the first day of culture, but the maximum effect of pregnenolone was much higher than that of progesterone in later stages. The responsiveness to human chorionic gonadotropin increased gradually along with testicular organization. The addition of either serum or pregnenolone prominently increased the activity of delta 5-3 beta hydroxysteroid dehydrogenase in testicular explants of 11 d.g., but not the number of positive cells as demonstrated by histochemical staining. These results suggest that insulin (or IGF-I) is required during the initial phase of testicular organization, which is reflected by an increase in testosterone production and sensitivity to gonadotropins.

Factors associated with poor visual outcome in acute retinal necrosis.

We reviewed 22 consecutive patients (26 eyes) with acute retinal necrosis to elucidate whether there was any sign at an early stage to predict poor visual outcome. Final visual acuity of the patients fell into two groups, one worse than 20/600 (15 eyes) and the other better than 20/30 (11 eyes). The patients with poor visual outcome showed diffuse retinal arteritis or arterial obliteration which was associated with reduced amplitude of electroretinographic a and b waves and elevated levels of circulating immune complex, even at an early stage when retinal exudates were still localised to the peripheral fundus. The retinal exudates in these patients extended rapidly to the posterior pole. In contrast, the patients with good visual outcome showed retinal arteritis limited within retinal exudates throughout the course. These signs at an early stage can be used as prognostic factors to manage patients with acute retinal necrosis more constructively.

HLA-DR9 associated with the severity of acute retinal necrosis syndrome.

We divided 22 patients with acute retinal necrosis syndrome into two groups depending on whether they had its mild (10 patients) or fulminant type (12 patients) and analyzed human leukocyte antigens, HLA-A, -B, -C and -DR loci, to elucidate immunogenetic differences between these two groups. The frequency of HLA-DR9 was found to be high in patients with the fulminant type (50.0%) compared with that in the patients with the mild type (0.0%), which was significant (p less than 0.05) in a chi 2 test with Yates' correction but did not remain significant when the probability was corrected by the number of antigens tested. The result suggests that immunogenetic predisposition plays a role in aggravation of acute retinal necrosis syndrome.

Oral prostaglandin E1 derivative (OP-1206) in an infant with double outlet right ventricle and pulmonary stenosis. Effect on ductus-dependent pulmonary circulation.

A small-for-gestational-age infant with cyanosis due to double outlet right ventricle with severe pulmonary stenosis and patent ductus arteriosus was treated with oral prostaglandin E1 derivative (OP-1206). The constricting ductus arteriosus dilated and the ductus-dependent pulmonary blood flow increased. The recommended dosage was 1.5-2.0 micrograms/kg/day which was lower than that of intravenous PGE1 or of oral PGE2. The administration interval was 6 hours, which was longer than that of oral PGE2. The patient was treated as an out-patient because continuous intravenous infusion was not necessary. Treatment was continued for 2 months without complication, at which time a Blalock-Taussig shunt operation was performed. Orally administered PGE1 derivative (OP-1206) was found to be equally effective to intravenous infusion of PGE1 for both short and long-term management of cyanotic heart disease in which the pulmonary blood flow is mostly dependent on the patency of the ductus arteriosus. Oral PGE1 derivative (OP-1206) may be a possible substitute for intravenous PGE1 infusion therapy.

[A case of sarcoid granulomatous interstitial nephritis improved by steroid therapy].

We report a case of sarcoid granulomatous tubulointerstitial nephritis diagnosed by renal biopsy. A 60-year-old man presented with productive cough, and exertional dyspnea of 3 months duration. A chest X-ray film revealed diffuse reticulonodular infiltrates in both lung fields. A transbronchial lung biopsy specimen showed inflammation of the alveolar septum associated with non-caseating granulomas. The patient also had tubular proteinuria and glucosuria. Ga-scintigraphy demonstrated an abnormal accumulation of gallium in both lungs and kidneys. Renal function tests revealed tubular dysfunction. Tubulointerstitial nephropathy was suspected. A renal biopsy specimen exhibited tubulointerstitial nephritis associated with numerous non-caseating granulomas, similar to the findings of the lung biopsy specimen. No glomerular abnormalities were evident. Later, a scalene node biopsy confirmed the diagnosis of sarcoidosis. Prednisolone therapy yielded a favorable outcome for both the renal and pulmonary involvement. During the corticosteroid therapy, measurement of the urinary beta-2-microglobulin concentration proved a valuable monitoring tool for assessing the recovery of the tubular impairment.

Two cases of hemangioma of the upper lip in infants--treatment using the Nd: YAG laser.

We report here two cases of treatment of upper lip hemangioma in infants, in which the Nd:YAG laser had a significant therapeutic effect. Case 1 was a 3-month-old male with swelling of the upper lip as the chief complaint. After galvano-caustic was performed under general anesthesia, sclerosing agent injection therapy (6 times, total 4.5 ml) was continued for 2 months. But the tumor was still growing about 2 years later, so resection was performed with Nd:YAG laser under general anesthesia. Case 2 was a 1-month-old male, also with swelling of the upper lip as the chief complaint. Resection was performed with Nd:YAG laser under general anesthesia. But the tumor was still growing about 4 months later, so it was repeated. The patients made satisfactory prognosis. Histologically, the final diagnoses of two cases were capillary hemangioma.

[Scanning electron microscopic observation of the collagen fibrillar network in Bruch's membrane by cell maceration].

Cellular elements, including retinal pigment epithelia, were removed by the cell maceration method to obtain a three-dimensional view of the collagen fibrillar networks of Bruch's membrane. A bird's eye view of the inner collagenous zone of Bruch's membrane was obtained using the eyes of albino rabbits and Japanese monkeys. The eyes were treated with a low concentration of NaOH for several days and then observed with a scanning electron microscope. This procedure allowed the exposed inner collagenous zone of Bruch's membrane to be observed. In addition, the outer collagenous zone was also be observed in some of the specimens. Both zones were formed by dense lattices of fine collagen fibrils, and these fibrils frequently formed bundles in the outer collagenous zone. In the posterior poles of the eyes obtained from Japanese monkeys, a network of wavy fine collagen fibrils was observed. In both outer and inner collagenous zones, most of the collagen fibrils ran parallel to the retina. Fenestration of the sheet-like elastic layer was frequently observed through gaps in the thin collagen fibrillar network of the inner collagenous zone. The method employed in this study was considered suitable to obtain a wide bird's eye view of the collagenous zone of Bruch's membrane.

[Three-dimensional observation of elastic tissue in Bruch's membrane by formic acid extraction].

Japanese monkey and albino rabbit choroid were treated with hot-formic acid to remove the cellular elements and the extracellular matrices excluding the elastic tissues so that both three-dimensional and bird's eye views of the elastic layer of Bruch's membrane could be obtained. The only elastic layer was extracted and observed with a scanning electron microscope. Elastic layers were frequently extracted as translucent monolayered sheets with oval, sector-shaped fenestrations of varying sizes. Elastic fibers of 100-200nm in diameter crossed each other in these fenestrations. Other fine fibers connecting these elastic fibers and their sheet-like elastic plate were also observed. In Japanese monkey eyes, lattice structures formed by thick elastic fibers, 200-300nm in diameter, were conspicuous. This method proved suitable for the observation of the three-dimensional structure of the elastic layer of Bruch's membrane.

[A case of lacrimal gland malignant lymphoma, associated with Sjögren's syndrome].

A 64 year-old female complained of left upper lid swelling. Painless mass lesions were found in her bilateral lacrimal gland areas. She also complained of a dry sensation in both eyes and her mouth. The rose bengal test, Schirmer's test and fluorescein dye test were positive. Lip biopsy revealed chronic sialoadenitis and rheumatoid factor titer was elevated. Anti-nuclear and anti-RNP antibodies were positive. Anti-SS-A and anti-SS-B antibodies were both negative. Left pre-and post auricular, cervical and inguinal lymph nodes were palpable. Excisional biopsies of bilateral orbital lesions and an inguinal lymph node revealed the presence of reactive lymphoid hyperplasia in the right orbit, and B cell type, diffuse, large cell lymphoma, in the left orbit and inguinal lymph node. Intensive chemotherapy resulted in complete remission. Lacrimal gland lymphoma must be considered when examining cases of Sjogren's syndrome.

Toxicity modification of sodium selenite by a brief exposure to heat or cold in mice.

The effect of a brief exposure to heat/cold on the subsequent development of toxicity of sodium selenite (SS) was evaluated in male ICR mice. Mice were exposed to one of three ambient temperatures (Ta; approx. 8 degrees, 22 degrees and 33 degrees C). One and a half hours after the beginning of the exposure, they were injected with 45 mumol/kg of SS subcutaneously. The exposure was terminated 3 h after injection and the mice were returned to Ta of 22 degrees C. Heat-induced enhancement of toxicity was recognized in some plasma enzyme activities 3 days after injection and in the suppression of body weight for up to 3 weeks. On the other hand, cold exposure alleviated SS toxicity in terms of these indices. Thus, the Ta during this short period was recognized to be important in determining subsequent development of SS toxicity. At the end of the thermal exposure, heat increased renal concentration of the injected Se. On the other hand, in the liver and the other organs examined, the highest Se concentration was found in the cold-exposed group, followed by the control (room temperature) and the heat-exposed. The relation between the modification of toxicity and the altered distribution was not clear. Neither glutathione level in the liver nor that in the kidney at the time of SS injection could explain the observed modification of the toxicity.

[The clinical significance of serum sialyl SSEA-1 antigen in obstetrical and gynecological patients].

To evaluate the clinical significance of serum SSEA-1 level, the antigen level was measured in sera of obstetric and gynecologic patients. A positive rate was low in patients with endometriosis (9.1%), myoma uteri (0%), benign ovarian tumor (15%), cervical squamous cell carcinoma (14%) and endometrial carcinoma (18%). In this series of study, interest was that positive cases in benign ovarian tumor group were all patients with dermoid cyst. On the other hand, a high positive rate was observed among the patients with cervical adenocarcinoma (50%), primary ovarian malignancies (55%) and secondary ovarian malignancies (50%). Among the patients with ovarian malignancies, serum sialyl SSEA-1 level significantly increased according to clinical stage. In patients with positive serum sialyl SSEA-1, rising or falling of the serum level of this antigen correlated well with progression or regression of the disease. Measurement of serum CA125 was also performed in patients with ovarian malignancies, which showed a significantly higher positive rate (96%) and revealed that this antigen has no correlation with sialyl SSEA-1. A low positive rate of serum sialyl SSEA-1 level (9.1%) was observed in gravidas, while a higher positive rate (43%) in puerperas, especially within three days after parturition. This evidence should be considered when serum sialyl SSEA-1 antigen is measured as tumor marker. All these observations suggest that the measurement of serum sialyl SSEA-1 level is useful not only in the diagnosis of ovarian malignancies but for the judgment of the effect of treatment and the search for their recurrences.
