Mucosal IgA antibody against Helicobacter pylori in chronic gastritis and intestinal metaplasia detected by the Tes-Tape method in resection specimens after gastrectomy for gastric cancer.

BACKGROUND: Subjects with atrophic body gastritis have a high prevalence of Helicobacter pylori seropositivity and a low prevalence of H. pylori infection. Disappearance of the organism appears to correlate with the development of intestinal metaplasia. To investigate this point, intestinal metaplasia was biochemically subclassified into complete and incomplete types by the Tes-Tape method, and tissue IgA and IgG antibodies against H. pylori were measured by enzyme-linked immunosorbent assay (ELISA). METHODS: Twenty-five stomachs resected for gastric cancer were examined using the Tes-Tape method. Serum H. pylori IgA and IgG antibodies and tissue IgA and IgG antibodies against H. pylori and tissue secretory IgA (sc-IgA) were examined in areas of intestinal metaplasia, nonmetaplastic gastric mucosa, and gastric carcinoma by ELISA: RESULTS: Tissue H. pylori IgA antibody was positive in 6 of 19 (32%) specimens taken from complete and 2 of 7 (29%) incomplete types of intestinal metaplasia and was positive in 6 of 14 (43%) nonmetaplastic gastric mucosa from the antrum and 14 of 23 (61%) from the body. Duodenal mucosa and cancer tissue were positive for tissue IgA antibody in 1 of 6 (17%) and 0 of 17 (0%), respectively. Tissue H. pylori IgG antibody was negative in all the tissues examined. sc-IgA in the areas of intestinal metaplasia was 120 +/- 65 (mean +/- standard error; ng/mg wet weight) and in the nonmetaplastic gastric mucosa was 113 +/- 72, showing no difference. Positivity and negativity of serum IgA and IgG antibodies against H. pylori coincided with presence or absence of tissue IgA antibody in nonmetaplastic gastric mucosa in 15 of 19 (79%) and 16 of 19 (84%) patients examined, respectively. CONCLUSION: Positivity rates of tissue IgA antibody against H. pylori were lower in the mucosa of intestinal metaplasia than in nonmetaplastic gastric mucosa and were negative in carcinoma. No significant difference in levels of sc-IgA between intestinal metaplasia and non-metaplastic gastric mucosa was found.

Mechanisms of reoxygenation-induced calcium overload in cardiac myocytes: dependence on pHi.

This study investigated the selective effects of intracellular (pHi) or extracellular change in pH on reoxygenation-induced Ca2+ overload in simulated myocardial hypoxia. Experiments were performed in cultured cardiomyocytes isolated from the ventricle of neonatal ICR mouse. A model of chemical hypoxia with 2 mM sodium cyanide was developed to mimic the ATP depletion of hypoxia. This chemical hypoxia was "reoxygenated" and the dynamics in intracellular Ca2+ concentration ([Ca2+]i) and pHi were monitored using the fluorescent dyes fura-2 and 2', 7'-bis (2-carboxyethyl)-5(6)-carboxyfluorescein, respectively. During a 40-min chemical hypoxia, pHi progressively fell from 7.2 to 6.6. Reoxygenation with control solution caused rapid recovery of pHi and a marked increase in [Ca2+]i (1884 +/- 136 nM). Intracellular acidotic reoxygenation produced by lactate apparently prolonged the time course of pHi recovery and significantly suppressed reoxygenation-induced Ca2+ overload (1170 +/- 118 nM, P = 0.008). Extracellular acidotic reoxygenation with 2-(N-morpholino) ethanesulfonic acid (pK = 5.96) buffer somewhat suppressed the Ca2+ overload; however, the maximal value of [Ca2+]i was not reduced significantly compared with the control (1790 +/- 122 nM, P = 0.130). In addition, inhibition of Na(+)-H+ exchange by amiloride potentiated prolongation of intracellular acidosis during reoxygenation and resulted in a minimal increase in [Ca2+]i (985 +/- 102 nM, P = 0.004). These results suggest that reoxygenation-induced Ca2+ overload is closely correlated with intracellular pH in the initial phase of reoxygenation, and the protective effects of extracellular acidosis is principally mediated by intracellular acidification of reoxygenated cardiomyocytes.

Cytochrome P4502E1 (CYP2E1) genetic polymorphism in a case-control study of gastric cancer and liver disease.

Cytochrome P4502E1 (CYP2E1) activates carcinogenic N-nitrosamines, benzene, urethane and other low molecular weight compounds. This enzyme is also inducible by ethanol, and metabolizes alcohol. A restriction fragment length polymorphism (RFLP) using the Rsa I restriction enzyme has been identified in the CYP2E1 transcription regulatory region; recent studies suggest that this polymorphism may affect gene expression. We investigated the frequency of the Rsa I RFLP in a Japanese population in relation to gastric cancer and liver disease susceptibility. The frequency of this polymorphism was determined in 150 gastric cancer, 16 hepatocellular cancer, 48 liver cirrhosis and 203 benign gastric disease (controls) patients. This preliminary study shows no association of the specific genotype with gastric cancer in all subjects (odds ratio = 1.04, 95% CI = 0.74-3.08 for the heterozygote and 0.57, 95% CI = 0.22-1.50 for the homozygous rare allele, respectively). To further confirm this lack of association, an age and gender matched case-control study should be performed. Separately, there was no association of the Rsa I RFLP with hepatocellular carcinoma (p = 0.911), but there was a suggested difference between the non-viral associated liver cirrhosis patients and control patients. Thus, this polymorphism may be related to ethanol metabolism and consequential liver diseases in a Japanese population.

[Endotoxin-induced lung injury. The roles of leukocytes and oxidants, and the efficacies of steroids and antioxidants].

Previous work in our laboratory has shown the relation between leukocytes (WBC) and the generation of oxidants in endotoxin (LPS) shock. The purpose of this study was to find out if WBC-derived oxidants can produce acute lung injury in guinea pigs given LPS. We alos evaluated the efficacies of steroids and antioxidants against the initial changes in LPS-induced lung injury. One group of guinea pigs (200-250 g, male) received 0.7 mg/kg (LD50, 24 hrs.) of E. coli LPS in the peritoneal cavity (group I). The animals in group II received 30 mg/kg of methylprednisolone (MP), followed by intraperitoneal LPS. The animals in group III were given 30 mg/kg of 2-aminomethyl-4-tert-butyl-6-propionylphenol hydrochloride (ONO-3144), a known as antioxidant (OH radical scavenger), and then an injection of LPS. The animals were killed at following time course: 30, 60 or 180 minutes after the LPS injection. Hematological examinations (WBC counts), total cell counts and differential counts in bronchoalveolar lavage (BAL) fluid were done along with light microscopic studies. Superoxide dismutase (SOD) activity, catalase activity and malonaldehyde (MDA) produced as a result of lipid peroxidation in the lung were measured and correlated with histological changes. Survival ratios of the three groups were compared. The results obtained were: 1) Significant leukopenia occurred in all groups. 2) In group I, WBC, especially eosinophils, were recovered by BAL and the total cell number of the BAL fluid had increased by 180 minutes after LPS injection, but MP or ONO-3144 treatment inhibited the migration of WBC (eosinophils and neutrophils) into alveolar lumen after LPS injection. 3) Histologic examinations revealed diffuse edema, hemorrhage, and marked leukocyte infiltration in the alveoli in group I, but not in group II or III. 4) SOD activity in all group diminished below the control level. Catalase activity had significantly increased by 180 minutes after LPS injection in group I, but not in group II or III. MDA had increased remarkably by 60 minutes after injection of LPS in group I, but MP or ONO-3144 treatment prevented such increases. 5) Animals in group II and III survived significantly longer than those in the other group. In conclusion, these findings suggest that LPS provokes WBC-mediated vascular damage in the lung and steroids or antioxidants can minimize the injury and prevent edema formation. Steroids might be useful for achieving quantifiable changes in LPS-induced WBC chemotaxis to the lung and for decreasing oxidant-induced lung injury.

[Intermittent mini-dose vancomycin intravenous administration and closed continuous irrigation technique applied to mediastinitis caused by MRSA following mitral valve replacement].

Mediastinitis caused by MRSA (Methicillin-Resistant Staphylococcus aureus) remains an intractable infection producing high mortality even in these days of advanced chemotherapy. The authors report a case of mediastinitis due to MRSA complicated with acute renal failure following mitral valve replacement. The patient's mediastinum had been thoroughly cleaned with physiological saline solution with 0.2% povidone iodine, and underwent a chemotherapy regimen of mini-dose vancomycin. The patient made favorable progress and recovered completely. Our patient's progress confirmed that when chemotherapy using vancomycin is administered in a patient whose condition is complicated with acute renal failure, closely monitoring the vancomycin serum concentration is essential. Intermittent mini-dose intravenous administration is sufficient to maintain an effective vancomycin serum concentration. In our case, vancomycin serum concentration measured before and at completion of dialysis revealed no appreciable decline.

Lysophosphatidylcholine, a component of stromal phospholipids, as a candidate vasoconstrictive factor in stroma-free hemoglobin.

Stroma-free hemoglobin (SFH) contains a trace amount of phospholipids, which has been implicated in the toxic reactions associated with SFH. We analyzed stromal phospholipids by high-performance liquid chromatography and found that SFH contained small quantities of lysophosphatidylcholine (LPC), which is known to be capable of producing a defect in endothelium-dependent arterial relaxation, in addition to major classes of constituent phospholipids in red cell membrane. LPC content was determined to be 1.65 nmol/ml (hemoglobin 8.1 g/dl). To evaluate the role of these stromal phospholipids in SFH-induced vasoconstriction, we next examined the effect of lipids on vascular tone in rabbit aortic strips. Preincubation with the crude lipid extract or the LPC purified from SFH significantly inhibited acetylcholine-induced relaxation in phenylephrine-precontracted tissues. The LPC-induced inhibition was reversed by incubation of the tissues in the absence of lipids, indicating the functional integrity of endothelium. From these results, we propose a possibility that LPC, a component of stromal phospholipids, is a candidate for vasoconstrictive factors present in SFH.

Effects of Ca channel agonists on 45Ca uptake differ depending on the state of NG108-15 cells.

We investigated the effects of various Ca channel agonists (Ca agonist) derived from 1,4-dihydropyridine on KCl-stimulated 45Ca uptake by differentiated and undifferentiated neuroblastoma x glioma hybrid NG108-15 cells with and without dibutyryl cAMP. Ca agonists Bay K 8644, YC-170 and CGP 28392 enhanced KCl-stimulated 45Ca uptake in differentiated NG108-15 cells, but only slightly in undifferentiated NG108-15 cells. The rank order of the enhancing effects was roughly Bay K 8644 > YC-170 > > CGP 28392. These results suggest that there is some difference between the mechanism by which these Ca agonists affect KCl-stimulated 45Ca uptake in differentiated and undifferentiated NG108-15 cells, although the nature of that difference is not clear.

Acellular and cellular hemoglobin solutions as vasoconstrictive factor.

The inhibitory effects of acellular and cellular hemoglobin (Hb) solutions on endothelium-dependent vasorelaxation were investigated in rabbit thoracic aortic strips. As acellular Hb solutions, 2,3-diphosphoglycerate (DPG)-depleted Hb and pyridoxylated Hb were examined. Cellular Hb solutions included washed human fresh red cells and liposome Hb encapsulated with pyridoxal-5'-phosphate (PLP). The tissues were precontracted with phenylephrine (PE), after which acetylcholine (ACh) was added to elicit a steady-state relaxation. Acellular Hb solutions cumulatively reversed ACh-induced relaxation, and these inhibitory effects reached a plateau at 10 micrograms/ml. Increasing oxygen affinity by pyridoxylation had little effect on this. In contrast, both red cells and liposome Hb solution showed moderate inhibitory effects, and they reached a plateau at 1 mg/ml. These findings indicate that acellular Hb solutions are more potent inhibitors than cellular Hb solutions by a factor of about 100, and that the encapsulation of Hb is a preferable method to mimic the red cell.

The quality control of stroma-free hemoglobin: lysophosphatidylcholine, a component of stromal phospholipids, as candidate vasoconstrictive factor.

We characterized stromal phospholipids in stroma-free hemoglobin (SFH) by normal-phase and cation-exchange HPLCs, and found that SFH contained not only four phospholipids which were the major constituent classes in membrane, but also several peaks which were not yet identified. The residual amounts of these lipids in SFH were changed with storage of red cell concentrates. The four major phospholipids decreased concomitantly with storage, whereas the unidentified peaks increased after 21 days and then decreased after 48 days. We also found that SFH contained lysophosphatidylcholine (LPC) at 5.71 micrograms/ml, which was the deacylated metabolite of phosphatidylcholine (PC). These results suggest that stromal phospholipids are degradable. Since LPC is known to be capable of producing a defect in endothelium-dependent arterial relaxation, we next examined the effect of stromal lipids on vascular tone in rabbit thoracic aortic strips. Preincubation with the crude lipid extract or the LPC purified from SFH by TLC significantly inhibited acetylcholine (ACh)-induced relaxation in phenylephrine (PhE)-precontracted tissues. These observations have led to the proposal that LPC, a component of stromal phospholipids, induces vasoconstriction as a result of inhibition of endothelium-dependent vasorelaxation.

[A case of bronchogenic cyst at the anterior chest wall].

Bronchogenic cyst in the subcutaneous region is rare and only 5 cases including this case have been reported in Japan. A 2-year-old boy was admitted to our hospital because of congenital mass at the anterior chest wall. Ultrasonography showed a 29 x 9 mm sized well-defined tumor with homogeneous low echogenicity in the subcutaneous tissue. Extirpation of the tumor was performed. Histologically, ciliated columnar epithelia with the surrounding smooth muscle and seromucous glands were observed. Diagnosis of bronchogenic cyst was made. Postoperative course was uneventful and no tumor has recurred.

[A case of pulmonary malignant lymphoma associated with localized amyloid plaque].

A 51-year-old woman admitted to our hospital. Chest X-ray film showed infiltrations in left S4 region, which had been pointed out since two years before. On exploratory thoracotomy, lingual segmentectomy was performed because no metastatic lesions were found in the mediastinal and hilar lymph nodes. Pathological and immunohistological examinations revealed findings corresponding to pulmonary malignant lymphoma (diffuse, small cell type, B cell) with amyloid plaque. The patient is doing well 20 months after operation without any signs of recurrence. The clinical and histological findings of this case may suggest a close relationship between malignant lymphoma and production of amyloid.

[Evaluation of physicians' decision making processes in primary care medicine].

To investigate how primary care physicians select appropriate diagnostic tests in their clinical practice, we analyzed the physician's decision making processes by means of written case simulations. We allocated the physicians who participated in this study to three groups; nine residents and ten attending physicians from the Department of Primary Care Medicine, and nine specialists from the Department of Medicine in our hospital. After they read a history and findings in each presented case, the physicians were requested (1) to make diagnostic hypotheses and predict probability of each hypothesis, (2) to select diagnostic tests and their purpose, and (3) to estimate contribution of the tests to the diagnosis. The results revealed that the attending physicians tended to order the diagnostic tests more strictly based on their hypotheses than the residents and specialists did. The attending physicians selected diagnostic tests from specific varieties rather than from "Essential Laboratory Tests" when they were confident in their hypotheses. On the other hand, they selected the tests mainly from "Essential laboratory Tests" when they were uncertain of diagnosis or needed to exclude serious diseases. They regarded the contribution of tests to the diagnosis as high when specific diagnostic tests were available for the presumptive diagnosis, or severe conditions were expected. These findings suggest that, in the setting of primary care practice, the application of laboratory tests depends on the severity of presenting conditions and pre-test certainty of the diagnostic hypothesis. Therefore, it may not be always appropriate to apply "Essential Laboratory Tests" uniformly to every patient irrespective of his/her condition.

[A case of mediastinal emphysema due to blunt trauma that a continuous drainage to the extrapleural space took effect].

A 70-year-old male was admitted to our hospital with chest bruise, resulting in fracture of the right rib. Nine hours later, the patient came to use again with a chief complaint of dyspnea and chest oppression. He had extensive subcutaneous emphysema in the upper half of the body. Chest X-ray films revealed remarkable mediastinal emphysema and collapse in a part of right lung. Chest CT scan disclosed enlargement of extrapleural cavity. These findings suggested progression of mediastinal emphysema. Continuous drainage to the extrapleural cavity was performed and the clinical course was uneventful. Thus, a satisfactory result can be obtained by extrapleural drainage in a patient with severe mediastinal emphysema.

[A case of transitional cell carcinoma of the prostate].

A 46-year-old man was admitted to our hospital with the complaint of gross hematuria and renal failure. The patient had undergone right nephrectomy 10 years earlier suffering from nephrotuberculosis, and the remaining left side manifested severe hydronephrosis and vesicoureteral reflux (VUR). After the immediate nephrostomy, renal function was recovered and hematuria was improved preservatively. By the rectal examination, a goose egg size and stony hard prostate was detected. The needle biopsy of the prostate revealed pathologically transitional cell carcinoma (TCC) G2, and the image findings showed stage of T4N0M0 of the prostatic carcinoma. Modified pelvic exenteration was performed, and specimens were examined by the step section method. The diagnosis was TCC, G2 > G3 and stage of pT4N0 in the right lobe of the prostate. This case is she 20th reported case in Japan. We emphasize the importance of the mapping of dissected specimens and the reviewed literature.

[A case of intrathoracic goiter composed of two tumors].

A 70-year-old female was referred to our hospital because of swelling at her frontal neck and an abnormal shadow on a chest X-ray film. CT scan and MRI showed a 11.0 x 7.5 cm well-demarcated tumor in the upper mediastinum. Thyroidal scintigraphy revealed an slight accumulation, leading to a diagnosis of intrathoracic goiter. During operation the mass was found to be composed of two tumors. The upper tumor was attached with fibrous adhesion to the right lobe of cervical thyroid but no connection. Histological examination showed follicular but not malignant adenoma of the thyroid.

Characteristics of 45Ca uptake stimulated by high KCl of differentiated and undifferentiated NG108-15 and PC12h cells.

The characteristics of KCl-stimulated 45Ca uptake by neuroblastoma X glioma hybrid NG108-15 cells induced to differentiate with dibutyryl cAMP (Bt2cAMP) and of PC12h pheochromocytoma cells induced to differentiate with nerve growth factor (NGF) were studied. The extent and rate of KCl-stimulated 45Ca uptake by differentiated NG108-15 cells induced with Bt2cAMP were significantly higher than those of the undifferentiated cells. However, differentiation of PC12h cells induced with NGF did not enhance their extent or rate of KCl-stimulated 45Ca uptake. The effects of Ca agonist and antagonists indicated that the characteristics of KCl-stimulated 45Ca uptake by Bt2cAMP-treated NG108-15 cells and NGF-treated PC12h cells mainly reflected those of peripheral L-type voltage-sensitive calcium channels activated by high KCl. These results suggest that differentiated neural cells did not all show an enhanced capacity for KCl-stimulated 45Ca uptake, although the characteristic patterns of differentiation (extension of neurite-like processes, etc.) and that of effect by Ca agonist or antagonists on NG108-15 cells and PC12h cells were similar.

Specific bindings of [3H](+)PN200-110 and [125I]omega-conotoxin to crude membranes from differentiated NG108-15 cells.

The characteristics of the specific bindings of [3H](+)PN200-110 (PN: L-type Ca channel antagonist) and [125I]omega-conotoxin G VI A (omega-CgTX: neuronal L- or N-type Ca channel antagonist) to crude membranes from undifferentiated neuroblastoma X glioma hybrid NG108-15 (NG108-15) cells and differentiated cells induced with dibutyryl cAMP (Bt2cAMP) were examined, because we have already observed that the magnitude and rate of KCl-stimulated 45Ca uptake by NG108-15 cells increased progressively during differentiation of the cells induced with Bt2-cAMP (unpublished results). The specific binding of [3H](+)PN to these crude membranes was saturable at various concentrations of 2.5-5.0 nM [3H](+)PN. Scatchard analysis showed that the specific binding of [3H](+)PN at equilibrium was significantly increased after differentiation of the NG108-15 cells with Bt2cAMP, but that the apparent Kd value for the specific binding of [3H](+)PN was not influenced by treatment with Bt2cAMP. The specific binding of [3H](+)PN to crude membranes from Bt2cAMP-treated NG108-15 cells was inhibited by a calcium agonist and antagonists, the order of their inhibitory potencies being (+)PN > nitrendipine > (-)PN > or = Bay K 8644 > > diltiazem = verapamil. Thus, PNs showed significant stereoselective inhibition of the specific binding of [3H](+)PN. On the other hand, [125I]omega-CgTX at concentrations of 0.075-0.6 nM showed scarcely any specific binding to these crude membranes, although at 0.6 nM it showed specific binding to crude membranes from rat brain in the same experimental conditions.(ABSTRACT TRUNCATED AT 250 WORDS)

Evidence for up-regulation of epidermal growth-factor receptors on rat periodontal ligament fibroblastic cells associated with stabilization of phenotype in vitro.

This study sought to understand the role of epidermal growth factor receptor (EGF-R) in periodontal ligament (PDL) fibroblasts. Rat PDL fibroblastic cells and ROS 17/2.8 cells (highly differentiated osteoblastic osteosarcoma cells) were cultured and treated with transforming growth factor-alpha (TGF-alpha), EGF, dexamethasone (Dex) or a combination of EGF and Dex. Alkaline phosphatase (ALP) activity, an early differentiation marker for mineralized tissue-forming cells, was measured using p-nitrophenylphosphate as a substrate. For Scatchard analysis of [125I]-EGF binding, cells were incubated in Dulbecco's modified Eagle's medium containing 0.2% bovine serum albumin and 0-64 ng/ml of [125I]-EGF for 4 h at 4 degrees C. Also, the synthesis of EGF-R protein and the expression of mRNA for EGF-R were measured by immunoprecipitation and Northern blot analysis, respectively. Untreated PDL fibroblastic cells showed a gradual increase in spontaneous ALP activity from 32.4 U/10(6) cells at 2 days to 49.6 U/10(6) cells at 7 days of culture. ALP activity was further increased to 70.8 U/10(6) cells at 7 days after treatment with Dex, whereas EGF treatment reduced it to 19.4 U/10(6) cells. Culture of PDL fibroblastic cells in the presence of a combination of Dex and EGF decreased the Dex-induced ALP activity from 70.8 U to 41.8 U/10(6) cells at 7 days. A similar inhibitory effect on ALP activity was found after treatment with TGF-alpha. In contrast, ROS cells maintained a high ALP activity (1748 U/10(6) cells) throughout culture, unaffected by EGF. Scatchard analysis demonstrated that PDL fibroblastic cells have both high- and low-affinity forms of EGF-R, while ROS cells did not have any detectable EGF-R. Treatment of PDL cells with Dex for 2 days decreased the synthesis of EGF-R protein, the expression of EGF-R mRNA and the number of EGF-R. In contrast, EGF treatment increased the expression of EGF-R mRNA. These data suggest that PDL fibroblastic cells express numerous EGF-R, but the number decreases during their differentiation into mineralized tissue-forming cells under the influence of Dex. Thus, EGF-R may function in the stabilization of phenotype in PDL fibroblastic cells.
