[Detection of periodontal pathogenic microorganisms in atheromatous plaque. Preliminary results].

INTRODUCTION: Recent studies suggest that chronic infections, including those associated with periodontitis, increase the risk for coronary vascular disease. We hypothesize that oral microorganisms including periodontal bacterial pathogens enter the blood stream during transient bacteremias where they may play a role in the development and progression of atherosclerosis. MATERIALS AND METHODS: To test this hypothesis, 34 human specimens obtained during carotid endarterectomy or bypass procedures were examined by use of specific oligonucleotide primers for Porphyromonas gingivalis, Prevotella intermedia, Actinobacillus actinomycetemcomitans and Bacteroides forsythus in polymerase chain reaction (PCR) assays. RESULTS: Twenty (59%) of the 34 specimens tested positive for bacterial 16S rDNA. Subsequent hybridization of the bacterial 16S rDNA positive specimens with species-specific oligonucleotide probes revealed that 32.4% of the 34 atheromas tested positive for at least one of the target periodontal pathogens. Further analysis of the results in the bacterial positive group (n = 20) shows that 55% of the atheromas tested positive for at least one of the target periodontal pathogens. CONCLUSION: These findings indicate that periodontal pathogens are present in atherosclerotic plaques, where they may play a role in the development and progression of atherosclerosis leading to coronary vascular disease and other clinical sequelae.

[Laparoscopic gastric banding using a modified gastric band with loops and a microport system. A report on 25 cases].

INTRODUCTION: The laparoscopic application of an adjustable silicone gastric band is an established procedure in the surgical treatment of pathologic adiposity. PATIENTS AND METHODS: Twenty-five patients with morbid obesity underwent the laparoscopic application of a gastric band between November 1998 and June 2000. A modified band (GastroBelt II) was used. RESULTS: Early and late complications were rare in comparison with previous procedures. Complications, which often require surgical intervention, such as slipping of the stomach (8-12%) or pouch dilatation (2-4%), were not observed. The total morbidity rate and mortality rate were both 0%. CONCLUSION: Critical selection of the patients before the operation and appropriate compliance produced an average loss of weight of 7% of the overweight at 6 weeks, 20% at 3 months and 28% at 6 months after the operation. Preconditions for this operation are a coordinated operation team and a clearly defined standardized postoperative care concept.

[Argon gas embolism in laparoscopic cholecystectomy with the Argon Beam One coagulator].

The report is about argon embolism in a 37-year-old female patient during laparoscopic cholecystectomy and coagulation of a liver bed bleeding with the "Argon Beam One". The presented case report is to show that in spite of technical perfection and correct use of the Argon Beam One coagulator unfavorable anatomical conditions can cause life threatening gas embolism.

[Obturator dislocation of the hip joint].

Luxatio obturatoria may occur in the case of a trauma affecting the hip joint when the latter is in a position of flexion, abduction and external rotation. The femoral head dislocates anteriorly and caudally towards the foramen obturatum. A recent case will be presented with a discussion of the general aspects of this injury, with special emphasis on concomitant injuries and their significance of the long-term prognosis.

[Diffuse eosinophilic gastroenteropathy].

Eosinophilic gastroenterocolitis is a very rare disease of the gastrointestinal tract. As a result, frequently the diagnosis is missed and/or symptoms get misinterpreted. Histological examination and blood analysis however support the finding of the correct diagnosis in most cases. We report here the case of a 47 year old female patient with diffuse gastroenterocolitis and we will comment the diagnostic problems.

[Camurati-Engelmann disease (progressive diaphyseal dysplasia). Differential diagnostic problems].

The Camurati-Engelmann disease is characterized by sclerotical hyperostosis, which can appear mainly bilaterally symmetrical at the diaphyses of the long bones, but also at all other bones of the skeleton. The Camurati-Engelmann disease is a rare one, up to now hardly more than 120 cases have been published. The rarity explains the fact that the disease is often misdiagnosed and mistaken for other bone diseases. This is a case report of a Camurati-Engelmann disease which was detected by radiological diagnosis. The problem of differential-diagnosis is described.

[Luxatio humeri erecta. A rare form of inferior shoulder dislocation].

In patients with erect dislocation of the shoulder, the arm is typically locked in an upright position. This rare variant of subglenoid luxation always causes damage to the ligamentous structures of the joint and sometimes additional complications arise from neurovascular lesions. Two recent cases are reported here. The mechanism of injury, therapeutic measures and prognosis are discussed.
