The Montreal Type A Intervention Project: major findings.

This article reports a comparison of three short-term treatments (aerobic exercise, cognitive-behavioral stress management, and weight training) in modifying behavioral and cardiovascular reactivity to laboratory psychosocial stressors in healthy Type A men. One hundred seven men completed the treatments and evaluations, 33 in the aerobic exercise group, and 37 each in the cognitive-behavioral stress management and weight-training groups. The stress management group showed significantly greater changes in behavioral reactivity (reductions of 13% to 23% below initial values) than the two physical exercise groups, which did not differ significantly from each other. For physiological reactivity, changes attributable to intervention were trivial for all three treatment groups. The positive finding of reduced behavioral reactivity as a result of the stress management intervention is of potential clinical significance and warrants further exploration. The lack of meaningful reductions in physiological reactivity also requires further exploration in that it raises questions concerning the ability of behavioral treatments in general to modify physiological reactivity, the ability of existing measures to assess accurately changes that are produced and, most fundamental of all, the relevance of physiological reactivity as an outcome measure for treatment efforts with Type As.

Three-dimensional structure of the T-layer of Bacillus sphaericus P-1.

The three-dimensional structure of the regular surface layer of Bacillus sphaericus P-1 (T-layer) was determined to a resolution of ca. 2.5 nm by electron microscopy and image analysis. The T-layer has P4 symmetry, a lattice constant of 13 +/- 0.2 nm, and a thickness of ca. 8 nm. The reconstruction revealed three distinct domains: a major, a minor, and an arm domain. In the z-direction, the domains are arranged in two planes creating two different surface reliefs.

Microsurgical endarterectomy under barbiturate protection: a prospective study.

Carotid endarterectomy has the potential to improve on the natural history of untreated carotid artery disease with respect to subsequent infarction in symptomatic patients with causative angiographic lesions. This benefit of a reduced risk of stroke can be realized only if the perioperative morbidity and mortality rates are kept low. An approach to symptomatic carotid artery bifurcation disease is outlined, with a defined protocol of microsurgical endarterectomy utilizing barbiturate protection during the period of potential focal temporary cerebral ischemia. This protocol includes preoperative antiplatelet therapy, barbiturate anesthesia, the avoidance of an internal shunt, the use of the operating microscope, and strict control of postoperative hypertension. A series of 200 consecutive endarterectomies performed within this protocol in 180 patients and the resultant combined permanent morbidity and mortality rate of 1.5% are reported.

Anti-CALLA antibodies identify unique antigens on lymphoid cells and granulocytes.

The common acute lymphoblastic leukemia antigen (CALLA) is a 100-kd surface glycoprotein that is present on normal and malignant lymphoid cells. It is a useful marker for distinguishing between clinically important types of acute leukemia. Anti-CALLA monoclonal antibodies (MoAb) also react with mature myeloid cells (granulocytes), where they identify an antigen having a similar molecular weight (mol wt). We now report that the antigens detected by anti-CALLA MoAb on human lymphoid and myeloid cells differ in their behavior and chemistry. Surface-labeling studies indicate that the antigen on lymphoid cells has a mol wt of approximately 100 kd v 110 kd for that on granulocytes. When cells are metabolically labeled with 35S-methionine, differences in the mol wt of these antigens are again observed. Unlike the lymphoid antigen, expression of that on purified granulocytes is not modulated by incubation with specific antibody. Sodium dodecyl sulfate-polyacrylamide gel electrophoresis analysis of proteolytic digests of the two antigens fails to clarify their chemical relationship. Thus the antigens detected on these two cell types may share an epitope(s) but be chemically distinct, or CALLA may exist in distinct forms and behave differently on lymphoid cells and granulocytes.

[Bone metastases of bronchial carcinomas. 3 cases of subperiosteal cortical osteolysis].

The morphologic appearance of osseous metastasis may rarely suggest a specific site of origin. A subperiosteal location has recently been reported to be typical for metastatic lung carcinoma. We report three cases of such metastasis which destroyed the cortex so that it appears saucer shaped. The primary tumor was a bronchogenic carcinoma in every case. We conclude that in the cancer-age group cortical metastasis is a highly characteristic feature of lung carcinoma.

[Intraventricular hemorrhage after carotid endarterectomy. Role of moyamoya-type collateral circulation].

Stenotic lesions of the cervical arteries due to atherosclerosis or irradiation may provoke the development of a Moya-Moya type collateral network. Rupture of a vessel participating in this collateral circulation may be the cause of a hemorrhagic accident. The hemorrhage may be subarachnoid, intracerebral or more usually intraventricular. Intraventricular hemorrhage occurred in a patient who had developed a Moya-Moya type collateral circulation secondary to atherosclerotic stenosis of a carotid artery. The hemorrhagic incident occurred during carotid endarterectomy, suggesting a predisposing role for hemodynamic modifications resulting from the operation.

Survival in trisomy 18. Life tables for use in genetic counselling and clinical paediatrics.

Clinical management of neonates with Trisomy 18 depends on a knowledge of life expectancy. True estimates of potential life span are required for genetic counselling purposes when parents interpret the genetic threat, not only in terms of the mathematical odds involved, but also in terms of the quality and length of life of an affected infant, should such be born. This paper reports the findings from a study to generate life tables for Trisomy 18. This study is a total population study over 10 years based on a primary population of 2.2 million. Forty-eight cases of Trisomy 18 were identified, five at amniocentesis. Four of the 43 clinical cases (9%) were mosaics. The median life expectancy for live-born infants was five days (range one hour to 18 months). Mean age at death was 48 days. Life tables, by sex and by sub-types (associated congenital abnormalities) are presented. The annual incidence is 14 per 100,000 total births, with a prevalence estimate of 0.06 per 100,000 total population.

The psychological characteristics of infantile hypercalcaemia: a preliminary investigation.

A sample of 23 children aged seven to 12 years with diagnoses of infantile hypercalcaemia was assessed on a battery of psychological tests and rating scales in an attempt to describe the psychological characteristics of this disorder. All children were found to have a mild or moderate degree of mental handicap. As a group, their expressive language skills were superior to their verbal comprehension. They displayed a high degree of behaviour and emotional disorders, most notably in terms of high levels of activity. Most children were reported as being highly sensitive to noise. This study clarifies some of the psychological correlates of infantile hypercalcaemia and some implications for management are suggested.

[Reflections on surgery of high lesions of the internal carotid. Experimental study of the floating endarterial graft].

The use of a floating graft is proposed in order to attempt resolution of surgical problems raised by distal internal carotid artery lesions located high in the neck. Surgical treatment is essential, but vascular suture is impossible below the petrosa and ligature of internal carotid artery is often the only solution. The floating graft, which is fixed proximally only and is allowed to float in the circulation, allows exclusion of the lesion and maintenance of the carotid vascular axis. Preliminary experimental studies in the rat have provided satisfactory results and research should be continued in larger animals prior to possible clinical use.

[Angiolymphoid hyperplasia with eosinophilia. Optical and histo-immunological study of 5 cases with an electron microscopy study of one of the cases].

5 cases of angiolymphoid hyperplasia with eosinophilia of the skin are reported. This rare condition was initially described in 1969 by Wells and Whimster. The microscopical pattern is characterised by an atypical vascular proliferation associated with an eosinophilic and lymphoplasmocytic granuloma. In our series, immunohistological study demonstrated the polyclonality of the plasmacytoid component. Electron microscopy performed in one case shows endothelial features even in cells forming clusters without well-defined lumen. These results support the endothelial origin of angiolymphoid hyperplasia with eosinophilia.

Intraglomerular metastases. Report of two cases.

Glomerular metastases are rarely observed. Two cases of such metastases are reported. In one patient, proteinuria was detected and the diagnosis was made by kidney needle biopsy; light microscopy, immunofluorescence, and electron microscopy studies were performed. The second case was an autopsy finding. The histologic patterns were, respectively, an intracapillary metastasis and associated intra- and extracapillary metastases. These findings raise a number of questions concerning the early detection of such neoplastic diseases, the nature of primary tumors, and the mechanisms of malignant dissemination and glomerular localization.

Temporal variations of hemolymph esterase activity and juvenile hormone titers during ovocyte maturation in Acheta domesticus (Orthoptera).

Using in vitro methods, juvenile hormone (JH) esterase activity and alpha-naphthylacetate esterase activity were determined in the hemolymph during the first reproductive cycle of the house cricket, Acheta domesticus. Biochemical properties of the hemolymph JH esterase were studied. alpha-Naphthylacetate esterases increased during the first gonotrophic cycle: peaks of their activity could be observed concomitant with peaks of JH esterase activity. The fluctuations in JH esterase activity correlated with those of hemolymph JH titers. The results are discussed.

Neuromuscular pharmacology of the anterior intestine of Chaetopterus variopedatus, a filter-feeding polychaete.

The motricity of the anterior intestine of Chaetopterus variopedatus was investigated using extracellular electrodes, force displacement transduction and a pharmacological approach. Rhythmic bursts of impulses were associated with peristaltic waves; the latter were not abolished by removal of the ventral nerve cord which caused a drop in tonus. Acetylcholine induced a contracture and an increase in the frequency of peristaltic waves; these effects were potentiated by eserine and reduced by atropine. Similar responses to adrenaline and noradrenaline were antagonized by propranolol and phentolamine in partially different manners. Gamma-aminobutyric acid (GABA) induced large, picrotoxin-sensitive contractures but failed to change the rate of peristalsis. Serotonin (5-HT) produced a short-term increase in the rate of peristalsis as well as a slight contracture, and a long-term inhibition of peristalsis, all actions antagonized by methysergide. The response to GABA was reduced or abolished during the inhibitory phase of the 5-HT effect. A tetrapeptide related to cholecystokinin/gastrin induced contractures and accelerated peristalsis at a concentration as low as 2.5 X 10(-13) M. It is concluded that multiple neurotransmitter pathways may modulate gut motricity by acting on the peristalsis pacemaker as well as on neuromuscular transmission mechanisms.

Late relapse of testicular cancer.

The complete response (CR) rate of disseminated germ-cell tumors with current aggressive chemotherapy and surgical resection of localized residual disease is between 70%-80%. Most patients who relapse do so within the first year of therapy. We have observed seven patients with germ-cell tumors treated with a variety of modalities who relapsed after more than two years in CR (45-87 months). Five patients are alive after salvage therapy with follow-up too short to assess the durability of second remission. There were no features distinguishing late relapse patients from all patients treated with chemotherapy for germ-cell tumors. Follow-up in patients treated for germ-cell tumors with chemotherapy should be extended to five years before curability can be established.
