[Early and long-term results of rehabilitation of cochlear implant patients]].

BACKGROUND: Cochlear implantation is nowadays a reliable and well-accepted method of auditory rehabilitation in selected adults and children. METHODS: We present rehabilitation data on 21 adult patients provided with the Nucleus CI22 M cochlear implant using the SPEAK strategy. RESULTS: Results of first tune-up show an open set speech understanding in approximately 50% of patients. Long-term results also reveal further improvement in patients who had no open set speech understanding. Our data seem to indicate similar results in comparison to other groups using the CIS strategy. CONCLUSIONS: In adults, open set speech understanding can be achieved even at the first tune-up. Results of early rehabilitation seem to be influenced by duration and etiology of deafness, experience with hearing aids, and other factors.

[Long-term results of multi-channel stimulation of the cochlear nucleus with auditory brain stem prostheses].

Since 1992 11 subjects with bilateral neural deafness due to cerebellopontine angle tumors have been provided with a multichannel auditory brainstem implant. The long term follow-up varies between 12 and 48 months. 10 subjects experienced auditory sensations with various pitch during stimulation of several surface electrodes, 7 of them are still full time users. 1 patient died 18 months after implantation after having been using his device continuously and 1 died due to pneumonic emboly without having been stimulated. 2 subjects revealed uncomfortable side-effects such as motoric or somatosensoric stimulations several months after initial stimulation. 1 of them is not using the device any more. The presented long term results suggest, that deaf neurofibromatosis-patients can regain acoustic contact to the environment, enlarge their communication skills and improve their quality of life by using a multichannel auditory brainstem prosthesis.

Large vestibular aqueduct syndrome and its implication for cochlear implant surgery.

The large vestibular aqueduct syndrome (LVAS) is commonly found when presurgical computed tomography is performed in advance of cochlear implantation. We present data on two patients with LVAS, in whom computed tomography (CT) and magnetic resonance imaging (MRI) was performed. Cochlear implantation when these malformations are present may be complicated by an intraoperative gusher, but it appears to be safe and produces a favorable outcome.

Hearing loss in multiple sclerosis: localization of the auditory pathway lesion according to electrocochleographic findings.

Multiple sclerosis is known to affect the myelin of the auditory pathway resulting in acute hearing loss. Two cases of sudden deafness due to multiple sclerosis have been evaluated by conventional audiometry, brainstem auditory evoked response audiometry and transtympanic electrocochleography. The abnormalities of the compound action potential in both patients (enhanced latency, abnormal adaptation using fast stimulus rate) and the normal receptor potentials (cochlear microphonic, summating potential), as well as the absence of brainstem responses suggest a disturbance of synchronization at the level of the first auditory neurone. The electrocochleography provides valuable information for the topodiagnosis of this and other neural hearing losses, especially in the absence of reliable brainstem responses.

[Lermoyez syndrome--electrocochleographic studies].

BACKGROUND: Lermoyez's syndrome is usually regarded as a rare variant of Menière's disease with hearing improvement and reduction of tinnitus during vertiginous attacks. Electrocochleographic studies of this syndrome are restricted to one published case up to now. METHODS: In our ENT department we clinically and electrocochleographically examined three patients suffering from Lermoyez's syndrome. These results were compared to those typically found in Menière patients. RESULTS: Two of the three patients did not show an elevated summating potential or increased ratio of summating potential and compound action potential (SP/CAP ratio). There was no hearing improvement following oral glycerol administration, and the SP/CAP ratio did not change during a Lermoyez attack. On the other hand, we observed normal cochlear microphonics in ears affected by Lermoyez's syndrome, indicating good hair cell function. In one ear, the threshold of cochlear microphonics was even better than the pure tone threshold. CONCLUSIONS: Our observations suggest that endolymphatic hydrops may not be always the underlying pathologic correlate of Lermoyez's syndrome. Thus Lermoyez's syndrome should not be simply regarded as a variant of Menière's disease.

Restoration of hearing with multichannel prostheses: 10 years' experience.

Our Cochlear Implant Program started in Hannover in 1984 and continued from 1993 on in Freiburg. Up to date, more than 550 patients have received a multichannel cochlear implant and 12 patients an auditory brainstem implant. During these 10 years of experience, many things changed, not only regarding surgery but also regarding indications, speech processing strategies and rehabilitation. The aim of this paper is to present the current state of cochlear implantation and to review important steps of development which have always been linked with efforts to eliminate possible pitfalls. As technology is developing rapidly, this review can only be provisional.

The restoration of hearing in neurofibromatosis type 2.

Central electrical stimulation of the auditory pathway can allow hearing in patients suffering from deafness localized in the auditory nerve. Developments in a multi-channel auditory brainstem implant based on the Nucleus Mini 22 Cochlear implant with transcutaneous signal transmission is discussed. The devices have been implanted in nine European patients suffering from Neurofibromatosis Type 2. Preliminary speech perception results and patient satisfaction are encouraging, and the data presented include some limited open speech recognition.

Central electrical stimulation of the auditory pathway can allow hearing in patients suffering from deafness localized in the auditory nerve. Pseudo-pseudohypertriglyceridemia: a case of increased free glycerol without evidence for glycerol kinase deficiency.

High concentrations of glycerol in plasma may result in overestimation of triglycerides in chemical and enzymatic assays. Known causes of increased glycerol concentrations in blood include glycerol-contaminated blood-collector tubes and pseudohypertriglyceridemia, a condition caused by genetic defects in the gene for glycerol kinase. We report a patient with sudden hearing loss who presented with an increased blood glycerol concentration after undergoing an oral glycerol test.

[3 different malignancies of the aerodigestive tract after chronic abuse of cannabis products].

The occurrence of multilocular malignant tumors in the upper aerodigestive tract in young patients with known marijuana abuse has been described by other authors. A case of a 28-year-old man who was known to abuse alcohol, nicotine and cannabis for some years is presented. He suffered simultaneously from a squamous cell carcinoma of the hypopharynx with bilateral cervical metastases, an adenocarcinoma of the transverse colon and a primary hepatocellular carcinoma. This case is the first reported that shows the occurrence of three separate malignant tumors with different histologies in the aerodigestive tract which could be related to a chronic abuse of cannabis.
