Transscleral and transconjunctival diathermy.

The authors developed a new diathermy electrode that can be used with simultaneous indirect ophthalmoscopic control. The electrode uses the same source and handle as the conventional diathermy but can be used transconjunctivally or transsclerally through full thickness sclera. Chorioretinal scars were successfully induced in 30 rabbit eyes (with or without retinal detachment). Long-term follow-up revealed normal maturation of the scars, with firm adhesion of the retina to the choroid but no gross or histopathologic evidence of permanent scleral damage.

Animals

Local ocular hypothermia in experimental intraocular surgery.

Local ocular hypothermia was evaluated in experimental open sky vitrectomy, closed vitrectomy, and anterior chamber irrigation and aspiration in 40 albino rabbits (80 eyes). The irrigating balanced salt solution was used at room temperature in control eyes and was cooled to 7 degrees C in experimental eyes. Experimental eyes demonstrated less intraocular bleeding volume, less fibrin production, and less postoperative inflammation. No detectable tissue intolerance to hypothermia was observed on short- or long-term follow-up.

Animals

Stage 5 retinopathy of prematurity. Prognostic value of morphologic findings.

Anterior and posterior segment findings were evaluated in 184 consecutive cases of stage 5 retinopathy of prematurity (ROP) (368 eyes). One hundred seventy patients had one or more examinations under anesthesia and 150 had open-sky vitrectomy (260 eyes) with or without additional surgery. Ocular findings were inspected and special attention was given to verify the morphology of the retinal detachment (RD) as predicted by ultrasonography. Immature iris and/or pupil, posteriorly or totally closed detachment funnels, vascularized epiretinal membranes, retinal folds, tractional retinoschisis, persistent hyaloid system, and subretinal blood and/or organization were the most significant indicators of bad prognosis. Evaluating stage 5 ROP eyes with these variables in mind is therefore essential.

Anterior Eye Segment

Retinopathy of prematurity. The association of a persistent hyaloid artery.

In a review of 160 cases of retinopathy of prematurity (ROP), 14 patients were found to have an associated persistent hyaloid vascular system. The following four different manifestations of a persistent hyaloid system were identified: hyaloid artery anastomosed to peripheral neovascularization (1 case); hyaloid artery present with attached retina (3 cases); fibrous-appearing stalk with detached retina (3 cases); and massive persistent hyaloid vascular system (7 cases). In some cases, the hyaloid artery was only an associated finding and unrelated to ROP. In other cases, the hyaloid artery appeared to be an integral component of the retrolental membrane. Recognition of a massive persistent hyaloid artery has important prognostic value because the vessels are adherent to the retina and the retrolental membrane complicating surgical repair. The role of the associated hyaloid artery in ROP is described.

Self-retaining iris speculum for open-sky vitrectomy.

We have designed a self-retaining iris speculum for use in open-sky vitrectomy. This speculum retracts the iris away from the central field and thus allows the surgeon better visibility and less reliance on an assistant's help for retraction. The possibility of iris laceration and dialysis with excessive bleeding, as sometimes seen with other forms of iris retraction, can be prevented.

Equipment Design

Vitreoretinal dissection set.

We developed and evaluated a vitreoretinal dissection set consisting of six microspatulas, a specially designed micro-olive tip, an internal drainage cannula, and a master handle. The microspatulas, which have a spear-headed shape with blunt edges and flat, smooth surfaces, offer a range of angulation from 60 degrees to 160 degrees as well as options for front, back, or tip opening for irrigation or aspiration purposes. All parts can be used through conventional 20-guage or 19-guage sclerostomies, and they all fit on the same handle as well as on any Luer lock system. This set allows for mechanical and/or hydraulic dissection, membrane engagement by suction, and simultaneous use of probe with scissors or forceps. It has been proved safe and effective in more than 100 vitrectomies, including cases of diabetic tractional detachment, proliferative vitreoretinopathy, retrolental fibroplasia, and macular pucker.

Humans

Plasma amino acid levels in cataract.

Fasting plasma amino-acid profiles were determined in 32 subjects with either cataractous lenses (24 subjects) or normal lenses (eight subjects). The findings were compared with established laboratory normal values. The amino-acid levels of all subjects with normal lenses were within the normal range for the subjects' age groups. In contrast, subjects with presenile or senile cataracts had markedly elevated levels of glutamine, with similar elevations of alanine in presenile cataract and of histidine in senile cataract. To the best of our knowledge, this is the first report of such a plasma amino-acid pattern in subjects with presenile and senile cataracts.

Adult

A case of pulmonary tuberculosis presenting with a choroidal tuberculoma.

A 17-year-old woman was referred because of a choroidal mass, with marked decrease in vision, in her left eye. Her medical history was negative. However, preliminary examination revealed a positive skin tuberculin test (PPD) and a chest x-ray picture of pulmonary tuberculosis. Bronchoscopy documented the diagnosis. Findings of other systemic investigations were negative. Fluorescein angiographic findings were compatible with the clinical diagnosis of a choroidal tuberculoma. A regimen of antituberculous medications was started, and both the choroidal and the pulmonary lesions gradually subsided. To our knowledge this is the first reported case of nonmiliary tuberculosis that presented solely with a choroidal lesion.

Adolescent

Record form for eye examination under anesthesia.

We designed a record sheet for eye examination under anesthesia, which is often required in children or babies with suspected ocular pathology and in adults with severe mental retardation. The form is both complete and concise, allows for schematic illustrations, and has proven very helpful in more than 200 examinations performed over the past year.
