Painful sequelae of coxa plana.

Thirty-one patients with an average age of twenty-seven years had painful hips as the result of coxa plana in childhood. The pain was severe enough for surgical relief to be considered. Radiographically the lesions could be subdivided into three groups: severe mushroom deformity of the femoral head, 43 per cent; less severe dysplasia of the hip, 27 per cent; and severe osteoarthritis, 30 per cent. Twenty-three hip operations were done on theses patients. Fifteen hips showed radiographic evidence of definitive premature closure of the femoral capital epiphysis. The series is presented to illustrate that coxa plana may be followed by major disability in the hip within the first three decades of life.

Adolescent↗

Sonographic clarification of the problems of neonatal hip instability.

Real-time ultrasound examinations of neonatal hips were performed to define the natural history of hip instability in the neonate. In clinically unstable hips, ultrasound confirmed displacement of the femoral head of variable severity that was usually bilateral even if unilateral instability had been diagnosed clinically. Ultrasound also revealed that the "reduction" after the Ortolani test may be spurious. Normal sonographic location resulted in all unstable hips that were splinted. In some unstable hips that were not splinted, resolution of sonographic displacement to normal was also documented. In other unstable hips that were unsplinted, persistent sonographic displacement occurred despite early resolution of clinical instability, and later treatment was required.

A sonographic study of the morphology of the preterm neonatal hip.

Preterm and low-birth-weight infants are reported to have an increased risk of late presentation of congenital dislocation of the hip (CDH). A sequential sonographic study of hip morphology was made of 82 live preterm infants. Eighty-two matched, term controls were scanned for comparison. The morphology of the hips was similar at birth. The increase in missed CDH cannot be explained in simple morphological terms. Abnormal birth sonograms resolved rapidly in the absence of clinical instability, casting doubt on the role of static sonographic appearances to predict the need for treatment in the immediate newborn period.

The functional outcome of staged surgery for the correction of talipes equinovarus.

Between 1988 and 1995, 110 idiopathic clubfeet from a cohort of 120 recalcitrant feet in 86 patients requiring surgical treatment were studied. There were 61 male and 25 female patients. The mean age at surgery was 9.5 months. In 91 feet, the surgery consisted of an initial plantarmedial release, followed 2 weeks later by a posterolateral release. Nineteen feet required only a posterolateral release. Feet were categorised preoperatively and prospectively according to a system suggested by Dimeglio into four groups, and the rates of relapse and wound healing data were previously reported. This paper reports the functional outcome of this cohort of idiopathic clubfeet and the results are related to the preoperative grade, the patient's gender, the age at which the child first walked, and whether the deformity is unilateral or bilateral. The interim functional outcome is good in the majority of cases despite a high rate of relapse in the more severe deformities.
