[Detection of squamous cell differentiation in experimental tumors of urothelium by using epidermal G2 chalone].

The presence of epidermal G2 chalone was investigated by different immunochemical methods in normal urothelium and in tumors of the urinary bladder induced in rats with N-butyl-N-butanol-(4)-nitrosamine. This chalone was not revealed in normal urothelium but found in 37 out of 56 tumors examined. In all these 37 tumors, squamous urothelial metaplasia was detected by electron or even light microscopy. The extent of morphological manifestations of squamous metaplasia showed a definite correlation with the concentration of chalone in tissue extracts. The method used may be applied as a subsidiary test for the determination of focal squamous metaplasia in biopsy material of human urothelial tumors which is of importance for prognosis of the disease.

Evaluation of growth rate of human transitional cell tumors according to sex chromatin (Barr bodies) content.

In 53 women with urothelial tumors of the bladder, renal pelvis and ureter of different in growth stages and grades of differentiation, a negative correlation among the value of the mitotic index and sex chromatin content was found. With a decrease in the number of nuclei containing sex chromatin bodies we have noted not only a proportional mitotic index increase but also a regular connected decrease of histological differentiation and a deeper invasion of the tumor. The significance of this simple test consists in the fact that one may judge the rate of tumor growth by the sex chromatin content in a small volume of biopsy material. A table of mitotic index values depending upon the quantity of nuclei with sex chromatin bodies in transitional cell tumors is presented.

[Immunologic determination of an epidermal G2-chalone-like factor as a marker of squamous cell structures in the urinary bladder and urine in rats].

The epidermal G2 chalone-like substance of the rat skin may be determined by immunochemical methods only in squamous epithelia and it is revealed neither in mucosa of the urinary bladder, nor in urine of control rats. The antigenic activity of the chalone was revealed in 37 extracts of 56 tumors of the rat bladder. All antigen-containing tumors were found to be squamous cell carcinomas, transitional cell carcinomas with squamous cell metaplasia or possessed ultramicroscopic signs of squamous cell metaplasia appearing as "pure" transitional cell carcinomas under light microscope. The epidermal G2 chalone-like substance was present in urine of 7 out of 10 rats bearing the antigen-positive tumors of the urinary bladder.

[Morphological characteristics of slowly and rapidly progressing human bladder papillomas].

A total of 477 transitional cell papillomas (TP) of the urinary bladder were divided into slowly (10.9%) and rapidly progressing (89.1%) ones according to the late results of the treatment and morbid anatomic features. Over a period of ten years following the excision of rapidly progressing TP the recurrencies were noted 5-fold and malignancies 12-fold as often comparatively to slowly progressing papillomas. Such papillomas were 1 cm and less in diameter and had a narrow base. The epithelial thickness reached 7--8 cell layers, and the mitotic index 3--4 fold surpassed the normal one (0.1%). The difference in histologic and ultrastructural differentiation of basal and intermediate cells of the epithelium persisted. Rapidly progressing TP were 0.5--2 cm in diameter, the epithelial thickness often exceeded that of 8 cell layers, while the mitotic index ranged within 0.5--1.0%. There were no sign of cellular atypia. The difference in histologic and ultrastructural differentiation of basal and imtermediate cells of the epithelium was in some cases effaced. TP subdivision into slowly and rapidly progressing papillomas will contribute to the improvement of the disease prognosis and statistical recording.

[Experimental carcinogenesis in the urinary bladder against a background of Freund adjuvant and levamisole administration].

Administration of complete Freund's adjuvant (CFA) to rats before carcinogenesis and in its early period was associated with an increase (not less than 2-fold) in the number of spindle-shaped dark cells that permeated the hyperplastic urothelium and, judging the ultrastructure, had a macrophage nature. These animals demonstrated a relationship between the increased number of dark cells and delayed development of the subsequent stage of carcinogenesis--papillomatous hyperplasia. CFA administration to the animals with a developed cancer of the urinary bladder did not affect it noticeably. It did not produce any effect on subcutaneous transplantation of cancer that had taken in 100% of cases. No morphological features of carcinogenesis were recorded either after levamisol administration. However, none of the tumours withdrawn from the rats given levamisol took under the skin of the recipients.

[Specific erythrocyte adherence reaction: an index of the malignant potentials of noninvasive tumors of the human urothelium].

Histological sections of 30 primary transitional cell tumors (15 papillomas, 2 cancers of the first, 3--the second and 10--the third stage) were employed for the reaction of specific adherenci of erythrocytes to defect ABO(H)-agtigens. The latter were absent in invasive cancers of grade III as well as in the noninvasive eptiheliomas which were associated with invasive recurrences during 2 years. To a considerable or full extent the antigens remained in tumors following the removal of which invasive recurrences beveloped accordingly in 3 years or later. It is suggeested that changing (loss) of ABO(H)-antigenic differentiation in urothelial tumors precedes the onset of invasion at least by 2--3 years.

[Malignant carcinoid tumor of the bladder].

One of the first observations of malignant carcinoid of the urinary bladder in a man of 42 is described. Histologically the tumour resembled solid-alveolar transitional cell carcinoma. The carcinoid was verified by finding in the tumour cells electron dense endocrine granules and helix-like rough endoplasmic reticulum. Possible sources of urothelium carcinoid development are discussed.

[Ultrastructural stereometry of highly differentiated transitional cell tumors of the bladder].

Cytomorphometric examination of the normal urothelium, 3 primary, 3 recurrent papillomas of the urinary bladder and 3 cancers of stage I indicated that average relative volumes of the nucleus and cytoplasm in their cells are nearly similar. Since these bolumes increase simultaneously in the tumors concerned, there is no change in the "nucleus-cytoplasm correlation" or "index" described above. Cytomorphometric indices of the nucleus and cytoplasma in papillomas are more close to those of high-differentiated cancer than of the normal urothelium. In histologically indistinguishable primary and recurrent transitional cell papillomas the indices would differ: in primary papillomas compared with the norma the amount of mitochondria is reliably increased statistically, but the volume of each of them is diminished, whereas in recurrent ones--vice versa.

[Intestinal metaplasia in glandular and cystic formations and human urothelial adenocarcinoma].

Thirteen cases of inflammatory changes in the urothelium with formation of glandular and cystous structures, 3 cases of glandular metaplasia in transitional cell carcinoma and 8 adenocarcinomas were examined. In inflammation, initial glandular and cystic structures have signs of somehow incompleted metaplasia of the intestinal type. In advanced glandular structures metaplasia is completed and urothelium acquires similarity in the structure and properties of the produced mucus with the large intestine epithelium (numerous goblet, cylindrical limbic and non-differentiated cells, prevalence of argentaffinic cells over argirophilic, lack of Panet cells, secretion of sulfomucines). In loci of glandular metaplasia of transitional cell carcinoma there are also signs of somehow incompleted intestinal differentiation. Most typical variants of urothelial adenocarcinoma are quite similar to glandular carcinoma by the structure and properties of the produced mucus.

[Significance of a urinary infection for the diagnosis, clinical course and histological characteristics of epithelial bladder tumors. The results of antibacterial therapy].

Examination of the case records and histological preparations of 1150 patients with epithelial tumors of the bladder revealed urine infection with inflammation in the bladder wall in 12.1% of the cases. The inflammation not only aggravated the clinical progress of the disease but also interfere with determination of the real tumor size on cytoscopy and cystotomography. Leucocytes, macrophages, edema zones in the granulation tissue were found in great numbers in addition to the usual lympho-plasmocytal infiltration in the cases with intravesical infection in the tumor stroma and perifocally. The signs of cystic or grandular cystitis and more seldom the foci of the transitional epithelium epidermization were found at a distance of 2-4 sm from the tumor in 66.6% of the patients. The signs of the infection and inflammation disappeared after antibacterial therapy in 96.8% of the patients. It was not possible to eliminate the infection and inflammation in 1/3 of the patients with exoendophite and endophite tumors, especially in those predisposed to necrosis.

[Inverted papilloma of the bladder].

No more than 40 cases of inverted papilloma (IP) of the urinary bladder have been reported in literature. The author presents 2 observations of IP in both of which the epithelial transitional cellular structures showed submerged pseudoinvasive growth and a peculiar inversion of the villous strucure. The tumor is benign clinically and histologically, its benign nature is confirmed by the remote results of observations. Despite the fact that the histogenesis of IP is not quite clear yet, the author, as most other investigators, classify IP according to the International Histological Classification as benign tumor differing from common papillomas of the urinary bladded only in the direction of the growth and the arrangement of villous structures.
