Incidence of lymph node metastasis in elective (prophylactic) neck dissection for oral carcinoma.

For cancers of the tongue, floor of mouth, mandibular gingiva and buccal mucosa, in which the widest diameter is greater than 2 cm, we perform neck dissection in continuity with resection of the primary growth as a part of the planned therapy, regardles of the clinical state of the cervical lymph nodes. Whenever the lesion is so situated that an in-continuity neck dissection cannot be performed (cancer of the palate) it may be wise to delay neck dissection until the lymph nodes become clinically apparent.

Free transplantation of lyophilized dura for vestibuloplasty: a clinical and histological study.

Covering epithelial defects in the oral cavity is often neccessary in maxillofacial surgery. Special problems arise in preprosthetic surgery because requisites for function and quality of epithelial grafts must be satisfied. Using free heterologous lyophilized dura grafts for vestibular sulcoplasty in 23 patients, we observed a good clinical and histological healing process. Considering the necessity of a second operation for skin or mucosal grafts (and the limited source of mucosa), we believe that the lyophilized dura can be used successfully in preprosthetic surgery.

Extractions of impacted mandibular wisdom teeth in the presence of acute infection.

The removal of 720 impacted mandibular third molars in the presence of acute pericoronitis, employing as atraumatic an operative technique as possible, did not give rise to serious complications, such as osteomyelitis, brain abscess, septicemia or facial space abscess. The results have been compared with a control group of 1,000 impacted mandibular third molars extracted without presence of acute infection.

[Preoperative embolization for extensive facial hemangiomas].

An extensive hemangioma of the lower face is discussed as an example illustrating the use of therapeutic--particularly preoperative--arterial embolization in the treatment of extensive facial hemangiomas. In this procedure a thrombusinducing material is inserted into the lumen of a blood vessel to occlude this vessel, and thus to reduce blood flow to the lesion. This promising technique opens up new possibilities in the treatment of extensive hemangiomas of the maxillofacial region. It may be used either as a definite therapy or as pretreatment procedure enabling a safer and simpler surgical approach.

[End-to-end anastomosis of the lacerated parotid duct].

The case of a patient with an extensive vertical laceration of the right cheek involving Stensen's duct is reported. Treatment of parotid duct injuries varies with location and type of the injury. Immediate repair of the duct can prevent troublesome and lengthy treatment at a later date, whereas ineffective primary management may lead to complications and render treatment much more difficult.

[Apicoectomies in molars].

The apicoectomy in molars is a well accepted, though not so popular, method for the treatment of molars with chronic inflammatory periapical disease. This paper is about the indications as well as the surgical technique of the operation. The procedure is connected with very good clinical results and no serious complications, therefore is suggested for wider clinical application.
