Laryngeal carcinoma in childhood. Report of a case and review of the literature.

A rare case of an 81/2 year old boy with a stage III well differentiated squamous cell carcinoma of the larynx, which responded to chemotherapy and irradiation, is reported. It is the 56th case in children up to 15 years of age and 20th case in children 10 years old or younger reported since 1868. Carcinoma of the larynx in children has similar clinicopathological features to those of adult laryngeal carcinoma, but markedly differs as concerning sex incidence ratio. The main differential diagnosis is juvenile papillomatosis and the principal risk factor is previous cervical irradiation. Delayed diagnosis is frequent since clinical manifestations might be attributed to the normal laryngeal maturation process or to a common respiratory infection. Treatment is problematic, for special effort must be made to preserve laryngeal anatomy and function and to avoid complications. Prognosis of childhood laryngeal cancer is unclear since the reported survival rates are not tumor stage related.

Benign and malignant pseudothyroiditis.

Benign and malignant lesions of the thyroid gland and its adjacent structures may present clinical, biochemical and radio-active scan features resembling subacute thyroiditis. An irregular clinical course, unexpected laboratory findings and unresponsiveness to medical treatment may indicate the true nature of the disease. Whenever a subacute or chronic thyroid inflammatory process is diagnosed, the possibility of benign or malignant lesions of the thyroid or of the surrounding organs should be kept in mind. If doubt is raised, further investigations should be carried out if indicated including exploratory surgery.

New clinical applications of the stapedial reflex.

Stapedial reflex data were obtained from 11 myasthenic, 18 hyperthyroid and six hypothyroid patients. Data were studied with respect to reflex threshold, amplitude and decay. Elevated threshold, decreased amplitude, and high and low frequency reflex decay were observed in myasthenic and hyperthyroid patients. Hypothyroidism merely increases stapedial reflex amplitude but causes no significant threshold or decay changes.

Histologic findings in a larynx reconstructed by a nasoseptal autograft after partial laryngectomy.

In the course of a 5-year follow-up of 16 patients who underwent plastic reconstruction of the larynx by a nasoseptal graft after extended frontal and frontolateral laryngectomies, one of them developed local recurrence of squamous cell carcinoma and was totally laryngectomized. Histological studies were made of the grafted cartilage and its mucosal lining. The microscopic findings are reported.

Coexistent carcinoma of the thyroid gland and Hashimoto's thyroiditis.

Three instances of Hashimoto's thyroiditis occurring in association with carcinoma of the thyroid gland are presented. Diagnosis was made by open surgical biopsy, revealing one papillary, one follicular and one mixed papillary-follicular carcinoma. Total thyroidectomy was performed and TSH suppressive therapy was administered postoperatively. All patients are alive with no evidence of local recurrence or distant metastases observed during five to 13 years of follow-up study. In our series, three of 18 patients had coexistent carcinoma of the thyroid gland and Hashimoto's disease. This figure is lower than the 25 per cent incidence of carcinoma of the thyroid gland in noninflammatory goiters observed at the same hospital. Although the number of patients is small, Hashimoto's thyroiditis does not seem to be a premalignant lesion and should not be treated by total thyroidectomy. A review of the literature points out that clinical, biochemical and radioactive scan findings in patients with Hashimoto's thyroiditis do not aid in differentiating between glands with or without coexistent carcinoma of the thyroid gland unless the goiter is uninodular and cold on the radioiodine scan. The prognosis of patients with carcinoma of the thyroid gland occurring in association with Hashimoto's disease is probably better than that of patients with carcinoma of the thyroid gland alone. It seems that the presence of the chronic inflammatory reaction may suppress growth and metastatic dissemination of the coexistent neoplasm of the thyroid gland.

Laryngeal reconstruction by composite nasal mucoseptal graft after partial laryngectomy. three years follow-up.

We have used autogenous nasal mucoseptal (NMS) graft for the reconstruction of the larynx after partial laryngectomy in glottic carcinoma invading the anterior commissure, for the past three years. The composite graft, readily available and obtained without sequela, provides satisfactory and large enough replacement tissue. It provides both a dependable rigid cartilaginous wall and a respiratory mucous lining of the lumen. The method was found to be simple and effective in bridging the laryngeal defect. The free NMS graft was used for reconstruction after extended frontal and frontolateral laryngectomy in 10 patients. The patients were successfully decannulated within 4 to 6 days after the operation and in none of the cases was nasogastric feeding tube or laryngeal stenting necessary. In eight patients anatomical and functional results were achieved; the neolarynx prevented a permanent tracheostomy and a fair voice was obtained.

Amniotic graft in the management of severe epistaxis due to hereditary haemorrhagic telangiectasia.

In selected cases of Hereditary Haemorrhagic Telangiectesia with severe, life-threatening epistaxis, a new surgical procedure has been performed. It consists of a thorough, near total excision of the diseased nasal mucosa and its replacement by an amniotic memebrane graft. The surgical details dealing with preparation of the amniotic graft are described. Surgery has been performed by this technique in six cases. During 9 to 30 min months of follow-up, no nasal bleeding has occurred and the haemoglobin level has remained normal. The aminiotic membrane has served as an adequate grafting material for the lining of the nasal chambers.

Malignant oncocytoma.

A case of malignant oncocytoma of the parotid gland in a 32-year-old male is presented. Ten months after parotidectomy an undifferentiated carcinoma, in which oncocytes still could be recognized, developed in the operated area. According to the literature available to us, this is the second reported case in which malignant transformation in a benign oncocytoma of the salivary gland has been observed.
