Serum CA-125 as a guideline for the timing of a second-look operation and second-line treatment in ovarian cancer.

In the assessment of response to treatment of ovarian cancer patients, clinical examination is unreliable in detecting small tumor masses. The second-look laparotomy is therefore an accepted procedure. The optimal timing of second-look laparotomy, however, is uncertain. We therefore examined the usefulness of serial serum CA-125 estimations in the timing of second-look laparotomies in 33 patients suffering from ovarian cancer. Increasing CA-125 concentrations were always followed by relapses or progressive disease, whereas decreasing serum concentrations indicated response to treatment. In future we propose to perform second-look laparotomies when CA-125 levels have declined to a steady plateau and to start a secondary treatment when CA-125 levels start to rise again.

Management of airway problems in lung cancer patients using the neodymium-yttrium-aluminum-garnet (Nd-YAG) laser and endobronchial radiotherapy.

This study summarizes our experience in the evolution of a management technique for unresectable or recurrent lung cancer using the neodymium-yttrium-aluminum-garnet (Nd-YAG) laser with high-dose endobronchial brachytherapy. The laser gave good palliation for hemoptysis and obstruction with a low complication rate, and the addition of endobronchial radiotherapy was a useful adjunct for maintaining airway patency. High-dose brachytherapy delivered by the Gammamed IIi remote afterloader proved to be the best method of delivering this treatment.

CA-125 serum concentrations during the menstrual cycle.

CA-125 is an antigenic determinant expressed on ovarian cancer cells and serves as serum marker in patients with ovarian cancer. It has also been detected, however, in serum of healthy women and in patients with benign gynecologic diseases. Previous studies in patients with ovarian hyperstimulation syndrome suggested that the production of CA-125 is correlated with proliferative ovarian activity. In this study, we describe the pattern of CA-125 secretion in 13 healthy women during menstrual cycle and during hormonal contraceptive treatment. That pattern is characterized by an increase of CA-125 serum concentrations parallel to the growth of the dominant follicle. It was not observed during hormonal contraceptive treatment. We conclude that CA-125 serum levels during menstrual cycle seem to be dependent upon the cyclic changes in the female genital tract.

Surgery for cervical cancer.

A total of 326 squamous cervical cancer patients treated by radical hysterectomy and lymphadenectomy in the Women's Hospital of Erlangen University were analysed according to their survival rates and to the histological examination results of their surgical specimens. A good relationship was found to exist between the pathological anatomical stage of the cancer, tumour volume, incidence of lymph node involvement, and survival rate. The result showed that good results from surgical treatment can be achieved even for very large cancers (Stage IIb) with a high frequency of lymph node involvement.

Quantitative studies on the vaginal flora of asymptomatic women and patients with vaginitis and vaginosis.

Vaginal washings of 22 patients with vaginitis, 11 with vaginosis, and 12 healthy subjects were investigated quantitatively and qualitatively for aerobic and anaerobic bacteria and yeasts. Gardnerella vaginalis was recovered from 9 of the vaginitis patients, 7 of the vaginosis patients, and 4 of the asymptomatic subjects. Obligate anaerobes were found in 11 of the vaginitis patients, 4 of the vaginosis patients, and none of the control subjects. Bacteroides bivius was the anaerobe most frequently isolated from symptomatic subjects. Anaerobic vibrios were recovered twice from symptomatic subjects. The counts for Gardnerella vaginalis and anaerobes when present were generally very high. The most frequent aerobes were beta-hemolytic streptococci (group B) and staphylococci.

Concentration of sulbactam and ampicillin in serum and the myometrium.

Sulbactam 1g and ampicillin 2g were administered intravenously to 12 patients, and concentrations of the drugs were determined in the serum and myometrium for up to 120 minutes after infusion. The ratio of sulbactam to ampicillin was about 1:2 in the serum and myometrium. Both drugs penetrated well into the myometrium, and median concentrations of sulbactam and ampicillin were 65% and 68%, respectively, of those in the serum. Sulbactam/ampicillin was effective for perioperative prophylaxis and should be effective as therapy for pelvic infections.

[Epidemiology and early recognition of cervical cancer].

Probably due to intensive methods of early recognition, the incidence of uterine cervix cancer is decreasing in all industrialized countries. The efficiency of early recognition programs is well documented in literature, but these results are by far not achieved in clinical practice: for example only 30% of all women of childbearing age are reached by prophylactic medicine. There are also problems with respect to the quality assurance in cytology and colposcopy. As to etiology, certain papilloma viruses (HPV 16 and 18) are today a preferential object of research, but an individual risk has not yet been described exactly. This, however, would be important for the individualization of diagnostic methods, above all if they involve invasive procedures (e.g. Schiller abrasio, conization). This is also true for the treatment of early forms of uterine cervix cancer. The treatment, standardized to a large extent in former times (great surgical intervention according to Wertheim-Meigs in all cases) has now been replaced by an individualized therapy with a possibility to choose between conization, laser treatment if necessary, simple hysterectomy, and radical operation.

[Bronchial carcinoid. Description of 2 cases].

Bronchial carcinoid is a rare tumor appearing mostly in young patients, affecting males and females in equal proportion. It has a low grade malignancy. Carcinoid syndrome is infrequent in this condition and the clinical manifestations are those of bronchial obstruction. Two cases of bronchial carcinoid, diagnosed by bronchoscopy are described.

A promoter whose utilization is temporally regulated during sporulation in Bacillus subtilis.

The formation of endospores in the Gram-positive bacterium Bacillus subtilis proceeds according to a temporally ordered program of gene activation. To investigate timing mechanisms in sporulation gene expression, we have isolated and sequenced the promoter region for a B. subtilis gene known as 0.3 kb whose transcription is switched on at about stage III of development. The 5' terminus of the 0.3 kb mRNA was mapped by the S1 nuclease procedure to a position just upstream from its apparent ribosome binding site and initiation codon and just downstream from the transcription termination site for an adjacent gene. This information enabled us to construct a transcriptional fusion in which the 5' region of the 0.3 kb gene was joined to the lacZ gene of Escherichia coli. When introduced into cells of B. subtilis, the 0.3 kb-lacZ fusion caused the synthesis of a fusion-specified RNA that originated from within the 0.3 kb promoter region and extended into the adjacent E. coli DNA, and the induction of beta-galactosidase synthesis at the third to fourth hour of sporulation. Enzyme synthesis required the 0.3 kb promoter, since a deletion of the 5' region of the 0.3 kb gene in the transcription fusion eliminated the production of beta-galactosidase. Induction of the 0.3 kb-lacZ fusion was under developmental control, since the production of beta-galactosidase was blocked or substantially impaired by chromosomal mutations in the sporulation genes spoOB, spoIIA, spoIIE and spoIIIE, but not by a spoIIC mutation. We conclude that the 0.3 kb gene promoter is subject to a developmental clock, which delays its utilization until an intermediate stage of sporulation, and discuss models for how the timing of gene expression is regulated.

[Role of Gardnerella vaginalis and anaerobes. Etiology of nonspecific colpitis].

Nonspecific vaginitis is a very common clinical syndrome. In patients with nonspecific vaginitis the normal, lactobacillus-dominated microbial flora is replaced by Gardnerella vaginalis and certain anaerobes, preferably bacteroides spp. The mechanisms by which anaerobes or Gardnerella vaginalis or a combination of these organisms produce the disease condition are still unknown. Treatment with ampicillin and certain other drugs is often effective, but metronidazole appears to be the most effective antimicrobial for this condition. The optimal dose and duration of metronidazole therapy require further study.

Nucleotide sequences that signal the initiation of transcription and translation in Bacillus subtilis.

We have determined the nucleotide sequence of two Bacillus subtilis promoters (veg and tms) that are utilized by the principal form of B. subtilis RNA polymerase found in vegetative cells (sigma 55-RNA polymerase) and have compared our sequences to those of several previously reported Bacillus promoters. Hexanucleotide sequences centered approximately 35 (the "--35" region) and 10 (the "--10" region) base pairs upstream from the veg and tms transcription starting points (and separated by 17 base pairs) corresponded closely to the consensus hexanucleotides (TTGACA and TATAAT) attributed to Escherichia coli promoters. Conformity to the preferred --35 and --10 sequences may not be sufficient to promote efficient utilization by B. subtilis RNA polymerase, however, since three promoters (veg, tms and E. coli tac) that conform to these sequences and that are utilized efficiently by E. coli RNA polymerase were used with highly varied efficiencies by B. subtilis RNA polymerase. We have also analyzed mRNA sequences in DNA located downstream from eight B. subtilis chromosomal and phage promoters for nucleotide sequences that might signal the initiation of translation. In accordance with the rules of McLaughlin, Murray and Rabinowitz (1981), we observe mRNA nucleotide sequences with extensive complementarity to the 3' terminal region of B. subtilis 16S rRNA, followed by an initiation codon and an open reading frame.

Concentrations of placental proteins (HPL and SP1) in maternal serum throughout normal pregnancy.

Throughout 65 normal singleton pregnancies 332 blood samples were obtained and analyzed for pregnancy-specific beta 1 glycoprotein (SP1) and human placental lactogen (HPL). The measurement of SP1 in maternal serum was made using radial immunodiffusion, that of HPL by using radioimmunoassay. There was wide variation in the number and timing of blood samples obtained from patients, and therefore the results could not be used to construct a regression curve by the usual methods applied to independent measurements. Consequently only one value per patient was chosen at random as a basis for calculations. A second degree polynomial for the logarithm of the serum concentrations yielded an optimal fit for both parameters. Tolerance intervals of 90% were calculated for use in future studies.

[Amniotic fluid embolism with coagulation disorder--a case report (author's transl)].

A case of amniotic fluid embolism during the delivery of a twin pregnancy is reported. After immediate resuscitation with artificial ventilation the mother and the neonates survived the acute episode. As a result of hyperfibrinolysis with premature separation of the placenta a dramatic postpartum haemorrhage occurred, which could be controlled by Fritsch' manoeuvre. Meanwhile it was possible to restore the coagulation disorder with fresh whole blood, fibrinogen and an antifibrinolytic agent (Aprotinin).
