The who, why and how of radiology teaching files.

A variety of teaching files in diagnostic radiology is needed to serve a broad spectrum of purposes and students. A method of acquiring a simplified teaching file to be used for and by medical students, non-radiology house staff, first-year radiology residents and senior staff members is described. The medical student, as a learning experience, acts as the procurer of this file.

Film mammography: new low radiation technology.

A new mammographic technique with Kodak MIN-R film and a Kodak MIN-R screen in a vacuum package has been evaluated in 607 cases. Direct mammographic comparison is made with the Du Pont Lo-dose system in the first 101 patients. The system provides excellent diagnostic quality images with at least a two-fold reduction in patient exposure compared to other current mammographic techniques.

Angiographic documentation of systemic-portal venous shunting as a cause of a liver scan ""hot spot'' in superior vena caval obstruction.

Four patients with superior vena caval obstruction were evaluated with contrast venography and radiocolloid liver scans to investigate systemic-portal venous shunting as a cause of a liver scan "hot spot". In two of the cases a hot spot due to systemic-portal venous shunting was documented by angiography. The venous anatomy and pathophysiology related to hot spot formation on the liver scan is discussed. To develop a liver scan hot spot it appears necessary to have considerable systemic-portal venous flow through anterior parietal and periumbilical venous channels.

Osteoblastomas of the axial skeleton shown by skeletal scanning: case report.

Two patients with osteoblastoma of the axial skeleton showed intense uptake of 99mTc-polyphosphate and 99mTc-diphosphonate on skeletal scans. Although this lesion is uncommon, it should be included in the differential diagnosis for young patients who show abnormal accumulation of 99mTc-phosphates in the axial skeleton.

B-scan ultrasound evaluation of the pancreas. Advantages and accuracy compared to other diagnostic techniques.

Over 200 patients were examined by B-scan ultrasound for suspected pancreatic disease; in 94 cases a final diagnosis was proved. The diagnostic accuracy of ultrasound, upper gastrointestinal examination, isotope pancreatography, and arteriography is compared. The diagnostic ultrasound features of the various pancreatic disease processes are described. Pancreatic sonography provides the most accurate and least expensive method for evaluating the pancreas with no known patient morbidity. The upper gastrointestinal series and radionuclide study are complementary to sonography. Ultrasound should be the initial examination for the evaluation of patients with suspected pancreatic disease or epigastric mass.

A practical approach to the diagnosis of abdominal aortic aneurysms.

In this study 217 patients with proved abdominal aortic aneurysm were reviewed and the diagnostic accuracy of ultrasonography was assessed in comparison with physical examination, plain abdominal roentgenography, and aortography for the evaluation of patients with a suspected abdominal aortic aneurysm. Ultrasonography is most accurate in detecting an aneurysm and in defining the size of the aneurysm. Routine aortography to evaluate the renal and iliac artery is not justified in the preoperative evaluation of patients with an abdominal aneurysm since the renal artery infrequently is involved by the aneurysmal process (3.7 percent) and the surgical results are consistently successful in those patients with renal and/or iliac artery involvement. If preoperatively needed, the renal status can be evaluated accurately with an isotope renogram combined with photoscan of the kidney. A practical approach to the diagnosis of pulsatile abdominal masses is proposed.

Comparison of two detail screen-film systems using a rheumatoid erosion model.

Two detail screen-film systems, Lanex Fine/Ortho M and Lanex Fine/Ortho G, were evaluated using a radiographic model for rheumatoid erosions. Radiographs of hand bones with cortical defects using both screen-film systems were obtained. Seven observers analyzed 48 sites on each of 22 films for the presence of these cortical defects. Using analysis of variance techniques and receiver operating characteristic curves, no significant difference was found between the two systems for the detection of these subtle cortical defects. The authors conclude that although the Lanex Fine/Ortho M system provides better definition of laboratory test objects than Lanex Fine/Ortho G, the two systems do not differ when used for detection of cortical lesions.
