The effect of single dose of three different types of high cholesterol diet on plasma total cholesterol (PTC) in 24 young healthy subjects (male:female = 1:1) aged 15 to 35 years was studied. One group (n1 = 8) was given a butter and milk diet (300 mg cholesterol and 95 g fat). In the first postprandial hour PTC level increased significantly in all but one subject (mean +/- S.D., 4.96 +/- 0.57 m mol/l to 5.61 +/- 0.60 m mol/l, P < 0.005). Second group (n2 = 8) was given a single egg diet (300 mg cholesterol and 6 g fat). In the first hour the PTC level decreased significantly in all but 2 subjects (4.82 +/- 0.58 m mol/l to 4.42 +/- 0.63 m mol/l, P < 0.02). Third group subjects (n2(3) = 8) were given a test diet consisting of crystalline cholesterol with 200 ml milk (1020 mg cholesterol and 14 g fat). The PTC level increased insignificantly. In the first hour in all but 2 subjects (4.94 +/- 0.43 m mol/l to 5.35 +/- 0.88 m mol/l, P > 0.10). In the third postprandial hour the PTC values in all the 3 groups showed a tendency to return to fasting values. Therefore, we conclude that the effect of dietary cholesterol on PTC depends not only on the amount of cholesterol content in the diet, but on the type of diet (cholesterol vehicle) as such and probably also on the fat content of the diet.
The management of young patients with valvular heart disease is a difficult problem in developing countries. At our Institution, 765 valves were operated upon in 523 patients over a period of two years. Rheumatic etiology was seen in 70% of the valves and 60% of all the valves were repaired. The hospital mortality and morbidity in the form of thromboembolic episodes were less in the repair group; 35 valves in 26 patients had to be reoperated upon for instability of the primary repair. Despite this high and early reoperation rate in the repair group and considering the higher mortality plus the problems of anticoagulation and limited durability of bioprostheses in the replacement group, conservation of the patient's own valve remains a better alternative in the long-term.
The cytomorphologic findings in fine needle aspirates from 18 cases of histologically confirmed primary mucoepidermoid carcinoma of the salivary gland were reviewed and correlated with the findings on the tissue sections. An accurate diagnosis of these tumors in fine needle aspirates was possible when all three components (i.e., epidermoid cells, intermediate cells and mucus-producing cells) were identified. The pitfalls in the cytologic diagnosis of this tumor are discussed.

A survey was conducted at Agra (India) to study the extent and pattern of the non-medical use of dependence producing drugs among the post-graduate students of the local colleges and a number of medical students of other colleges of the state of Uttar Pradesh who were posted for training at the Mental Hospital. The study was confined to the academic year 1975-76 and covered 1,200 students. The present interim report is based on a sample of 564 students covered during 1975 (1st stage). The data on the total sample of 1200 (2nd and the final stage) are still being analysed. The results reveal that 73.88 per cent male, and 25.96 per cent female students had a drug experience at some time or another. Drug use was highest (80.66 per cent) among male medical students. The substances commonly used by males were: alcohol, barbiturates, Mandrax (methaqualone diphenhydramine hydrochloride). Vesparax (hydroxyzine hydrochloride), Equanil (meprobamate). Librium (chlordiazeproxide), pain killers (minor analgesics such as aspirin, and cannabis (bhang, ganja, and charas). The female students mainly used Equanil and pain killers. Among the 23 reasons offered for the use of drugs, the majority of students (50-59 per cent) stated that their main reason for drug use was "to relieve tension and facilitate relaxation". The next motivating factor for indulgence was "for the sake of fun" (30-39 per cent). The student drug users reported a number of effects produced by various substances. The most commonly mentioned effects were: excessive sleepiness, sluggishness, giddiness, inability to concentrate on studies, poor physical co-ordination. They expressed their opinion on various aspects of the drug use problem and favoured stringent measures to curb it.
Phenotype and gene frequencies of 10 blood group and 12 red cell enzymes are examined in urban Hindu and Muslim and tribal Lambadi samples of Andhra Pradesh, South India. Genetic heterogeneity is suggested between the tribal and the urban samples, both in their gene frequencies and in their breeding structure.

Esophageal intubation with a plastic prosthesis is a well established palliative treatment for esophageal carcinoma. The technique is safer with endoscopy than previous surgical techniques. Advantages of stent include rapid and long lasting relief of dysphagia in most patients with carcinoma esophagus. Repeated procedures are not required. Placement of prosthesis is the treatment of choice in BEF. Cost is less compared to other palliative modalities such as laser. SEMS have distinct advantages over conventional prosthesis as they may be inserted with less trauma and fewer complications. Diet needs occasionally to be limited to soft or blenderized foods to prevent occlusion. A disadvantage of uncovered SEMS is short duration of palliation due to tumor ingrowth which can be overcome with availability of covered SEMS. Starvation is the most common cause of death in patients with esophageal malignancy. Prosthesis combats deterioration and leads to rapid weight gain. Overall, single time procedure without general anaesthesia, short hospital stay and immediate improvement in dysphagia are considerable gains.
Records of one hundred and two patients with idiopathic ulcerative colitis were retrospectively studied to estimate the prevalence of associated tuberculosis. There were 49 males and 53 females with a mean age of 33 +/- 13 years. Seven patients (3 pulmonary and 4 intestinal) had associated tuberculosis. All patients with intestinal tuberculosis were females and had small bowel involvement. Patients with co-existing tuberculosis were on steroids at the time of presentation and responded well to anti tubercular treatment with gradual weight gain and remission of diarrhoea and fever. We conclude that in India, the prevalence of tuberculosis is high in patients with idiopathic ulcerative colitis on steroids and a high degree of suspicion is required to detect these cases.

Encouraging results of Transluminal Angioplasty for Budd-Chiari Syndrome due to obstruction of Suprahepatic inferior vena cava have been reported in the last decade. We present our experience with two such patients, treated with balloon dilatation producing dramatic clinical improvements and control of disease on follow-up.
