A survey of hemoglobinopathies which was carried out in the Takamatsu district during the period from January to August 1979 detected six families with abnormal hemoglobins. Approximately 6010 inhabitants were screened. Three of these families had the same new Hb variant (Hb Takamatsu beta 120 Lys leads to Gln) that has not been previously reported. Existence of a blood relationship among these three families could not be established even after careful family studies. This abnormal hemoglobin was not associated with adverse symptoms and gave normal hematologic findings in the carriers. The isopropranol test was negative, oxygen affinity was within the normal range, and biosynthetic ratio in reticulocytes was around 1.0. One of the difficulties in the structural analysis of this hemoglobin was related to complete superposition of abnormal beta XT-12b,13 on a beta T-8,9 peptide in the fingerprint of the trypsin digest of aminoethylated aberrant beta X chain. This was overcome by collection of abnormal tryptic beta core (beta XT-10-13) from unmodified beta X chain, and subsequent digestion by chymotrypsin. Edman analysis of the chymotryptic peptides thus obtained successfully confirmed the substitution to be beta 120 Lys leads to Gln.

Clinical study of sustained release cephalexin (S-6435 and S-6436) was conducted with the following results: 1) Pediatric patients with 1.5 approximately 12 years of age having acute tonsillitis and pharyngitis were orally administered 500 approximately 1,000 mg/day of S-6435 or S-6436 divided into 2 doses after breakfast and dinner. Out of 21 patients receiving the preparations, 19 patients satisfactorily responded to them and 2 fairly did. 2) In only one patient of the 21, rash was observed during the treatment. The side effect, however, disappeared 2 days after the initiation of the treatment, and was considered to be due to underlying disease of the patient. No other side effects were observed. Since it has become our concern to conduct injectable therapy in children, oral therapy should first be recommended to them. However, oral drugs, like a regular cephalexin, requiring q.i.d. regimen are inconvenient in giving the drugs to patients because they have to take the drugs once at midnight. S-6435 and S-6436, however, have no such disadvantages and prove to have as much effectiveness and safety as regular cephalexin has.
Twelve cases of peptic ulcer with diabetes mellitus were found in 165 hospitalized diabetics. All of them had gastric ulcer and no duodenal ulcers were found. The incidence of peptic ulcer in diabetics was comparatively higher than the previously reported series. But there was nosignificant correlation between the duration of diabetes and the onset of gastric ulcer. The gastric ulcer with poorly controlled diabetes showed more intractability than those without triopathy and well-controlled diabetes.

A method for effective elimination of drug resistance (R) and sex (F) factors in Escherichia coli K-12 strains by treatment with sodium dodecyl sulfate (SDS) is presented. Growth of E. coli harboring R or F factors in Penassay Broth containing SDS led to the loss of all or part of these genetic elements. Appearance of drug-susceptible or F(-) cells among survivors was observed after the culture reached the stationary phase. Drug-susceptible cells which had lost all of their resistance markers by SDS treatment could be efficiently infected with R or F factors. Among isolated segregants which came from resistant cells, tetracycline-susceptible cells were the major segregant class. Drug-susceptible cells gave no revertants to drug resistance. By treatment of F(+) cells with SDS, unusual F(+) cells which retained mating ability but showed resistance to M12 phage were also isolated, together with mutants of another type which lost mating ability but retained sensitivity to M12 phage. Since SDS is more toxic to R(+) cells than R(-) cells, the isolation of drug-susceptible or F(-) cells under these conditions may be partly attributable to selective growth of drug-susceptible or F(-) cells in SDS-Penassay Broth.

BACKGROUND: Global and regional ventricular function may be evaluated by using gated myocardial perfusion single photon emission computed tomography (SPECT). This study investigated two parameters of regional contraction of the left ventricle, segmental wall motion (WM) and wall thickening (WT), to determine their similarity and disparity in each myocardial segment in patients with normal myocardial perfusion. METHODS AND RESULTS: Thirty-five patients with normal myocardial perfusion and cardiac function (mean left ventricular ejection fraction, 62.6%+/-8.8%) were included in this study. A 1-day stress/rest protocol was used as a means of acquiring technetium 99m (Tc-99m) sestamibi gated SPECT protocol for each patient. A commercially available software package for quantitative gated SPECT (QGS) was used to generate cine loop three-dimensional surface display and SPECT images. The left ventricle was divided into 9 segments to score WM and WT (on a scale of 0 to 4, with 0 being normal and 4 being severely reduced) by 6 independent observers. The WM score was significantly higher than the WT score in the septum, whereas the WM score was lower than the WT score in the inferior segment. Similar WM and WT scores were observed in the remaining segments. CONCLUSIONS: Heterogeneous myocardial WM and WT were observed by using QGS software. These findings suggest that different criteria are required in each segment to evaluate segmental WM and WT by means of gated myocardial perfusion SPECT.
Human mucosal microvascular endothelial cells (HMMECs) were isolated and cultured from the mucosa of nasal inferior turbinates. These cells were identified by human factor-VIII antigen and UEA-I. We also investigated alterations in intracellular calcium concentration [Ca2+]i, in HMMECs using the Ca(2+)-sensitive dye fura-2. Depolarization by high K+ (150 mM) induced no changes in [Ca2+]i in HMMECs, thereby indicating the lack of voltage-dependent Ca2+ channels in these cells. Extracellular ATP increased [Ca2+]i in HMMECs both in the presence and absence of extracellular Ca2+. These results suggest that ATP may induce a release of Ca2+ from intracellular stores in HMMECs. Adenosine induced no changes in [Ca2+]i in HMMECs. It is thus likely that HMMECs obtained from the nasal inferior turbinate may carry P2-purinoceptors but not P1-purinoceptors.
An increased number (density) or sensitivity (affinity) of histamine H1 receptors could cause an increase of allergic symptoms. Using the specific H1 receptor blocker 3H-mepyramine, we have demonstrated for the first time an H1 receptor number (Bmax) and a binding affinity (Kd) in guinea pig and human nasal mucosa. H1 antihistamines inhibited that 3H-mepyramine binding sites represent H1 receptors in nasal mucosa. Bmax and Kd values were not significantly different among the nonallergic sinusitis, vasomotor rhinitis, or the nasal allergy group, suggesting that H1 receptors do not change quantitatively and qualitatively in allergic nasal mucosa. There was no significant correlation between H1 receptor number and clinical data (IgE, peripheral eosinophils, RAST). These data suggest that H1 receptor number and affinity are not main etiological and pathophysiological factors in nasal allergy.
