[Changes in the carotid sphymogram and apex cardiogram in the mesosystolic click-telesystolic apical murmur syndrome].

Synchronous records of ECG, PhCG, CSG and ACG were made of 25 patients with the syndrome of mesosystolic click-telesystolic apex murmur as well as of 62 subjects with healthy hearts. Mesosystolic retraction in CSG was found in 14 patients (57%) and incisura of the ACG systolic wave in 22 patients (88%). Similar changes were not established in the subjects with healthy hearts. The possibility of explanation of the changes found are discussed and those CSG and ACG changes are concluded to be useful as additional bloodless methods in the diagnosis of the syndrome of mesosystolic click-telesystolic apex murmur.

Application of the apexcarotis diagram in ischaemic heart disease with angina pectoris and myocardial infarction.

The apexcarotis diagram is a new mechanocardiographic method, uniting into a single diagram synchronous recordings of the carotidogram and apexcardiogram. Characteristic features of the apexcarotis diagram (ACD) in ischaemic heart disease with angina pectoris are the reduction of the area of ventricular ejection with a turn to the right toward the point S (p less than 0.001) and an extension of the diastolic subsegment A by more than 10% (p less than 0.001). In patients with myocardial infarction, in addition to the above changes, there occur a reduction of the area of ventricular filling (p less than 0.01) and an enlargement of the angle alpha (p less than 0.02). In 8 patients with angina pectoris and 7 patients with myocardial infarction, the coronary disease was established by coronary angiography as well as by catheterization of the right and left heart. In these patients, a reduction of the area of ventricular ejection was found, which was closely correlated with the elevation of left ventricular enddiastolic pressure (r = -0.72, p less than 0.001).

[Assessment of left ventricular function in hypertension by means of phase analysis].

Polygraphic records of ECG, PCG, carotissphygmogram and apexcardiogram were performed and 16 systolic and diastolic intervals were taken in 86 patients with hypertonic disease, distributed according to the stage of the disease according to Mjassnikov classification as well as in 62 subjects with intact hearts. A lengthening of asynchronous contraction was established as well as of the isovolumetric contraction, pre-blood driven period, isovolumetric relaxation and shortening of the left-ventricle time of blood driving, the ratio pre-blood driven peroid/left ventricle time of blood driving showing the most pronounced changes. Regression and correlation analysis in the comparison of those changes in the intervals with a means blood pressure in patients from the three stages of hypertonic disease, show that the best correlation is obtained in the patients from the III stage. The results obtained are discussed and a conclusion is drawn that the ratio pre-blood driven period/left ventricle time of blood driving is with the highest practical importance from the systolic and diastolic intervals in the assessment of the adequacy of the left-ventricle compensation.

[Apex carotis diagram and benign systolic murmurs].

The apex-carotis diagram is a new mechanocardiographic, non invasive method consisting of integration of the carotidogram and apexcardiogram--synchronously recorded. The value of the amplitude height of these curves at every 0.025 sec are transferred to an orthogonal coordinate system and later the points connected in a chronological order thus obtaining the apex-carotidogram (ACD). ACD have been constructed in 62 healthy subjects and in 26 people with innocent systolic murmur, aged between 14--65 years. Our investigation showed no difference between the control group and the people with innocent murmur. The points of connection, lines, segments, fields and axis of ACD were the same in both groups. The angles alpha, beta, gamma, delta, epsilon measured quantity and calculated showed no statistical difference (p greater than 0.05).

Assessment of left ventricular contraction and relaxation by systolic and diastolic time intervals measured from the first derivative of apex cardiogram in cardiomyopathy.

To determine the usefulness of the time intervals obtained from the first derivative of apex cardiogram (dA/dt) in assessing contraction and relaxation in cardiomyopathy, 11 patients with hypertrophic obstructive cardiomyopathy (HOCM) and 9 with congestive cardiomyopathy (COCM) with hemodynamically and angiographically documented diagnosis were studied. The control group numbered 50 normal subjects. Since contraction and relaxation are dependent on preload and afterload, the time interval from R wave of the electrocardiogram to the positive peak of dA/dt (R to dA/dt) and two relaxation parameters derived from the negative peak dA/dt, as well as early apex cardiographic relaxation index EARI) and total apex cardiographic relaxation index (TARI) were determined in all the subjects investigated. In HOCM the R to dA/dt were shortened (55 +/- 13 msec vs. 76 + 14 msec for controls, p less than 0.01) and both EARI and TARI were augmented (EARI: 11.5 +/- 9 vs. 4.3 +/- 5 in controls, p less than 0.001; TARI: 127 +/- 29 vs. 70 +/- 18 in controls p less than 0.02). In COCM the R to dA/dt were elongated (124 +/- 14 msec vs. 76 +/- 14 msec in controls, p less than 0.001) and both EARI and TARI were decreased (EARI: 2.4 +/- 4 vs. 4.3 +/- 5 in controls, p less than 0.005; TARI; 43 + 17 vs. 70 +/- 18 for controls, p less than 0.02). A significant correlation between these indices and some internal indices of myocardial performances was demonstrated. These findings indicate that the interval R to dA/dt, EARI and TARI may be used as reliable indices for the evaluation of contraction and relaxation in cardiomyopathy, independently of preload and afterload.
