Phosphoramidon inhibits the intracellular conversion of big endothelin-1 to endothelin-1 in cultured endothelial cells.

Effects of various protease inhibitors on the conversion of big endothelin (ET)-1 to ET-1 in cultured endothelial cells were analyzed. A metal protease inhibitor, phosphoramidon, decreases the amount of ET-1 and increase that of big ET-1 released. This effect is dose-dependent and not nonspecific. When the contents of ET-1 and big ET-1 in the cells after culturing in the medium with or without phosphoramidon were measured, the ratio of ET-1: big ET-1 in the cells was 3.3 : 1 and phosphoramidon inverted the ratio in the cells to 1 : 3.5. These data strongly suggest that a phosphoramidon-sensitive protease converts big ET-1 to mature ET-1 intracellularly.

[Preoperative discrimination between benign and malignant ovarian tumors by multivariate analysis].

To obtain a precise preoperative diagnosis of benign and malignant ovarian tumors, multivariate analysis was performed using a combination of tumor markers, diagnostic imagings, clinical findings and epidemiological data. Twenty-eight factors were selected for a discriminant analysis, which included 10 factors for tumor markers, 8 for diagnostic imagings, 4 for clinical features and 6 for epidemiological data. On 235 patients (malignant: 100, benign: 135), the usefulness of the discriminant analysis was evaluated retrospectively. As a results, an accuracy of preoperative discrimination was 82% for a discriminant analysis of tumor markers alone, 91% for diagnostic imagings, 84% clinical findings and 65% for epidemiological data. Twenty-one factors were selected by chi-square test or T-test. When 21 factors were used, the sensitivity and the specificity for the diagnosis of malignant ovarian tumor were 98% and 88% respectively. The results indicate that the discriminant analysis is valuable for preoperative discrimination of patient with ovarian tumors.

Analysis of big endothelin-1 digestion by cathepsin D.

Digestion of big endothelin (ET)-1 by cathepsin D, which is the only substantially identified protease showing ET converting enzyme activity, was characterized. Increased doses of cathepsin D showed decrease of immunoreactive (ir-) ET produced from big ET-1. Time course of big ET-1 conversion showed marked increase of ir-ET in a relatively short period, but further incubation resulted in the decrease of ir-ET. Incubation at various pHs with different doses of cathepsin D revealed that low doses of cathepsin D yielded the maximum production of ir-ET at pH 3.5-4.0, but higher doses of cathepsin D showed a bimodal curve of ir-ET production, which may be due to degradation of ir-ET. HPLC analysis revealed that cathepsin D cleaves Asn18-Ile19 bond in addition to Trp21-Val22 bond of big ET-1. These data suggests cathepsin D is not a physiological endothelin converting enzyme.

[Dynamic CT under angiotensin II-induced hypertension--application for the differential diagnosis between benign and malignant solid ovarian tumors].

Dynamic CT was applied to 9 patients with solid ovarian tumors of an unknown nature after ultrasonography and tumor marker tests with CA125, TPA and IAP. Dynamic CT was performed under the condition of normotension and angiotensin II (AT II)-induced hypertension. The results obtained were as follows. 1. Intratumoral blood flow increased 25 percent in patients with malignant tumors, decreased 7 percent in tumors of low potential malignancy, and increased 6 percent in patients with benign tumors due to the pressor effect of AT II. 2. Changes in intratumoral blood flow were not uniform in various parts of a malignant tumor. 3. Differential diagnosis of malignant and benign tumors and tumors of low potential malignancy was possible by this method. 4. The results indicated that chemotherapy under AT II-induced hypertension would have a better effect on the treatment of malignant ovarian tumors.

Induction of cytotoxicity of peritoneal exudate cells by agrimoniin, a novel immunomodulatory tannin of Agrimonia pilosa Ledeb.

The cytotoxic activities of the PEC after an i.p. injection of agrimoniin, a tannin contained in Agrimonia pilosa Ledeb. were studied. The plastic nonadherent PEC had significantly higher NK cell activity than the untreated control, and the adherent PEC were cytostatic toward MM2 and MH134 cells. The adherent PEC did not cause tumor cell lysis by themselves, but were cytolytic against MM2 cells in the presence of anti-MM2 sera. In the course of these effects of PEC after the i.p. injection of agrimoniin, the augmentation of NK cell activity was the earliest reaction, reaching a peak at 2 days after the injection; then, cytostatic activity increased. The induction of antibody-dependent cell lytic activity was a later reaction, which reached a peak at 6 days after the injection.

Antitumor activity of cisplatin analogs against malignant ovarian tumor xenografts into nude mice.

The antitumor activity of new platinum analogs was studied at equitoxic doses to that of cisplatin (CDDP) in BALB/C nu-nu nude mice bearing xenografts of human ovarian malignant tumors. The following two tumor lines were used: OH-1 (serous cystadenocarcinoma) and MP-1 (mucinous cystadenoma, LPM). 1) Antitumor activity was determined for all agents from the T/C ratio for OH-1. In particular, 254-S and JM-8 were found to be much more active than the other agents. 2) Antitumor activity was determined with JM-8 from the abdominal diameter ratio for MP-1. Both JM-8 and CDDP yielded active responses which were evaluated by the CEA value. Therefore, JM-8 and CDDP were considered to be the most active agents for MP-1. 3) We evaluated the mean body weight loss of tumor bearing mice in order to study the side effects, and a great weight loss was observed with 254-S. 4) A large variety of effects were observed in the histological evaluation for the two tumor lines, and were thus difficult to discuss. Our findings suggest the necessity of individual chemotherapy for each histological type of ovarian malignant tumor.

A preliminary study of a combination chemotherapy with ifosfamide, adriamycin and cisplatin for endometrial carcinoma.

Seven patients with uterine endometrial carcinoma received a postoperative combination chemotherapy with ifosfamide (1g/m2) daily for 5 days, and adriamycin (50mg/m2) and cisplatin (50mg/m2) on day 1 (IAP). All except one patient with the disease without myometrial invasion received 5 courses or more of IAP treatment. Of four patients with measurable disease, three achieved complete response (CR) and the other patient obtained partial response (PR), indicating a 100% response rate. In the PR case, the patient had previously been treated by radiation treatment for cervical cancer, and the histology of the tumor was papillary serous adenocarcinoma, a malignant variant of endometrial cancer. Six patients underwent second look operation following the planned IAP treatment courses, and the operation confirmed no pathological evidence of disease in any, although in one disease recurred 7 months after the second laparotomy. Side effects were remarkable, including an 87% incidence of grade 4 leucopenia. However, the patients recovered spontaneously from the hematologic toxicity within 3 weeks of treatment. Other toxic effects including central nervous system toxicity were controllable and acceptable. The results indicated that IAP combination therapy was feasible and promising in the management of patients with endometrial carcinoma.

Antitumor effect of agrimoniin, a tannin of Agrimonia pilosa Ledeb., on transplantable rodent tumors.

The effect of agrimoniin, a tannin contained in Agrimonia pilosa LEDEB., on ascites type and solid type rodent tumors was investigated. When agrimoniin was intraperitoneally (i.p.) administered at dosages over 10 mg/kg before or after the MM2 cell i.p. inoculation, this tannin almost completely rejected the tumor growth in the mice. This tannin prolonged the life span of mice bearing MM2 or cured by the intravenous or per oral pre- or postmedication. Agrimoniin also inhibited the growth of MH134 and Meth-A solid type tumors. Agrimoniin showed strong cytotoxicity on MM2 cells in vitro, but the activity was diminished to about 4% of the initial activity by the addition of fetal calf serum to the culture. On the other hand, i.p. injection of agrimoniin increased the number of peripheral white blood cells and the ratio of monocytes. On the 4th day after the i.p. injection of the tannin, cytotoxic adherent peritoneal exudate cells were also increased. The spleen of the mice was enlarged, and the spleen cells possessed the capacity to take up 3H-thymidine. Agrimoniin showed weak direct migration activity against spleen cells from non-treated mice. These results indicate that agrimoniin is a potent antitumor tannin and suggest that the antitumor effect may be due to this tannin enhancing the immune response of the host animals through the actions on tumor cells and some immunocytes.

Study on abnormalities in the appearance of finger and palm prints in children with cleft lip, alveolus, and palate.

The aetiology of abnormalities in the appearance of finger and palm prints in children with cleft lip, alveolus, and palate was examined by comparing the frequencies of abnormalities in hereditary cases and sporadic cases, and those in hereditary cases with those in their parents. The following results were obtained: Comparison of hereditary cases and sporadic cases. Abnormalities in the appearance of finger and palm prints were greater in hereditary cases than in sporadic cases. Abnormalities in the appearance of finger and palm prints were greater in parents of hereditary cases than in parents of sporadic cases. The rate of agreement in the appearance of finger and palm prints between parents and children was greater in hereditary cases than in sporadic cases. Comparison within hereditary cases. Abnormalities in the appearance of finger and palm prints were greater in affected children when one of the parents was affected (gamma = 1/2) than when both parents were normal (gamma = less than 1/2). Abnormalities in the appearance of finger and palm prints were greater in parents, one of whom was affected, than in normal parents. The rate of agreement in the appearance of finger and palm prints of parents, one of whom was affected, and their gamma = 1/2 children cases was higher than of normal parents and their gamma = less than 1/2 children. It was concluded that the above results could be explained by multifactorial inheritance.

Diagnosis of breast lesions by aspiration biopsy cytology.

From November 1981 to the end of August 1984, 456 patients with breast lesions underwent aspiration biopsy cytology (A.B.C.). This study includes 109 for whom the diagnosis was histologically confirmed at surgical biopsy. Seventy-five lesions were histologically proven to be malignant and 34 were benign. The accuracy of diagnoses with A.B.C. was; true positive 86.7 per cent (65/75) of the time, true negative 82.6 per cent (28/34) of the time, false negative 5.3 per cent (4/75) of the time and false positive 5.7 per cent (2/34) of the time. Unsatisfactory or inadequate aspirated tissue made A.B.C. diagnosis difficult in 5.5 per cent (6/109) of the cases. Three out of 7 with malignant tumors, who were wrongly diagnosed as benign by A.B.C., had tumors with a diameter of 1.0 cm or less. Two benign cases which were falsely diagnosed as malignant also had small tumors about 1.0 cm in diameter. Although A.B.C. is more reliable than other conventional supplementary diagnostic techniques (mammography, ultrasonography, etc.), it is important to carefully follow clinically questionable cases which appear negative, using A.B.C..
