[The value of limb-saving surgery in the treatment of osteosarcoma].

Since 1981, 38 limb-saving operations for malignant bone tumors have been performed. Of these, 22 cases were osteosarcomas. The principle of limb-saving surgery for osteosarcoma is the same as for curative wide resection in cases of soft tissue sarcoma. Among the osteosarcoma cases for which limb-saving procedures were carried out, five complicated cases (two skip metastases and two infections) were noted. However, in the other cases, patients had better functional benefits and psychological acceptance compared with those subjected to radical ablative procedures. Indication for limb-saving surgery is usually decided by patient age, existence of skip metastasis and venous infiltration, effect of preoperative adjunctive therapy and the relationship of the tumor with the major artery necessary for preserving the affected limb. If the adjunctive therapy is effective, local control can be achieved by establishing a wide margin.

[Surgical treatment of malignant soft tissue tumors].

The ultimate survival of patients with soft tissue sarcoma is determined by a number of factors. Radical removal by adequate surgery is one of the most important factors together with early treatment and chemotherapy. We usually select curative wide resection, amputation, or resection after radiotherapy as forms of radical surgery for soft tissue sarcomas according to each clinical stage. The method of curative wide resection is based on biological barrier effects. In this report we discuss the operative results obtained in 148 cases of soft tissue sarcoma which we have treated over the past ten years, and also discuss the causes of recurrence after radical operation. Among 55 primary NoMo which were treated by the curative wide resection cases, the recurrence rate was 5.5%, the metastatic rate was 21.8%, and 5-year survival was 79.3%. These results were better than those for 30 recurrent and additional NoMo cases. Of cases involving the extremities, 81% were controlled by limb-saving operations.

[Combination chemotherapy including cis-platinum in Ewing's sarcoma].

A clinical study of cis-platinum was conducted on 5 patients with Ewing's sarcoma. Cis-platinum was preoperatively administered at doses of 70-100 mg/m2. Good response on imaging diagnosis was recorded in all cases. With regard to side effects, 2 of the 5 patients suffered from hearing loss, but only slight bone marrow suppression was observed. We concluded that cis-platinum was effective for the preoperative treatment of Ewing's sarcoma.

Occurrence of Gal beta (1----3) GalNAc-Ser/Thr in the linkage region of polygalactosamine containing fungal glycoprotein from Cordyceps ophioglossoides.

The nitrous acid deamination liberated the N-acetyl galactosamine containing glycoproteins from galactosminoglycan (CO-N) which has been isolated from Cordyceps ophioglossoides culture. Mild alkaline borohydride degradation of the purified glycoprotein released oligosaccharide alditols. The smallest oligosaccharide alditol was characterized to be Gal beta (1----3)-GalNAcol. This result indicated that polygalactosamine part attached to protein part via Gal beta (1----3) GalNAc-Ser/Thr as the linkage region. To our knowledge, this is the first report of Gal-GalNAc-Ser/Thr linkage from fungal glycoprotein.

Developmental changes in the inner surface structure of the bovine large intestine.

The developmental pattern of the bovine fetal large intestine was studied with particular reference to the appearance and decline of the intestinal villi during the fetal period. In the bovine large intestine, the first rudimentary villus and goblet cells were seen in the rectum in a fetus estimated to be 3 months old. By 5-6 months, the goblet cells, absorptive cells in the intestinal crypts, and vacuolated cells in the villi were present along all segments of the large intestine. By 8-9 months, the villi have disappeared from the colon and rectum, epithelial cells no longer contain vacuoles, and absorptive and goblet cell populations are emerging from the crypts. These histological results suggest that development in the bovine large intestine follows a recto-cecal gradient and the most distinct turning point during the fetal period is the first disappearance of fetal villi in the rectum of fetuses estimated to be 7 months old. After this stage, the mucous membrane of the colon and rectum matured rapidly before birth. In contrast, the cecum may seem to require further development in perinatal life.

Cloning and sequencing of cDNA of Sarcophaga peregrina humoral lectin induced on injury of the body wall.

A previous paper described the purification of a lectin induced in the hemolymph of larvae of Sarcophaga peregrina (flesh-fly) on injury of their body wall (Komano, H., Mizuno, D., and Natori, S. (1980) J. Biol. Chem. 255, 2919-2924). This paper describes cDNA cloning and the complete nucleotide sequence of the gene for Sarcophaga lectin. Although active lectin consists of alpha and beta subunits in a molar ratio of 2:1, the fat body of injured larvae was found to contain only mRNA for the alpha subunit, suggesting that these two subunits are derived from a common gene and that the alpha subunit is converted to the beta subunit post-translationally. The alpha subunit was found to consist of 260 amino acid residues with an additional signal sequence of 19 or 23 amino acid residues.

[Multidisciplinary treatment of bone and soft tissue sarcomas].

The result of the group study on adjuvant chemotherapy with HD-MTX, ADR and CPM for 25 cases of osteosarcomas is as follows: Overall survival at three and a half years is 58.4% and the disease-free survival is 40%. In our clinics, 66 cases of osteosarcomas were treated by the radical surgery from 1955. Overall survival of these cases at five years was 49%. Overall survival between 1955 and 1974 was 43% and that from 1975, the year when ADR was introduced, to date was 55%. The data of the group study of soft tissue sarcoma sponsored by Ministry of Health and Welfare show that in 318 cases of soft tissue sarcomas of extremities, the recurrence rate during the period between 1962 and 1976 is 52%, metastasis rate 69%, and overall survival at 5 years 42%. From 1972 to 1983, in 414 cases treated by the orthopaedic clinics of the same group the recurrence rate become as low as 23% metastasis rate 45% and overall survival at 5 years 56%. In our clinics, 89 patients with soft tissue sarcomas were treated by the curative wide resection and the recurrence rate is 12%, metastasis rate 25%, and overall survival at 5 years 78%. In this series, in 80% of cases limb salvage is succeeded.

Intracellular localization of two distinct Ca2+-proteases (calpain I and calpain II) as demonstrated by using discriminative antibodies.

Intracellular localization of two molecular species of calpain (Ca2+-dependent cysteine proteinase) was studied by immunocyto- and histochemical methods employing antibodies strictly monospecific for the respective antigens. Apparent immunological cross-reactivity between the larger subunits of calpain I (low Ca2+-requiring form) and calpain II (high Ca2+-requiring form) was calculated to be 15-17%, and two steps of affinity chromatography were needed to obtain antibodies which can discriminate between the two proteases. Indirect immunofluorescent staining of cultured PK 15 cells revealed diffuse staining of the cytoplasm with both antibodies against calpain I and calpain II. Preincubation with Ca2+-ionophore had no effect on the staining patterns. Sections of porcine kidney were stained by the avidin-biotinylated peroxidase complex method. The proximal and distal tubules and collecting duct were stained, but the glomerulus, macula densa, and vascular vessels were not stained by either anti-calpain I or anti-calpain II antibodies.

Structure and antitumor activity of an alkali-soluble polysaccharide from Cordyceps ophioglossoides.

A water-insoluble extracellular glucan (CO-1) was isolated from the precipitate formed on incubation of the culture filtrate of Cordyceps ophioglossoides at 37 degrees for 19 h. CO-1 was homogeneous as judged by h.p.l.c., electrophoresis, and ultracentrifugation, and the average molecular weight was determined by h.p.l.c. to be 632,000. The 1H- and 13C-n.m.r. and the i.r. spectra indicated that the glucosidic linkages in CO-1 were beta. From the results of methylation analysis, Smith degradation, n.m.r. studies, and enzymic hydrolysis, it was concluded that CO-1 is composed of a backbone of (1----3)-linked beta-D-glucopyranosyl residues with a beta-D-glucopyranosyl group attached to O-6 of every second D-glucopyranosyl residue of the backbone. CO-1 strongly inhibited the growth of Sarcoma 180 solid-type tumor. CO-1 polyalcohol, which was prepared by Smith degradation of CO-1, exhibited more-potent antitumor activity than CO-1. The absorption maximum of Congo Red shifted significantly in the presence of CO-1.

[Curative limb saving operation (curative wide resection)].

The recent results of surgical treatment of soft tissue sarcomas have dramatically improved. The result of the group study of soft tissue sarcomas, which was supported by the Ministry of Health and Welfare (1962-1976) showed that the local recurrence rate was 50%, and the metastatic rate was 60%. In contrast with this result, the recent results of our curative wide resection showed the local recurrence rate of 10%, and the metastatic rate of 30%. These procedures are based on the clinico-pathological findings which we studied. Curative wide resection is a method of radical wide resection, similar to radical local resection (reported by Enneking) and radical local excision (reported by Stenar). Our procedures are the least aggressive of the three radical wide resections, but our results are the same as the other wide resection methods. To obtain good results with these procedures, accurate preoperative diagnosis and skillful surgery are necessary.

Juxtaglomerular cell tumor. With special reference to the tubular component in regards to its histogenesis.

A left kidney tumor was found in a 33 years old female with diastolic hypertension and hyperreninemia. A yellowish white 6 X 4 cm tumor with capsule was located in the upper pole. Polygonal mesenchymal cells with Bowie-positive and rhomboid-shaped granules and occasional tubular component were identified. The diagnosis of J-G cell tumor was made. The first electron microscopic observation of the tubular epithelium showed immature features. 50-100 A microfilaments in the epithelium were identical to those reported in epithelium of Wilms' tumor. The results support that the tubular component is a tumor constituent and that the case is a biphasic tumor. Biphasic pattern and clinical characteristics of reported J-G cell tumor with tubular component are those of mesoblastic nephroma and adult multilocular cystic nephroma. Therefore, J-G cell tumor with tubular component would have to be classified as a metanephric blastema origin tumor as like Wilms' tumor, mesoblastic nephroma, and multilocular cystic nephroma.
