New trial of endoscopic treatment for gastric submucosal tumors.

Recently conservative treatment of gastric submucosal tumors is increasing in spite of the absence of definite criteria for histological diagnosis. We have developed a new method of endoscopical treatment of gastric submucosal tumors that have submucosographically intraluminal Pattern I or II, or partly intramural Pattern III. For such tumors with a maximum diameter less than 5 cm, Ist step: partial resection is carried out with the use of high-frequency electric currents, IInd step: for the residual tumor tissue, in vivo tissue solidification is undertaken by local injection of absolute ethanol to induce necrotic exfoliation of the tumor tissue and to prevent of hemorrhage.

Diagnosis of the depth of cancer invasion by submucosography.

A new method was developed to diagnose the depth of cancer invasion using submucosography combined with special charcoal suspension containing mitomycin C, and applied to 8 cases of gastric cancer. Correct diagnosis of the depth of the cancer invasion was obtained in 7 of the 8 cases. As some cases of early gastric cancer can be treated endoscopically, it is important to know the condition of the invasion before any treatment is performed.

Ultrasonic endoscopy.

With the use of a linear scan type ultrasonic endoscope, the possibility of gastric fiberscopy with ultrasonic procedure in the estimation of the extent of deep infiltration of gastric cancer and in the transgastric diagnosis of pancreatic tumors was studied. Results revealed that the echogenicity of the tumor was more clearly recorded by ultrasonic endoscopy than routine ultrasonography taken via the abdominal wall. With improvements in both the probe and manipulation technique, the device is expected to achieve far-reaching contributions to the diagnosis of the deep invasion of gastric cancer and small-sized pancreatic tumors.

Age-associated change in macronuclear DNA content in Paramecium caudatum.

Macronuclear DNA content in Paramecium caudatum was found to be almost unchanged with a mean of about 400C during the earlier two-thirds of the life span in terms of the number of fissions and then it dropped rapidly to about one-fifth of the initial content. The age when rapid DNA decline occurred corresponded to that when the characteristics of senescence appeared. This decreasing pattern of macronuclear DNA content contrasted with earlier observations in P. tetraurelia, P. bursaria and Tetrahymena thermophila. The data suggested that in P. caudatum the distribution pattern of macronuclear DNA to daughter cells changed from an almost equal distribution in younger cells to an unequal distribution in older cells, while the relative volume of the macronucleus to the whole cell remained almost constant throughout the life cycle.

[Pericardial puncture under the ultrasonic cardiotomographic guidance: application to pericardial effusion].

Pericardial puncture drainage was attempted in five patients with pericardial effusion under the guidance of real time two-dimensional echocardiography (ultrasonic cardiotomography). The needle fixed to the scanner probe was inserted from the anterior chest wall under the observation of ultrasound images. After the needle was placed into the pericardial space, then the drainage catheter was introduced according to the Seldinger technique. The image of the needle should be appropriately monitored from the site of puncture as near as possible because the pericardium is so close to the anterior chest wall. Both mechanical and linear sector scanners were used in this study, and linear type was proved to be useful for this purpose. It was concluded that the pericardial puncture under the guidance of ultrasonic cardiotomography is safer and reliable compared to the conventional blind puncture technique.
