[Vascularised osteocutaneous scapular flap in the reconstruction of mandibular defects after war wounding].

Vascularized osteocutaneous scapular flaps belong into the most convenient ones in reconstruction of mandibular defects followed by massive loss of adjacent skin and mucous membrane due to its vascular supply, bulkiness, suitability and mobility of cutaneous component of the flap. In seven wounded patients such defects of the mandible and adjacent soft tissues after war wounding have been reconstructed at the Clinic for Maxillofacial Surgery of the Military Medical Academy - Belgrade during the six years period with vascularized osteocutaneous scapular grafts. Here we present localization and structure of the defect, features of harvested compound graft, the procedure of reestablishment of mandibular continuity, immediate and late complications during the consolidation period, as well as the analysis of the success rate.

[Surgical treatment of rare anatomic variations and anomalies of the thyroid gland].

Choristoma and thyroid gland malpositions are embryonal disorders. The aim of this article is ten years analysis of their diagnostic and operative treatment. In 1425 thyroidectomies we have found: one lingual, intratracheal, intracardial thyroid and ovarial struma; six separated intrathoracic goitre and 10 mostly intrathoracaly (80%) situated goitre; 1.4% out of all. Correct preoperative diagnosis we have in lingual thyroid, two intrathoracal thyroid and in all mostly intrathoracal thyroids, and in others after histopathological examination. Operative approach was: collar incision with medial sternotomy in nine, collar incision with lateral thoracotomy in three, right thoracotomy in two, and only collar incision in one. There were two operative complications, one recurrent nerve lesion and one mediastinal haematoma. There was no hypocalcaemia and mortality. Thyroid gland anomalies are very rare and associated with lot of problems in diagnostic and treatment. In specialised institutions frequency of operative complications is the same like in routine thyroid gland surgery.

Seizure recurrence after a first generalized tonic-clonic seizure, in children, adolescents and young adults.

A sample of 78 patients (32 females and 46 males) who had a first unprovoked generalized tonic-clonic seizure between the age of 3 and 21 years was studied prospectively. Duration of follow-up was 2-10 years (mean 5.2 years). A second seizure occurred in 69.2% (54 of 78), most commonly (38 of 54, 70.37%) in the first three months after the first seizure. There were no significant differences in the total number of relapses among various aetiological groups. For idiopathic aetiology, seizure recurrence was significantly more common if the first seizure occurred during sleep (24 of 29, 82.75%) than in the waking state (5 of 13, 17.25%). The second seizure occurred in the same state, i.e. night sleep or awake in 72.2% (39 of 54) of patients. The presence of epileptiform patterns in the first two EEGs in the waking state or in sleep was significantly associated with a highly increased risk of seizure recurrence.

Frontoethmoidal fractures as a result of war injuries.

Ninety frontoethmoidal fractures, as a result of war injuries from 1991 to 1994, produced 0.7% of all war injuries (i.e., 9% of head and neck injuries treated in the Military Medical Academy, Belgrade). These injuries were mainly part of complex endocranial, orbital, and zygomaticomaxillary injuries, and were rarely isolated. The central part of these injuries were in the ethmoidal region, which was isolatedly injured in 23.3% of the cases. There was 53.3% injuries with small fragments of explosive devices inside the wound, and 16.7% of through-and-through wounds were caused by sniper bullets. The priority over all definitive operative procedures belonged to osteoplasties whenever possible, because the primary goal was protection of life. About 3.3% of the patients died because of severe hemorrhagic shock and endocranial injury. There was only one case of recurrent meningitis. Most of the patients had multiple injuries and were mutually operated on by two different surgical teams.

[A compound odontoma].

Compound odontoma is a malformation manifested as a localized lesion before a non-erupted or displaced tooth consisting of numerous small tooth-like components. The lesion develops asymptomatically. The recognition of its clinical features helps the prevention of deformities and caries in the adjacent teeth by an on time and adequate surgical treatment.

[Large dermoid cysts of the mouth floor].

Dermoid cysts of the mouth floor are very rare. The incidence of these cysts is up to 3.6% of all cysts in the oral cavity. The article presents the material from the Clinic of Maxillofacial Surgery MMA. Eleven cases of large dermoid cyst are discussed. All of them were removed completely and no recurrence developed in longer period of time postoperatively.

Evaluation of ultrasound in the diagnosis of pathologic processes in the parotid gland.

The values of ultrasound in differentiation of pathologic processes in the parotid gland were studied in 61 patients. The ultrasound findings were compared with operative and histologic findings. In 28 patients with chronic inflammatory processes, the ultrasound findings were in complete correlation with the histologic findings, showing enlarged parotid glands, homogenous glandular epithelium, and small, multiple cysts. In 31 patients with a tumor of the parotid gland, ultrasound scanning detected the presence of the tumor. In determining the location of the tumor, the method is highly specific when it is localized in the superficial lobe (100%), and it is less specific when it is localized in the deep lobe of the parotid gland. Also, it is highly specific in determination of tumor size (96%). In two cases with retention cysts, the ultrasound findings also correlated with the histologic and operative findings. The results obtained show that ultrasound is a reliable diagnostic method for detection of pathologic changes in the parotid gland.

[Ultrasound as a diagnostic aid in maxillofacial surgery].

Ultrasonography is a relatively new diagnostic aid in maxillofacial surgery. It can be used in maxillofacial surgery in three modifications: Doppler sonography, A-scan sonography, B-scan sonography. As clinicians we wanted to determine what contribution ultrasound examination has made to the diagnosis of superficial maxillofacial masses, when compared with clinical examination, intraoperative findings and with other methods of investigations. Since 1985 B-scan sonography has been performed in the examination of congenital, inflammatory and neoplastic masses of the head and neck on more than 1500 patients. The ultrasound examination was performed in the Department of Radiology by radiologists. The value of the ultrasound findings varied depending on the soft tissue lesions which were examined and the experience of the radiologists who performed the examination. The advantages of the diagnostic ultrasound are in being noninvasive, without any known deleterious biological effect, rapid, painless, inexpensive and easily reproducible. Ultrasonography is a procedure which nowadays is routinely performed in our department for the preoperative evaluation of our cancer patients and postoperative follow-up as well as for the patients with inflammations, diseases of the salivary glands and soft tissue swellings.

[The (18)(q 22----qter) deletion in patients with complete clinical features of the De Grouchy syndrome].

A case of a 4-year old boy with de Grouchy syndrome was reported. The patient showed generalised muscular hypotonia, marked mental retardation (RQ = 30), developmental milestones retarded, craniofacial dysmorphic features, congenital heart disease, abnormalities of the external genitalia and skeletal deformities. The karyotype analysis revealed a partial deletion of the distal bands of chromosome 18:48 48 xy del (18) (q 22----qter). Qualitative and quantitative characteristics analysis of digital and palmar dermatoglyphics supported the diagnosis.

Pattern-reversal visual evoked potentials in children with migraine: a correlation analysis.

Visual potentials evoked by pattern reversal (PRVEPs) were studied in 64 normal children (age range 7 to 15 years) and in 20 age-matched patients with common and complicated migraine (age range 6 to 16 years). Most of them were studied before and during prophylactic therapy. A Quadristim set (Alvar) was used to present checkerboard patterns on a TV monitor, to amplify the EEG signals and to average and plot the evoked potentials. The potentials were elicited by binocular full-field 2/s checkerboard reversal, recorded from an electrode 4 cm above the inion, and analyzed for latency, amplitude and waveform morphology. The degree of similarity between pairs of PRVEPs was determined by the correlation coefficient. In all our patients no pronounced influence of the disease itself was demonstrated on the PRVEP latencies and amplitudes, but in most of them the correlation coefficients were significantly different from that showing the measure of normal PRVEP variations. Prophylactic medication influenced non-significant increase of the coefficient. This increase seems to be related to recovery from medication in terms of attack frequency. No significant difference in PRVEP results of common and complicated migraine was noticed. The PRVEP distortion can be revealed by the correlation coefficient but not by other PRVEP parameters. Therefore, this parameter may be useful as a sensitive measure of both PRVEP distortion and PRVEP improvement. Our results give further evidence that non-demyelinating disorders, but with ischemic damage and abnormalities in neurotransmitters, can produce measurable changes in PRVEP morphology.

[Reading disorders in children with partial epilepsy].

This study represents a neuropsychological evaluation of reading ability in children with partial epilepsy (PE), aged 7-14 years, of normal intelligence, without neurological deficits and physical handicaps, appropriate environment stimulation and learning opportunities. A control group consisted of 202 healthy school children. Reading disabilities were significantly more frequent in children with partial epilepsy, especially in patients with PE with complex symptomatology. The children from the control group had better results of the tests of both verbal and performance intelligence, but children with reading disabilities had greater performance IQ than verbal IQ. Significant intergroup difference was related to the children aged 7-10 years. Positive evidence of left dominant hemisphere superiority for reading has been shown. Dyslexia was significantly more frequent in patients with left hemisphere epileptic foci, than in those with right hemisphere focal EEG changes.
