Serum vitamin B 12 and folic acid levels in patients with fasciolopsiasis.

Serum vitamin B12, serum and red cell folate concentrations and vitamin B12 absorption were studied in 100 patients with fasciolopsiasis. A mean value of serum vitamin B12 level in the patient group was found to be significantly lower than that of normal subjects and 14% of these patients had serum vitamin B12 level less than 100 pg/ml. Serum UBBC and TBBC levels in the patients were significantly higher than those of the normal subjects. Serum TCI and TCIII increased significantly while TCII decreased. Vitamin B12 absorption was found to be impaired in 3 of 9 patients studied. There was no relationship seen between serum vitamin B12 level and vitamin B12 absorption. The mean values of serum folate and red cell folate levels in the patient group were significantly lower than those of normal subjects. Fifteen of 100 patients (15%) had serum folate level less than 3 ng/ml, while all of them had red cell folate higher than 100 ng/ml. Serum folic acid binding protein levels (FABP) in these patients, were not significantly different from those of normal subjects.

Clinical trial of acebutolol (Sectral) in hypertension.

Acebutolol (Sectral) was used successfully in a clinical trial involving thirty-two hypertensive patients. Twenty-three patients received acebutolol at a dosage of 400--800 mg daily as sole treatment and nine patients received concurrent treatment with thiazide diuretics. In both groups of patients there was a substantial fall in diastolic and systolic pressure to levels within the normotensive range. The combination of acebutolol with thiazide diuretics was a particularly effective form of anti-hypertensive treatment where a greater anti-hypertensive effect was required. Acebutolol did not significantly slow the heart rate. The only side-effect was slight pitting oedema of both legs in one patient. Acebutolol appears to be a useful anti-hypertensive in the management of hypertension.

Coagulation studies in leptospirosis.

Coagulation and serum F.D.P. studies in 10 leptospirosis patients (5, L. bataviae, 2, L. autumnalis, 2, L. australis, and 1, L. akiyami A.)showed prolongation of prothrombin time, partial thromboplastin time and thrombin clotting time, slight low platelet count, depletion of factor V and slightly high serum F.D.P. in only four cases. Four cases had bleeding and one case died because of severe haemorrhage but with only slight changes in coagulation factors and slightly high serum F.D.P. These findings suggested that the damage of capillary endothelium was more pronounced than the coagulopathy which may be due to liver cell damage.

Coagulation studies in falciparum and vivax malaria.

Coagulation factors and serum fibrin degradation products (FDP) were studied in 31 cases of acute falciparum malaria with severe complications, 13 cases without severe complications, and 6 cases of vivax malaria. Seven of 13 cases of acute falciparum infection with severe complications died and the coagulation studies were markedly changed and there were high levels of serum FDP. The degree of changes in coagulation and serum FDP varied with the severity of the symptoms and signs and there were virtually no changes in the cases of vivax malaria. It was speculated that changes might be the result rather than the cause of pathogenic processes in falciparum malaria.
