Radiation-induced malignant melanoma: the dog that did not bark.

A history of exposure to ultraviolet rays, such as those in sunlight, has been strongly associated with malignant melanoma. Gamma rays consist of much more energetic photons, but yet they have much more tenuous links to melanoma. A literature review suggests that relatively little evidence has been adduced for gamma radiation as a cause of malignant melanoma. An exploration of the reasons behind the paradoxical lack of melanoma induction by gamma radiation may yield clues to the etiology of melanoma.

Emergence of resistance to VP-16/cisplatin chemotherapy: study in a lymphoblast model system.

Etoposide (VP-16) and cisplatin are widely used in the treatment of malignancy. It is a common clinical observation that patients may initially respond to this two drug combination but later become resistant to it. Data from the CCL-159 lymphoblast cell line suggests that the emergence of resistance may be by means other than multiple drug resistance gene expression. The data suggest that chemotherapeutic failure may be mediated in part by a relatively inefficient method of drug resistance, the unstable expression of low-level drug resistance by a minority of cells. These results may help explain how patients whose malignancies are initially sensitive to VP-16/cisplatin later develop drug resistance.

Potential use of IgA protease therapy in IgA myeloma patients.

Multiple myeloma is generally not cured with conventional cytotoxic chemotherapy. Biologic agents and bone marrow transplantation have been among the newer avenues explored to increase the cure rate. It is suggested that IgA proteases too may be of value in the treatment of IgA myeloma.

Turner syndrome as a candidate pseudoautosomal disorder.

Turner syndrome has been clearly associated with the absence of an X chromosome, but it remains uncertain how this deletion produces either the range of defects regularly associated with the syndrome or those only occasionally seen. It is of particular interest in this regard that the pseudoautosomal portion of the X chromosome has recently been identified as the location for the GM-CSF receptor gene. It is submitted that Turner syndrome, with its hemizygosity for the psuedoautosome, may be an important model for studying the GM-CSF receptor gene as well as other associated genetic material.

Concise review: genetic bases for sideroblastic anemia.

The best-documented mode of transmission of familial sideroblastic anemia is as an X-linked trait. Mitochondrial mutation has also occasionally been noted in association with sideroblastic anemia, but it is likely to be present more often than has so far been described. It should especially be suspected in cases of inherited multisystem disease, where the pedigrees do not indicate a sex predominance among those affected, and where there is no evidence of transmission by the paternal line. Sporadic cases of sideroblastic anemia may represent mitochondrial mutations either at the germ cell or the somatic cell, and the most sensitive strategy of screening for both would be the study of mitochondrial DNA from circulating erythrocytes. It seems likely that such studies would extend the understanding of mitochondrial function and disease.

Flow cytometric detection of tumor metastases during urologic surgery.

Flow cytometric studies were performed on peripheral blood obtained before, during, and subsequent to urologic surgery. The results suggest that 7 of 11 patients had a brief period during surgical manipulation when they had cells from the primary surgical site in the peripheral blood. Concentration of such cells was generally less than 10(2)/mL even during the intraoperative period. These findings suggest that perioperative tumor seeding is a real concern, and that flow cytometry can be used to study maneuvers to reduce its incidence.
