[A case of successful endoscopic treatment for traumatic bronchial rupture].

Successful endoscopic treatment with fibrin glue for traumatic bronchial rupture was reported. 21-year-old male, who was injured in a traffic accident was brought to ICU in our hospital. Six days after injury, bronchoscopic findings showed a rupture from right truncus intermedius to lower bronchus (B8). We preferred an endoscopic treatment with fibrin glue because of poor respiratory function caused by lung contusion as a complication and those resulted in successfully healing. In general, basic strategy for traumatic bronchial rupture is the earlier diagnosis and surgical treatment. But in a case of high risk for operation, we recommend endoscopic treatment with fibrin glue might be useful.

[Is TLFB method applicable to Japanese drinking population?: a methodological study on measurement of alcohol consumption].

In order to measure alcohol related problems, it is fundamental task to begin with to know the drinking quantity of the people. This study tries to explore the comparative, methodological possibilities of Timeline Follow Back (TLFB) as well as Self-Monitoring (SM) methods to measure drinking quantity, using the sample of Japanese and Sri Lankans. Especially TLFB method has not being so far utilized in Japan for this purpose. This pilot study has done in two stages. In the first stage, it has focused to get data on the correct drinking quantity up to two months period. After analyzing data of the first pilot survey, it was decided to reduce in the second-staged study the time frame to obtain data on drinking quantity only up to one month period. In the second-staged study, 37 Japanese and 36 Sri Lankans living in Japan has been selected. Results show that it is more appropriate SM method than TLFB method to investigate drinking quantity of the Japanese. Further it was evidenced that in order to employ TLFB method to investigate correct drinking quantity of the general Japanese population, it has to be further improved for overcoming ambiguity of respondents' memory on their alcohol consuming behavior. Moreover, it is understood that some characteristic drinking behaviors, influenced by Japanese drinking culture may be another barrier in getting correct drinking quantity data. However, the hypothesized notion of this cultural barriers was not supported. An alternative hypothesis "reflexive sensitivity to drinking", was implied.

Two distinct commonly deleted regions on chromosome 13q suggest involvement of BRCA2 and retinoblastoma genes in sporadic breast carcinomas.

BACKGROUND: Frequent allelic losses on the long arm of chromosome 13 in sporadic breast carcinomas suggest that a tumor suppressor gene(s) on 13q is involved in this type of carcinoma. The presence of a familial breast carcinoma susceptibility gene, BRCA2, and the retinoblastoma susceptibility gene (RB) on the same chromosomal arm implies that one or the other, or both, of these genes may be critically affected by those allelic losses. METHODS: To investigate the possible involvement of BRCA2 and RB in sporadic breast carcinomas, the authors examined allelic losses in 246 breast carcinomas with 14 polymorphic microsatellite markers on 13q12.q14. RESULTS: Allelic loss was observed in 95 of the 246 sporadic breast carcinomas (39%). Detailed deletion mapping identified two commonly deleted regions. The more proximal of these two segments was located in a 6-cM interval flanked by marker loci D13S289 and D13S267 and containing the BRCA2 gene; the more distal region was located in a 9-cM interval flanked by marker loci D13S328 and D13S172 and containing the RB gene. Allelic loss on 13q was found more frequently in tumors of the solid tubular histologic type (36 of 66; 55%) than in other types (52 of 146; 36%) (P = 0.0096). Furthermore, a significant association was observed between allelic loss on 13q and the absence of progesterone receptor (P = 0.0001). CONCLUSIONS: The results indicate that BRCA2 and RB are independent targets of allelic loss and that inactivation of either of these genes may play a role in the development of some sporadic breast carcinomas, particularly those of the solid tubular type.

Effects of low dose mixtures of four N-nitroso compounds on hepatic foci development in the rat.

Potential synergism between four N-nitroso compounds (nitrosomorpholine, nitrosodimethylamine, nitrosodiethanolamine, nitroso-oxazolidine) in rat liver carcinogenesis was examined in the medium-term bioassay. Male F344 rats were initially given diethylnitrosamine (DEN, 200 mg/kg, ip) and beginning 2 weeks later received test chemicals for 6 weeks individually at a full or 1/4 dose of that proven to be carcinogenic individually or in combination. All animals were subjected to partial hepatectomy at week 3 and killed at week 8. Induction of immunohistochemically-demonstrated glutathione S-transferase placental form (GST-P) positive foci was evaluated. The numbers and size of GST-P positive foci were significantly higher than the control levels by the treatment with each nitrosamine at full (1/1) and one quarter doses (1/4), excepting nitrosodiethanolamine and by combination of the four chemicals at 1/4 and 1/16. Because the dose-response curves were considered non-linear for most nitrosamines, synergistic effects were not apparent for the 1/4 mixture. Interestingly, however, the values for rats treated with these four chemicals in combination at the 1/4 dose level were almost the same as the average of four individual treatments at the full dose, and those for the 1/16 dose mixture were almost the same as the average of 1/4 individual treatment groups. These results indicate that these nitrosamines worked additively, rather than synergistically, in rat liver carcinogenesis.

Multiple expression of Ly-6C and accumulation of a Ly-6C pre-mRNA in activated macrophages involved in rejection of an allografted tumor.

We established a monoclonal antibody against a surface antigen of activated macrophages (Møs) involved in the rejection of an allografted mouse tumor, Meth A. Bacteria-elicited Møs also expressed the antigen but resident or inflammatory Møs, or other leukocytes, did not. The antigen was identified as Ly-6C by isolation of cDNA clones encoding it. Two-dimensional immunoblotting revealed that Ly-6C of the Møs exists in multiple forms with a similar size but with different isoelectric points. A Ly-6C pre-mRNA also accumulated abundantly in the Møs. Thus, the multiple expression of Ly-6C and the accumulation of the pre-mRNA are the features of allograft- and bacteria-elicited Møs.

Low susceptibility of the spontaneously hypertensive rat (SHR) to quinoline-induction of hepatic hemangioendothelial sarcomas.

Effects of quinoline and captafol, both of which are hemangiocarcinogenic agents, were investigated in spontaneously hypertensive rats (SHR). Male SHR and Wistar Kyoto rats (WKY), the parent strain of SHR, were administered quinoline (0.2%) or captafol (0.15%) supplemented in the diet for 32 weeks. Resultant incidences of hepatic hemangioendothelial sarcomas were in animals receiving quinoline 93% for WKY and only 7% for SHR. A few hepatocellular nodules were also induced in both strains. No histopathological lesions were observed in the other organs. Thus, the SHR proved unexpectedly less susceptible to vascular carcinogenicity than its WKY counterpart.

The effect of hepatitis B virus X gene expression on response to growth inhibition by transforming growth factor-beta 1.

Hepatitis B virus X protein (HBx protein), which seems to be involved in hepatocarcinogenesis, was studied for its effect on cell growth regulation. We examined the response to growth inhibition of transforming growth factor beta 1 (TGF-beta 1) in HBx gene-introduced cells. HBx gene in pRc/CMV was transfected to mink lung epithelial cells (Mv1Lu cells) and a stable transformant was obtained. The inhibition rates of [3H] thymidine incorporation by addition of TGF-beta 1 (0.08 ng/ml) to parent cells and pRc/CMV-transfected cells were 34% and 26%, respectively. However, the inhibition rates in the HBx gene-transfected cells were 3-8%. The amount of TGF-beta type II receptor on the surface of HBx gene-transfected cells was about half of that on the parent or pRc/CMV-transfected cells. Our results indicated that expression of HBx gene reduces the response to growth inhibition by TGF-beta 1.

Subchronic toxicity study of L-isoleucine in F344 rats.

A subchronic toxicity study with L-isoleucine was conducted using F344 rats. Groups of 10 rats of each sex were given diet containing 0, 1.25, 2.5, 5.0, or 8.0% L-isoleucine for 13 wk. No treatment-related effects were observed in terms of body weight change, food consumption or hematology. In both sexes given 8.0% L-isoleucine, increased or a tendency for increased urine volume and relative kidney weights were observed. Furthermore, the high-dose L-isoleucine treatment brought about an elevation of urinary pH and variations in serum electrolytes. However, histopathological alterations related to these changes were not observed in any organs of either sex. In conclusion, the present study demonstrated that L-isoleucine possessed minimal toxicity at dietary levels of 5.0% and 8.0%, while it did not exert any adverse affects at a dietary level of 2.5% or less.

Inhibitory potential of pregnancy and lactation on mammary carcinogenesis induced by a food-derived carcinogen, 2-amino-1-methyl-6-phenylimidazo[4,5-b]pyridine, in Sprague-Dawley rats.

Effects of pregnancy and lactation on a food-derived carcinogen 2-amino-1-methyl-6-phenylimidazo[4,5-b]pyridine (PhIP)-induced mammary carcinogenesis were investigated in female Sprague-Dawley rats. When rats were administrated PhIP in the diet (100 ppm) for 15 weeks, palpable subcutaneous tumors were first detected at week 24 in three nulliparous rats, while tumor latency was apparently increased in animals undergoing delivery and weaning during the PhIP administration period. However, since the incidence of palpable tumors was not consistently increased in the nulliparous animals, possibly due to the short PhIP-treatment, and the fact that spontaneous tumors developed in parous animals after week 44, the final incidences and multiplicities of histopathologically confirmed adenocarcinomas at week 48 were not significantly different between the two groups. These findings art partly supported by the epidemiological evidence that breast cancer is more prevalent in nulliparous than in parous women.

Expression of heparin-binding EGF-like growth factor in rat liver injured by carbon tetrachloride or D-galactosamine.

We reported recently that heparin-binding EGF-like growth factor (HB-EGF), a member of the EGF family, is a new hepatotrophic factor for the regeneration of rat liver after partial hepatectomy. The current study examined changes in the amount of HB-EGF mRNA in liver injured by hepatotoxins. The level of HB-EGF mRNA was very low in normal rat liver, but increased markedly in the liver of rats injured by CCl4, showing two peaks, the first at 6 h and the second at 36 h. Western blot analysis showed that HB-EGF protein in the liver of CCl4-treated rats (at 6 h) was increased about 3.4-fold above normal. The level of HB-EGF mRNA also increased markedly in the liver of rats treated with D-galactosamine, showing a major peak at 18 h, and a smaller one at 6 h. These results indicate that HB-EGF may play a role in the regeneration of the liver following hepatotoxic injury.

Medium-term bioassay for the hepatocarcinogenicity of hexachlorobenzene.

Hexachlorobenzene (HCB) is an important environmental contaminant derived mainly from industrial and agricultural sources. It is carcinogenic in mice, rats and hamsters. It has now been studied in a medium-term bioassay for carcinogenicity based on the induction of preneoplastic lesions in the liver. We report here that the bioassay can rapidly detect carcinogenic doses of HCB and that there is a clear dose-response relationship. At the lowest dose of HCB administered, the incidence of preneoplastic lesions in the liver was no different from that in controls.

Regulation of heparin-binding EGF-like growth factor expression by phorbol ester in a human hepatoma-derived cell line.

Heparin-binding EGF-like growth factor (HB-EGF) is a recently identified potent mitogen for smooth muscle cells and fibroblasts. HB-EGF has been shown to be an EGF receptor ligand, and also to stimulate epithelial cell growth. A human hepatoma-derived cell line, Mahlavu, was analyzed for the production of HB-EGF mRNA and active HB-EGF protein. It was found that the cell line synthesized very low or undetectable basal level of HB-EGF mRNA. However, the addition of 12-O-tetradecanoylphorbol-13-acetate (TPA) led to a rapid and transient rise in HB-EGF mRNA level. HB-EGF in Mahlavu cells appears to be regulated by a protein kinase C (PKC) pathway, since PKC inhibitors, H7, staurosporin, and calphostin C, abrogated the induction of HB-EGF mRNA by TPA. Unlike vascular smooth muscle cells, induction of HB-EGF gene transcription by TPA was blocked completely by incubation with cycloheximide, suggesting that protein synthesis may be a prerequisite for HB-EGF gene transcription in Mahlavu cells. Mahlavu cells were also found to release a bioactive HB-EGF-like protein into conditioned medium which stimulates DNA synthesis in EP170.7 cells. This activity was neutralized by an anti-HB-EGF antibody. These results indicate that HB-EGF gene transcription is regulated via a PKC pathway, resulting in secretion of active HB-EGF into the culture medium of hepatoma-derived Mahlavu cells.

Inhibitory effects of phenolic compounds on development of naturally occurring preneoplastic hepatocytic foci in long-term feeding studies using male F344 rats.

Five phenolic compounds, namely caffeic acid, sesamol, hydroquinone, catechol, and 4-methoxyphenol, were fed to groups of 30 male F344 rats at dietary levels of 2, 2, 0.3, 0.8, and 2%, respectively, for 2 years. Retardation of body weight and elevated relative liver weights were noted for all groups. Formalin-fixed and paraffin-embedded liver tissues from rats killed terminally were cut and stained for glutathione S-transferase placental form (GST-P) and tumor growth factor alpha (TGF alpha) immunohistochemically. Numbers and areas of GST-P-positive (GST-P+) foci per unit area of liver section were measured, and the respective treated/control proportional values were calculated to be 58 and 57% for caffeic acid. 58 and 54% for sesamol, 71 and 71% for hydroquinone. 58 and 133% for catechol, and 49 and 39% for 4-methoxyphenol. These data were comparable with results obtained with medium-term liver bioassays (Ito test). However, no intergroup differences were detected with regard to quantitative findings for TGF alpha foci, which were relatively rare. Long-term inhibitory effects of phenolic compounds on liver carcinogenesis, predicted from the Ito test, were thus confirmed in the present feeding studies using quantitative analysis of immunohistochemically demonstrable GST-P+ foci as end point marker lesions.

Cough threshold for capsaicin increases by azelastine in patients with cough-variant asthma.

To assess the effects of azelastine in patients with cough-variant asthma, we measured the cough threshold for capsaicin (the concentration required to elicit more than five coughs) in 16 patients with cough-variant asthma before and after 4 weeks of treatment with azelastine (2 mg; b.i.d.) or placebo. After treatment, coughing decreased in all patients and the cough threshold for capsaicin increased significantly, from 0.67 +/- 0.30 microM to 4.76 +/- 1.55 microM (P < 0.01) in the azelastine group. However, the cough threshold for capsaicin did not increase significantly, from 0.86 +/- 0.33 microM to 1.11 +/- 0.35 microM (P > 0.10) in the placebo group. These results suggest that azelastine inhibits coughing in patients with cough-variant asthma.
