Novel type of interstitial cell (Cajal-like) in human fallopian tube.

We describe here--presumably for the first time--a Cajal-like type of tubal interstitial cells (t-ICC), resembling the archetypal enteric ICC. t-ICC were demonstrated in situ and in vitro on fresh preparations (tissue cryosections and primary cell cultures) using methylene-blue, crystal-violet, Janus-Green B or MitoTracker-Green FM Probe vital stainings. Also, t-ICC were identified in fixed specimens by light microscopy (methylene-blue, Giemsa, trichrome stainings, Gomori silver-impregnation) or transmission electron microscopy (TEM). The positive diagnosis of t-ICC was strengthened by immunohistochemistry (IHC; CD117/c-kit+ and other 14 antigens) and immunofluorescence (IF; CD117/c-kit+ and other 7 antigens). The spatial density of t-ICC (ampullar-segment cryosections) was 100-150 cells/mm2. Non-conventional light microscopy (NCLM) of Epon semithin-sections revealed a network-like distribution of t-ICC in lamina propria and smooth muscle meshwork. t-ICC appeared located beneath of epithelium, in a 10-15 microm thick 'belt', where 18+/-2% of cells were t-ICC. In the whole lamina propria, t-ICC were about 9%, and in muscularis approximately 7%. In toto, t-ICC represent ~8% of subepithelial cells, as counted by NCLM. In vitro, t-ICC were 9.9+/-0.9% of total cell population. TEM showed that the diagnostic 'gold standard' (Huizinga et al., 1997) is fulfilled by 'our' t-ICC. However, we suggest a 'platinum standard', adding a new defining criterion- characteristic cytoplasmic processes (number: 1-5; length: tens of microm; thickness: < or =0.5 microm; aspect: moniliform; branching: dichotomous; organization: network, labyrinthic-system). Quantitatively, the ultrastructural architecture of t-ICC is: nucleus, 23.6+/-3.2% of cell volume, with heterochromatin 49.1+/-3.8%; mitochondria, 4.8+/-1.7%; rough and smooth endoplasmic-reticulum (1.1+/-0.6%, 1.0+/-0.2%, respectively); caveolae, 3.4+/-0.5%. We found more caveolae on the surface of cell processes versus cell body, as confirmed by IF for caveolins. Occasionally, the so-called 'Ca2+-release units' (subplasmalemmal close associations of caveolae+endoplasmic reticulum+mitochondria) were detected in the dilations of cell processes. Electrophysiological single unit recordings of t-ICC in primary cultures indicated sustained spontaneous electrical activity (amplitude of membrane potentials: 57.26+/-6.56 mV). Besides the CD117/c-kit marker, t-ICC expressed variously CD34, caveolins 1&2, alpha-SMA, S-100, vimentin, nestin, desmin, NK-1. t-ICC were negative for: CD68, CD1a, CD62P, NSE, GFAP, chromogranin-A, PGP9.5, but IHC showed the possible existence of (neuro)endocrine cells in tubal interstitium. We call them 'JF cells'. In conclusion, the identification of t-ICC might open the door for understanding some tubal functions, e.g. pace-making/peristaltism, secretion (auto-, juxta- and/or paracrine), regulation of neurotransmission (nitrergic/purinergic) and intercellular signaling, via the very long processes. Furthermore, t-ICC might even be uncommitted bipotential progenitor cells.

[Comparative results of total detections and detection in a group of patients with high risk for tuberculosis in an urban environment].

In the territory of 5 anti-TBC dispensaries in the area of Bucharest 2 circumscriptions have been selected at random in which radiophotographic detection was to be carried out integrally and 2 other circumscriptions were selected for the investigation of groups at high risk of developing tuberculosis. All the new cases of active pulmonary tuberculosis that have been recorded, through all the methods in these 20 circumscriptions in 1973 were analized retrospectively and the reasons appeared that had determined their existence was mentioned or was ommitted on the occasion of previous investigations. These reasons were correlated with the data concerning the method for detection that had been employed, according to the indications of the tuberculosis dispensaries. In view of the evaluation of the degree of real covering of the population in integral rf investigations, the team of the Institute of Phtysiology investigated representative lots of population from each dispensary and checked the catagraphy of the population in the field, house-by-house. It was noted that 20--21% of the subjects that had been programmed for the integral rf examination (according to catagraphic criteria) were absent when the checking in the field, that should have been programmed, were not included in the catagraphy. On the basis of this study the methodology for radiophotographic detection for groups with risk of developing tuberculosis was established.

[261 cases of bronchopulmonary cancer detected by radiophotography].

In Bucharest, district 6, of the 1 151 313 radiophotographies performed, 261 revealed bronchopulmonary cancers (22.6 per 10 000) of which 73% males and 27% females, the 50--70 years age group being the most affected (181 cases, i.e. 70%). The 85% deaths occurred within 2 years after detection. In 68% (177 cases) cancer was peripheral and in 32% centrohillar. The peripheral localization in 68% of the cases opens prospects for an early detection by dynamic follow up of the pulmonary radiophotography at shorter intervals, especially in the age group with a higher cancer risk (50--70 years), indicating in some cases immediate admission to a surgical department.

Investigations on the presence of enteroviruses in drinking water.

Out of 220 tap-water samples examined, 3.64% were found to contain enteroviruses. The 11 isolates included 3 attenuated poliovirus, 2 Coxsackie A4 and 6 untyped strains. The necessity of improving the treatment of drinking water, especially by disinfection, is outlined.

[Current incidence and clinical significance of non-culturable bacilli or those culturable with difficulty].

An analysis of 246 patients with positive smear present bacilli and negative cultures revealed that in only 9% of the cases no organized previous treatment had been applied, although almost half of these had benefited from short courses with streptomycin associated with penicillin before an exact diagnosis of tuberculosis had been made. Since the remaining 91% had had 1--2 months of chemotherapy, which frequently included rifampicin, the authors stress that microscopically detectable germs are frequently incapable to develop on culture media. These are frequently non-viable bacteria and in such conditions no subsequent modifications of the currently applied therapy scheme should be attempted. The authors show that only 26,2% of the germs from 519 positive cultures have developed after 21 days from the seeding. At 30 days the overall proportion increased to 71,2% while at 45 days 22,7% more positive cultures appeared and an additional 6,1% cultures became positive at 60 days. Tubercle bacilli become increasingly demanding and slower growing. If at 21 days only one colony was evident in 6% of the cases, while an infinite number of colonies were present in 58% of the cultures at 60 days a single colony was found in 42% of the cases, in contrast with only 9% cases with an infinite number of colonies (P less than 0,001). The delay in the growth of bacilli was parallel with the duration of chemotherapy. The results are compared with data from the literature.

[The clinical and epidemiological aspects of a focus of epidemic keratoconjunctivitis].

An outbreak of epidemic keratoconjunctivitis was followed from October 5 to November 14, 1987, in a Transylvania city, from clinical, epidemiological and virological points of view. Fixation of complement and seroneutralization was used in view of identification of the virus strains isolated from the outbreak.

[Strictly supervised antitubercular chemotherapy in district and plant medical dispensaries in the city of Craiova].

The mode of administration of chemotherapy is evaluated, in conditions of integration, and under strict supervision, in tuberculosis patients in 12 medical dispensaries and in 6 enterprise dispensaries from Craiova over a period of one year. A total of 88,54 +/- 17,99 per cent of the number of programmed doses was administered in the territorial medical dispensary, as compared with 81,9 +/- 21,3 per cent in the tuberculosis dispensary. In enterprise dispensaries 97,69 +/- 16,17 per cent of the doses was given effectively, as compared with 78,17 +/- 29,39 per cent of doses given to the same patients in the tuberculosis dispensary. During the sick-leave 92,14 +/- 9,19 per cent of the doses were given, in the medical territorial dispensary as against 86,98 +/- 17,58 per cent in the tuberculosis dispensary. The difference is not significant. After reinsertion 77,53 +/- 24,84 per cent of the programmed doses were administered in the tuberculosis dispensary, 88,2 +/- 19,84 per cent in the territorial medical dispensary and 97,12 +/- 5,66 per cent in the enterprise dispensary. The differences were not significant. The authors conclude that administration of anti-tuberculosis chemotherapy can be perfectly well carried out in integrated conditions in the urban environment, especially in interprise medical units, after reinsertion. The phtysiologist should make a periodical survey in these units in view of securing a strict administration of the treatment regimens.

[The current treatment of epistaxis].

The authors present the results of a study regarding 1906 cases of epistaxis admitted in the Colţea ORL Clinic, of a total of 25,246 admitted (5.56%) between January 1, 1983-December 31, 1987. Here are presented the distribution of the cases on years of study and etiologic types, sex, age groups and sex, causes (it is remarked the dominant incidence of high blood pressure--59.24%), seasons, with a higher frequency in the cold season, frequency related to professions and the place of the nasal fossa as well as the place of the vascular break--a fore place or a hinder place. The treatment applied to the admitted patients is charted on a diagram, the authors discussing the therapeutical measures that must be applied in the cases of serious epistaxis, as well as epistaxis at patients with multiple health problems, underlining the value of the interdisciplinary collaboration. There are also discussed the infectious complications of the nasal wads and great stress is laid upon avoiding errors in therapeutical measures.

[Current aspects in Menière's syndrome].

The authors report, on the basis of the data in the literature and their own experience, the main elements in etiopathology, positive and differential diagnosis and treatment of Ménière's syndrome. The new discoveries of the genesis of endolymphatic hydrops, methods of clinic diagnosis and paraclinical investigations (liminal and supraliminal tonal audiometry, impedance measurement, Luscher's, Metz's audiometric tests, and the balance test--Fowler's) are described. After presenting the affections which have to be left aside in the diagnosis of Ménière's syndrome, the paper reports shortly on the medical and surgical therapeutic methods used nowadays.
