Plasma collection using nafamostat mesilate and dipyridamole as an anticoagulant.

Nafamostat mesilate (FUT-175) is a strong protease inhibitor and is used as an anticoagulant in extracorporeal circulation. In the present study, we administered FUT and dipyridamole as anticoagulants during donor plasmapheresis, and the additional effect of dipyridamole was examined. In dogs weighing about 15 Kg, donor plasmapheresis was performed using a cellulose triacetate membrane plasmaseparator (PEX-15, Nipro), and 300 ml of filtrated plasma was obtained within 30 minutes. These dogs were divided into two groups, 50 mg/h of FUT alone was infused (group A, n = 10), and the same dose of FUT and 25 mg/h of dipyridamole were infused (group B, n = 5) during plasmapheresis. Changes of the coagulation system, blood cell counts and complement system were investigated. In clinical cases, an artificial liver support therapy using FUT as an anticoagulant was performed on 8 patients with acute liver failure, and blood compatibility was examined. In dogs, RBC counts and hematocrits were almost stable, but WBC counts were decreased to about 70% of the previous value. Platelets counts were about 60% and 80% of the previous value, in group A and group B, respectively, at the end of the plasmapheresis (P less than 0.01). Clotting factor VII was diminished to about 20% and 35% of the previous value in group A and B, respectively. In clinical cases, all patients received the plasma exchange combined with hemodialysis using FUT as an anticoagulant, and no distinct side effect was observed during plasma exchange.

An immunocytochemical demonstration of calcineurin in human nerve cell tumors. A comparison with neuron-specific enolase and glial fibrillary acidic protein.

Human central and peripheral nerve cell tumors were examined in detail using antibodies to calcineurin, glial fibrillary acidic protein (GFAP) and neuron-specific enolase (NSE). Forty-eight formalin-fixed and paraffin-embedded specimens of human neuronal tumors, including 27 medulloblastomas, were examined. Calcineurin-positive cells were found in all peripheral nerve cell tumors and the two gangliogliomas, whereas 20 of the 27 medulloblastomas and one of the two cerebral neuroblastomas did not contain calcineurin-positive cells. Differentiation of cells along the neuronal lines was positively correlated with calcineurin immunoreactivity. NSE-positive cells were found in all of the tumors with the exception of the one cerebral neuroblastoma. NSE immunoreactivity was not invariably consistent with calcineurin immunoreactivity and non-neuronal cells were often positive. Calcineurin-positive cells were all devoid of GFAP, but NSE-positive cells expressed GFAP in some tumors. GFAP-immunoreactive cells were found only in central nerve cell tumors, and not in peripheral tumors. In addition, GFAP-positive cells in some tumors such as retinoblastoma and medulloblastoma morphologically revealed not only neoplastic but also reactive astrocytic features.

Glutathione contents in rat livers after acute and chronic exposure to ethylene oxide.

Wistar rats were subjected to a 6 hr exposure to ethylene oxide once at the concentration of 500 ppm, 3 times a week for 12 weeks as a chronic experiment. Hepatic glutathione contents were determined after these treatments. The specific content of reduced form of glutathione (GSH) in the rat livers of chronic exposures was 28.9 nmoles/mg protein, which is not significantly different from that of control group. On the other hand, the hepatic content of GSH in rats subjected to a 4 hr exposure to ethylene oxide at a concentration of 2500 ppm decreased markedly to the levels of 5% of control value. The present results suggest the involvement of glutathione, at least in part, in the detoxication of ethylene oxide.

[The inhaled effects of fenitrothion powder on blood cholinesterase in rats].

Effects of fenitrothion (Sumithion) powder on red cell acetylcholinesterase (AChE) and plasma cholinesterase (PChE) were studied in vitro and in vivo. Experiments in vitro revealed that fenitrothion has almost the same potency to inhibit AChE (in pre-washed red cell membrane) and PChE. Rats (Wistar, male) were exposed to fenitrothion powder for 4 hours using a new apparatus which can generate powder constantly over a long period. In this experiment, however, no apparent suppression of AChE was found while PChE was markedly inhibited. This inhibition persisted for 3 days after the exposure. This discrepancy was interpreted to be due to the inevitable procedure in preparing red cell specimens, i.e., washing of the red cells. After acute exposure in vivo, fenitrothion appears to be easily removed from the red cells by washing. Thus, PChE, rather than AChE, seems to be a good indicator of an acute exposure to fenitrothion powder in vivo.

[Analysis of urinary metabolites of rats exposed to ethylene oxide].

Urinary metabolites of rats exposed to ethylene oxide(EO) were analyzed by gas chromatography-mass spectrometry(GC/MS). Male Wister rats(220-240 g) were exposed to EO for 6 hours. The urine was collected during 18 hours after the exposure and extracted with ethyl acetate. For the water-soluble metabolites, ethanol was added to the urine centrifuged and the supernatant fraction was evaporated to dryness. The residue of the extract was methylated with diazomethane and trimethylsilylated with bis(trimethylsilyl)trifluoroacetamide. Ethylene glycol, 2-hydroxymercapturic acid, 2-methylthioethanol and 2-mercaptoethanol were identified as the metabolites of EO. These results suggest that the inhaled EO was hydrolyzed to ethylene glycol, and conjugated with glutathion to form the mercapturic acid and methylthio metabolite.

A histological comparison of the tissue interface of bioglass and silica glass.

The objectives of the present article are to confirm the bone bonding phenomenon of Bioglass (BG) developed by Hench et al., and to observe the singularity of tissue reaction to it. BG and nonreactive silica glass (SG) were implanted in the femurs of rabbits and rats. Histological examination revealed that a relatively acellular zone with little inflammation was formed on BG surface at 1 day after implantation. Neither fibrous tissue nor a distinct boundary was observed between BG and bone after 7 days. On the contrary, a moderate postoperative inflammatory reaction was observed on SG at 1 day, and fibrous tissue was observed between SG and bone after 7 days. From these findings, it was confirmed that BG bonded directly with bone. As the relatively acellular zone observed on BG surface at 1 day was replaced by bone after 7 days, the formation of this zone might play an important role in bone-bonding process. Further research should be focused on the mechanism and biological meaning of bone bonding, for this phenomenon can not be explained by the conventional pathological theory of foreign body encapsulation.

Hepatic assist device, using membrane plasma separator and dialyzer.

Our hepatic assist device is composed of a membrane plasma separator, blood and plasma pumps, hemodialyzer and controller. Using this device, the patients plasma is replaced with fresh donor plasma in a 5000 ml amount daily. This procedure of plasma exchange takes place in the intensive care unit, until the patient recovers consciousness or cerebral death is confirmed. In the initial results of this plasma exchange, 5 out of 10 patients with fulminant hepatic failure survived. Even with fatal cases, prolongation of survival time was observed. Our hepatic assist device, performing an easy and safe procedure for plasma exchange, appears to be the most promising method of providing long-term hepatic support for acute liver failure at the present time.

Cumulative toxicity potential of methomyl aerosol by repeated inhalation.

There are few investigations concerning the cumulative toxicity of agricultural chemicals by repeated inhalation. In this study, Wistar male rats were exposed to methomyl powder (mass median aerodynamic diameter, 4.4 microns) for a single 4-hr exposure, or for 4 hr/day, 5 days/week for 3 months. The average exposure concentrations were controlled at 9.9 mg/m3 for the single exposure and at 14.8 mg/m3 for repeated exposures by a dust generator consisting of a continuous fluidized bed with an overflow pipe and a screw feeder. After the repeated exposures, plasma and red cell cholinesterase activities, and lipid concentrations of the rat lungs were measured and histopathological examinations were performed. There was no evidence of cumulative effects on the red cell cholinesterase activity, histopathological changes and lipid concentration in 3-month repeated inhalation.

Phosphorylation and inactivation of brain glycogen synthase by a multifunctional calmodulin-dependent protein kinase.

Glycogen synthase was partially purified from canine brain to about 70% purity. The purified enzyme showed differences from the properties of the skeletal muscle enzyme with respect to molecular weights of the holoenzyme and subunit and phosphopeptide mapping. The multifunctional calmodulin-dependent protein kinase from the brain phosphorylated brain glycogen synthase with concomitant inactivation of the enzyme. Although about 1.3 mol of phosphate/mol subunit was maximally incorporated into glycogen synthase, 0.4 mol of phosphate/mol subunit was sufficient for the maximal inactivation of the enzyme. The results indicate that brain glycogen synthase is regulated in a calmodulin-dependent manner similarly to the skeletal muscle enzyme, but that the brain enzyme is different from the skeletal muscle enzyme.

Conduction aphasia and arcuate fasciculus.

Three patients are presented who developed conduction aphasia after a small infarction almost exclusively confined to the arcuate fasciculus. All of them were diagnosed as conduction aphasia within a week after the stroke and showed a rapid amelioration. On the basis of the 3 patients and conduction aphasics in the literature, the relation between conduction aphasia and the arcuate fasciculus is discussed.

Clinical evaluation of the Omniscience aortic disc valve prosthesis.

The clinical results and hemodynamics were evaluated in 100 cases of aortic valve replacement, using the Omniscience valve, in the period December 1980 through 1984. Late survival rate, clinical improvement and cardiac and prosthetic valve performance were acceptably satisfactory. The overall incidence of thromboembolism was 4.7%/patient year. The opening angle of the disc was less than 60 degrees in 18% of the studied cases. Further investigations are required to determine if such results were related to the valve structure or to some cause in the recipient. Assessment of the durability of the Ominiscience cardiac valve prosthesis must await the results of long-term observation.
