Poxvirus infection in Nile crocodiles (Crocodylus niloticus).

An outbreak was encountered of numerous yellowish cutaneous nodules in one- to two-year-old farmed Nile crocodiles (Crocodylus niloticus) in Kasaba Bay Crocodile Farm at Lake Tanganyika in Zambia during 1988. Out of 4000 crocodiles of different age groups, 300 yearlings were affected and 82 of those affected died. The lesions were prominent on the head, especially around the eyelids, the nostrils, both sides of the mouth, ventral neck, ventral pale belly, limbs and the root of the tail. Histologically, the epidermal lesions revealed large focal areas of marked acanthosis accompanied by hyperkeratosis and parakeratosis. In the prickle cells, there were multiple small neutrophilic granular inclusions and large eosinophilic homogeneous cytoplasmic inclusions. Electron microscopically, there were numerous poxvirus particles and matrices in the cytoplasm of many prickle cells. Dark blue homogeneous cytoplasmic inclusions in toluidine blue sections consisted of an electron opaque matrix with mature viral particles (160 x 200 x 230 nm). Furthermore, there were clumps of granular matrix containing immature viral particles (200 x 399 nm) in the cytoplasm. Various features of viral developing processes were observed.

[Report of a case successfully weaned from long left ventricular assist with complete recovery after coronary artery bypass grafting].

A 45-year-old female underwent coronary artery bypass grafting (CABG) due to medically uncontrollable unstable angina. After completion of CABG, left ventricular contraction was remarkably diminished to maintain systemic circulation. Because intraaortic balloon pumping and veno-arterial bypass with large amount of catecholamines could not improve left ventricular wall function, we decided to use left ventricular assist device (LVAD). After application of LVAD, the patient could easily be weaned from cardio-pulmonary bypass. LVAD was used for 25 days and the patient was successfully weaned from LVAD too, and are now living 6 months postoperatively. As far as we know, this case is the first survival patient after the longest cardiac assist in our country.

[Bone marrow transplantation for MDS].

Five patients with myelodysplastic syndrome (RA: 4 cases, RAEB in T: 1 case) were treated with myeloablative immunosuppressive therapy followed by bone marrow transplantation (BMT). Median age was 20-y-o (11-31-y-o). All patients were prepared with cyclophosphamide and total body irradiation. Engraftment was documented in all patients. One patients (case 4, 31-y-o female) died of brain hemorrhage due to the thrombocytopenia refractory to platelet transfusion because of anti-platelet antibody in 34 days after BMT. A patient with RAEB in T was also died of respiratory failure from interstitial pneumonia on Day 173. One patient (case 1, 22-y-o, female) progressively became granulocytopenic and thrombocytopenic status after BMT. She suffered from life-threatening infection and then received a second bone marrow cell infusion from the same donor without any preparative conditioning. These results suggest that BMT could be the treatment of choice for MDS, especially for the patients with RA who have poor prognostic factors including life-threatening cytopenia and or cytogenetical abnormalities.

[Consideration of management of type B dissecting aneurysms of the thoracic aorta: clinical comparative study between surgical and medical treatments].

Between 1970 and 1989, 116 patients with type B dissecting aneurysms of the thoracic aorta were seen in our institution and affiliated hospitals. The patients were classified into 5 groups according to the acuity (acute vs chronic) and modes of therapy (surgical vs medical). Group I: 24 patients with acute B dissection were treated surgically during the acute stage. Group II: 21 patients with acute B dissection were initially treated with intensive medical therapy and followed by elective operation during the subacute stage. Group III: 22 patients with acute B dissection were treated medically. Group IV: 42 patients with chronic dissection were treated surgically. Group V: 7 patients with chronic B dissection were treated medically. The 5-year survival rates including early mortality were 70.5 +/- 9.4% for Group I, 88.9 +/- 7.5% for Group II, 68.3 +/- 11.2% for Group III, 64.6 +/- 8.4% for Group IV and 71.4 +/- 17.1% for Group V. The 5-year survival rates of Group II was significantly better than those of Group I, III and IV, respectively. The present data suggests that acute type B dissection without complications (bleeding, visceral or lower limbs ischemia) should be treated initially with intensive medical therapy and then followed by elective operation during the subacute stage, if the false lumen were not thrombosed.

[Have the results of mitral valve replacement improved?].

The operative mortality following valvular surgery remains substantial for specific high risk groups despite recent improvement in surgical techniques, anesthetic managements, and postoperative care. To test the contention that the results of valvular surgery are better in recent years, we examined patients undergoing isolated or combined mitral valve replacement in an earlier era (1980-1984; n = 68) and a later era (1985-1989; n = 106). There were no significant differences concerning the clinical characteristics between an earlier era and a later era, although patients in an earlier era were more seriously in hemodynamics, and in a later era were older, a higher prevalence of reoperations and a combined tricuspid valve surgery. The operative mortality was 10.3% in an earlier era, 6.6% in a later era, and so it is not significantly improved in a recent 5 years, however the incidence of premature death (death within 30 days) except valve re-replacement was significantly improved. Using univariate multiple logistic model, the predictors for operative death after mitral valve replacement in an earlier era were advanced NYHA functional class, larger CTR, higher systolic pulmonary pressure and pulmonary capillary wedge pressure, associated preoperative organ dysfunction and heart lung machine time. In a later era, these were advanced NYHA functional class, higher right atrial mean pressure, associated preoperative organ dysfunction, valve re-replacement, heart lung machine time and aortic cross clamp time. Multivariate analysis including these significant factors could not demonstrated the most independent predictors of operative mortality after mitral valve replacement in both an earlier era and a later era.(ABSTRACT TRUNCATED AT 250 WORDS)

[Mixed blast crisis with the cytogenetic evidence of three clonal evolutions].

A 46-year-old man was diagnosed as having chronic myelogenous leukemia (CML) in chronic phase in Dec. 1985. In Dec. 1987, anemia and leukocytopenia progressed, and the percentage of blast cells increased in the bone marrow. The blast cells were lymphoblastoid and positive for TdT. It was treated as a lymphoid crisis with vincristine and prednisolone, and complete remission was achieved. However, the blasts (11%) were observed in the bone marrow in Mar. 1988, and the chromosomal analysis revealed 46, XY, t (2q-; 11q+), t (9q+; 22q-) in 13 out of 20 cells. In June, the percentage of the blasts increased again, but chromosomal analysis showed a different karyotype, 46, XY, t(2p-; 11p+), t(9q+; 22q-) which was observed in 9 out of 10 cells. Then, myeloblastoid cells increased rapidly in spite of the chemotherapy in Dec. 1988. The chromosomal analysis showed 46, XY, 2p-, 7q-, 9q+, 11p+, 22q- in all analyzed cells. The rearrangement of the bcr gene could be detected by the Southern blotting. The blasts were positive for CD7, CD11, CD13, CD33, CD36, CD41 and CD42, suggesting that the blasts had the surface phenotypes of both myeloid and megakaryocytoid-lineage. This is a case with the mixed blast crisis that changed from the lymphoid to the myelo-megakaryocytoid in nature, in which three clonal evolutions were observed during the clinical course.

Testicular damage induced by megadoses of pyridoxine.

Pyridoxine hydrochloride, 125 mg/kg, 250 mg/kg, 500 mg/kg or 1,000 mg/kg, daily, was intraperitoneally injected into Wistar male rats and its effects on weights and mature spermatid or sperm counts in the testis and the epididymis were investigated. After six weeks administration, weights of the testis and the epididymis in the 500 mg/kg and 1,000 mg/kg groups dramatically decreased and weights of the epididymis in the 125 mg/kg and 250 mg/kg groups also decreased significantly. Mature spermatid counts in the testis and sperm counts in the epididymis decreased in the 500 mg/kg and 1,000 mg/kg groups, and sperm counts in the tail plus body of the epididymis also decreased in the 250 mg/kg group. From these results, it was elucidated that megadoses of pyridoxine induced testicular damage in rats.

[Effects of ethylene oxide on hepatic microsomal cytochrome P-450: an in vitro study].

The effect of ethylene oxide on rat hepatic microsomal cytochrome P-450 was studied in vitro. Cytochrome P-450, but not b5, was decreased by ethanol. When hepatic microsome was bubbled with ethylene oxide gas, cytochrome P-450 content remained unchanged when compared with CO2 bubbling. The addition of NADPH 1 mM also did not change cytochrome P-450 significantly. The excessive high exposure to 500 mM ethylene oxide also did not affect microsomal cytochrome P-450. These results indicate that ethylene oxide does not destroy directly the hepatic microsomal cytochrome P-450 under these conditions.

Distribution, possible origins and fine structure of neuropeptide Y-containing nerve fibers in the rat liver.

The distribution, possible origins and fine structure of neuropeptide Y (NPY)-containing nerve fibers in the rat liver were investigated by immunohistochemistry, nerve transection and immunoelectron microscopy. Light-microscopic immunohistochemistry showed NPY fibers forming a complex network in and around the walls (tunica adventitia and tunica media) of hepatic vessels. They were also closely associated with interlobular bile ducts. The NPY fibers in the liver were almost completely eliminated by transection of the greater splanchnic nerves just distal to the celiac ganglion. Transection of the greater splanchnic nerves just proximal to the celiac ganglion resulted in a marked decrease in NPY fibers, but a significant number remained intact. Under electron microscopy. NPY terminals without a covering of glial processes were seen not only in proximity to smooth muscle cells within the tunica media of hepatic vessels but also in the subendothelial areas of the tunica intima. Some NPY axon terminals devoid of glial ensheathment were located close to the basal lamina of interlobular bile ducts. Occasionally, single axon terminals with NPY were found in the vicinity of or in contact with hepatic cells. There was a small number of NPY fibers that had lost their glial sheaths while running toward lymphatic capillaries. These findings suggest that hepatic NPY arises from the celiac ganglion and paravertebral sympathetic ganglia, and that it is involved in more complex physiological processes than the previously described neuropeptides in the liver, which are localized exclusively to hepatic vessel walls.

[Effects of inhalation of ethylene oxide on female rats].

The effects of systemic toxicity including reproductive toxicity of ethylene oxide on female rats were studied. When Wistar female rats were exposed to 250 ppm of ethylene oxide for six hours a day, five days a week for ten weeks, they showed inhibition of body weight gain and paralysis of the hindlegs. Hematological examination revealed macrocytic and normochromic anemia with high reticulocyte counts. The estrus cycle of the exposed group was prolonged and the percentage of the diestrus stage increased. There was no atrophy in the ovary or the uterus. However, the activity of glutathione reductase in the ovary decreased by 18% and that of glutathione-S-transferase increased by 30%. These results indicate that ethylene oxide has a similar effect on both female and male rats and that the female reproductive system is also affected.

Co-localization of corticotropin-releasing factor- and enkephalin-like immunoreactivities in nerve cells of the rat hypothalamus and adjacent areas.

The co-localization of corticotropin-releasing factor (CRF)- and enkephalin (ENK)-like immunoreactivities in nerve cells of the rat hypothalamus and adjacent areas was investigated by the simultaneous application of immuno-beta-galactosidase staining and the peroxidase-antiperoxidase (PAP) method to the same sections. CRF-like immunoreactive cells were stained blue with immuno-beta-galactosidase staining and ENK-like immunoreactive cells brown with the PAP method. Double-labeled cells with overlap of blue and brown immunoreaction products were identified in all subregions of the paraventricular hypothalamic nucleus (PVH), in contrast to previous studies showing the occurrence of double-labeled cells only in the parvocellular part of the PVH. Other areas that contained double-labeled cells were: the medial preoptic area, bed nucleus of the stria terminalis, periventricular hypothalamic nucleus, lateral hypothalamic area, dorsal hypothalamic area and subincertal nucleus. These findings suggest that nerve cells with both CRF- and ENK-like immunoreactivities may be more actively involved in neuroendocrine regulation and neural transmission than previously considered.

[21-hydroxylase deficiency associated with adrenal tumor: case report of two brothers].

Described herein are two brothers with 21-hydroxylase deficiency (21-OHD) associated with adrenal tumors, and these possible mechanisms are discussed. A 34-year-old male was admitted on Jan. 9, 1984 because of an enlarged and tender left breast. Physical examination revealed short stature (152 cm, 76.5 kg), gynecomastia and shortening of metacarpal bone. His testes were small (2.6 X 1.6 X 1.9 cm). Urinary excretion of 17-OHCS was within normal range (5.9 mg/day), but those of 17-KS, 17-KGS and pregnanetriol were markedly increased (44.4, 110 and 22.6 mg/day, respectively). Plasma concentrations of progesterone and ACTH and urinary excretions of estrone, estradiol and estriol were also increased. Urinary excretions of 17-KS were decreased to 11.7 mg/day and 17-KGS to 22.3 mg/day after the ingestion of 2 mg/day dexamethasone for two days. The computed tomography and a scintigraphy with 131I-Adosterol revealed a tumor in the left adrenal gland, and the adrenal arteriography revealed a neovascularity and a tumor stain in the tumor. These data indicated that the patient was suffering from both 21-OHD and the left adrenal tumor. At this point, adenoma or adenocarcinoma of the adrenal gland was suspected. The left adrenal tumor (85 g) was resected on April 10, 1984, and the pathological diagnosis was adrenal adenoma. The patient's endocrinological abnormalities, however, did not improve after the operation. Urinary excretions of 17-KS and KGS were increased to 57.9 and 108.5 mg/day, respectively, in the patient's elder brother, and 63.3 and 127.9 mg/day, respectively, in his younger brother, indicating that they also had 21-OHD. Interestingly, an adrenal tumor was diagnosed by abdominal computed tomography in the elder brother who had the same HLA typing as the present case. The three brothers had 21-OHD, and two of them had both 21-OHD and adrenal tumor. To our knowledge, this is the first report documenting the co-existence of adrenal tumors in brothers with 21-OHD. This suggests that adenoma can be one of the complications of 21-OHD, probably due to the chronic stimulation by ACTH, and that a possible linkage to HLA may exist in such cases.

[Excretion of porphyrin and porphyrin precursor during ethylene oxide inhalation].

The effect of chronic inhalation of ethylene oxide on urinary coproporphyrin and delta-aminolevulinic acid were studied. When Wistar male rats were exposed to 500 ppm ethylene oxide three times a week, daily urine volume was increased by 200-300% from the first week to the fifth week of the experimental period. After exposure, daily coproporphyrin excretion and urinary coproporphyrin per mg of creatinine increased by 250% and 141%, respectively. On the other hand, daily excretion of delta-aminolevulinic acid in urine tended to increase but did not increase significantly by creatinine-correction. We think this is the first report of ethylene oxide induced experimental porphyria.

Isolation and characterization of herC, a mutation of Escherichia coli affecting maintenance of ColE1.

Two modes of ColE1 DNA replication are known, one dependent on RNase H, and the other RNase H independent. The cer114 mutant of the ColE1 replicon is defective in both modes and carries a single base pair alteration 95 bp upstream of the replication origin. An Escherichia coli mutant which restored maintenance of the cer114 replicon was isolated. This host suppressor mutant is defective in RNase H and carries a herC mutation located at 62 min of the E. coli chromosome. The herC mutation is recessive to its wild-type allele and supports maintenance of the mutant replicon in the absence of RNase H. The herC mutation alone conferred cold-sensitive growth, suggesting that the herC gene product is essential for cell growth. The 1832 bp E. coli DNA fragment, containing the wild-type allele of the herC mutation, was cloned and an open reading frame for the HerC protein was determined.

Testicular toxicity and alterations of glutathione metabolism resulting from chronic inhalation of ethylene oxide in rats.

Wistar male rats were exposed to ethylene oxide (EtO) at a concentration of 500 ppm, 6 hr a day, 3 days a week, for 2, 4, 6, or 13 weeks. Testicular toxicity and changes in glutathione metabolism in the testis were investigated. The relative weights of the testes and the epididymes of the EtO-exposed group decreased in a time-dependent manner. Light microscopic examination revealed degeneration and exfoliation of germ cells. Although the severity of damage became apparent over the course of exposure, some seminiferous tubules showed germ cell recovery at 13 weeks compared with 6 weeks. There was no alteration in plasma testosterone concentration. Glutathione reductase (GR) activity decreased during the entire examination period, and recovery from the decrease was not achieved by addition of flavin adenine dinucleotide (FAD). On the other hand, glutathione peroxidase (GPx) activity decreased at 2 weeks, and then increased at 6 and 13 weeks. In spite of alterations in the glutathione redox cycle, the level of reduced glutathione (GSH) in the testes was not affected. Glutathione S-transferase activity, measured with 1-chloro-2,4-dinitrobenzene (CDNB) as substrate, increased at 6 and 13 weeks and, measured with 1,2-epoxy-3-(p-nitrophenoxy)propane, increased at 4, 6, and 13 weeks. These data indicate that chronic inhalation of EtO induces testicular atrophy. Alterations in the glutathione redox cycle and glutathione S-transferase activity might play important roles in the toxicity and the detoxifying mechanism of the testis.

Lipid peroxidation and the metabolism of glutathione in rat liver and brain following ethylene oxide inhalation.

The effects of repeated exposure to ethylene oxide on lipid peroxidation and glutathione metabolism in both rat liver and brain were examined. Increased levels of malondialdehyde (MDA) in the liver were observed after 6 and 13 weeks of exposure to ethylene oxide. The increased level of MDA observed in the hepatic homogenates of the treated rats reflected that of the microsomal fraction. On the other hand, no change in the level of MDA was detected in the brain of rats either at a 6- or 13-week treatment. Glutathione reductase activity was found to decrease at 6 or 13 weeks in liver and brain of treated rats. Both reduced and oxidized forms of glutathione in homogenates of liver and brain obtained from treated rats were, however, similar to those of the control at 40 h after the last exposure in individual experiments. To elucidate the cause of lipid peroxidation, the time course of glutathione content after exposure with ethylene oxide were studied in more detail. Significant decreases in both GSH and GSSG content in these organs were detected shortly after exposure to ethylene oxide and their levels recovered gradually with time and reached the control values at 40 h in the liver, although the changes were less significant in the brain as compared with those in the liver. These results suggest that enhancement of lipid peroxidation in the microsomal fraction of the liver after repeated exposure to ethylene oxide may possibly arise from repeated depletions of glutathione to certain critical levels and less removal of lipid peroxidation.
