[Chromosomal investigation in infertile cases of azoospermia].

Chromosomal examination in 85 patients with azoospermia revealed 62 patients (73%) with the karyotype 46, XY and 23 patients (27%) with other abnormal karyotypes. In this population, there were 17 patients with 47, XXY. Other anomalies were: 46, XYp+, 46, XXp+, 46, XYp-, 45, X, t (Ynf; 21), 45, XY, t (13q 15q), and 46, XY, r (18). Serum LH and FSH levels were high and testosterone level was low in the patients with 47, XXY compared with the other 6 patients (p less than 0.01). Also the mean value of testicular volume was low in the patients with 47, XXY (p less than 0.05).

[A case of Werner's syndrome associated with bladder cancer].

A 41-year-old woman was diagnosed as having Werner's syndrome associated with bladder cancer. The patient noticed sudden, total gross hematuria in September, 1985. Cystoscopy revealed a papillary tumor with a stalk that was accompanied by a daughter tumor and concealed the left ureteral orifice. The tumors were 25 X 20 X 10 mm and 15 X 10 X 5 mm. Double contrast cystograms, computed tomography and transurethral echo showed no invasion of muscle layer. Intravesical instillation of mitomycin (10 mg), cylocide (300 mg) and adriacin (30 mg) was carried out 3 times per week for 4 weeks. Tumor size was reduced, and then TUR was performed. High power section of the removed bladder tumor showed pathologically PNT, TCC, grade II, INF alpha, pTla, lyo and v(-). The patient had such clinical manifestations as short stature with low body weight, thin limbs and stocky trunk, senile face, early graying hair, highpitched voice, bilateral cataracts, osteoporosis, sclerodermia-like signs, flat feet, tendency toward diabetes mellitus and parental consanguinity. Hyaluronic acid was not detected in the urine. To the best of our knowledge, this seems to be the 30th report describing the association of malignancy with Werner's syndrome in Japan; besides, only one other case of bladder cancer in Werner's syndrome has been reported to date in the world.

A comparison of the effects of quinolinate and N-methyl-aspartate on neurons in rat piriform cortex.

Quinolinate has been proposed to be the endogenous ligand of the N-methyl-aspartate (NMA) receptor in the mammalian central nervous system. We have compared the relative potency and pharmacologic sensitivity of quinolinate- and NMA-induced excitation on pyramidal cells in the rat piriform cortex. Quinolinate was considerably less potent than NMA, but the NMA responses were more sensitive to antagonism by NMA-receptor antagonists. These results suggest that, at least in this preparation, quinolinate and NMA are pharmacologically distinguishable and may act at different receptor sites.

Receptors for excitatory amino acids on neurons in rat pyriform cortex.

The actions of a variety of agonists and antagonists of the excitatory amino acids on rat pyriform cortex pyramidal neurons were studied in a submerged, perfused brain slice. The order of potency for the agonists, applied by ionophoresis, was kainate greater than quisqualate greater than N-methyl-D-aspartate greater than aspartate = glutamate. The endogenous monosynaptic excitation of pyramidal neurons upon stimulation of the lateral olfactory tract was blocked post-synaptically by DL-2-amino-4-phosphonobutyric acid, although this drug did not consistently block any of the exogenous responses. The synaptic excitation was not blocked, however, by antagonists presumed specific for the quisqualate (glutamate diethyl ester), kainate, (gamma-D-glutamylglycine), or N-methyl-D-aspartate (DL-2-amino-5-phosphonovaleric acid, DL-2-amino-7-phosphonohetaonic acid) receptors. Several antagonists blocked N-methyl-D-aspartate responses at lower concentrations than those to aspartate, and other antagonists distinguished between kainate and quisqualate responses. These results suggest that 1) pyriform neurons have a variety of receptors that have properties somewhat different from those found in other preparations and 2) the endogenous transmitter activates a receptor distinct from those activated by kainate, quisqualate, and N-methyl-D-aspartate.

[A case of soft vesical calculi].

A case of soft vesical calculi is reported. A 72-year-old male, visited our clinic complaining of discharge of white muddy substance. KUB film revealed calcification in the vesical region. Urinalysis showed UTI, and culture of urine was positive for Proteus mirabilis. Thereafter, the patient suffered from discharges of similar calculi several times. Two of three infrared spectroscopic charts showed mixture of calcium phosphate and ammonium hydrogen urate and that of the remaining disclosed magnesium ammonium phosphate. An alcian blue-PAS double staining of this calculus revealed the presence of acid and neutral glycosaminoglycans, and bacterial colonies. These calculi were thought to be different from the so called 'matrix calculi'.

[Epidemiological study on urolithiasis in Mie Prefecture. 1. Present status in 1985].

To determine the present status of urolithiasis in Mie Prefecture, we analyzed the 1,314 patients of urolithiasis at 17 Departments of Urology and 2 Departments of Medicine in 1985. The ratio of male patients to female patients was 2.6 to 1. The most frequent incidence of urolithiasis was observed in Iinan county. The incidence of urolithiasis in the urban area was the same as that in the country. Most of the stones (96.9%) were in the upper urinary tract. The incidence of lower urinary tract calculi tended to be high in southern Mie Prefecture. The ratio of upper urinary tract calculi to lower urinary tract calculi in the urban area was the same as that in the country. The peak incidence in males was in the forties, while that in females was in the fifties. The average age was 44.5 years old. Ureterolithotomy was the most frequent (37.5%) surgical therapy, percutaneous nephrolithotomy and shock wave lithotomy done in 8.0% and 6.3%, respectively. The most frequent component of the urinary tract calculi was calcium oxalate and/or calcium phosphate (84.0%). The incidence increased in summer (April through September).

Neostriatal evoked inhibition and effects of dopamine on globus pallidal neurons in rat slice preparations.

Spontaneous unit discharges were recorded extracellularly from globus pallidal (GP) neurons in rat slice preparations. The firing rates of GP neurons ranged from 2.0 to 24.0 spikes/s and their firing patterns were predominantly of two types: regular and irregular. Stimulation of the neostriatum evoked two distinct types of inhibition which were dependent on GP neuronal firing patterns, a brief inhibition (about 75 ms) followed by resetting rhythmic neuronal activities and a relatively long-term inhibition (about 100 ms). These inhibitions evoked by neostriatal stimulation were attenuated or completely blocked by bath application of either bicuculline or strychnine (2 X 10(-5)-10(-4) M) but not by naloxone. Bath application of dopamine (10(-4)-10(-3) M) produced slow increases in the firing rates by 30-65% in about a half of GP neurons tested. Iontophoretic application of dopamine (10-20 nA) attenuated inhibition in GP neurons by 40-55% induced by either iontophoretically applied GABA (5-30 nA) or neostriatal stimulation without affecting their spontaneous firings. These results suggest that dopamine may produce change in the firing patterns of GP neurons by either acting directly or attenuating GABAergic inhibitory transmission from the neostriatum.

Kainic acid responses and toxicity show pronounced Ca2+ dependence.

Responses of pyramidal neurons to ionophoretic kainate, quisqualate and N-methyl aspartate were studied in a submerged rat piriform cortex slice as a function of Ca2+ and Mg2+ concentrations. The results suggest that the channel activated by kainate is unusually influenced by Ca2+, excitotoxicity is Ca2+-dependent and a function of Ca2+ concentration, and the excitotoxic actions of various amino acid agonists are correlated with the Ca2+ dependence of their responses.

N-Acetylaspartylglutamate: possible role as the neurotransmitter of the lateral olfactory tract.

N-Acetylaspartylglutamate, an endogenous brain peptide that binds with high affinity to a subpopulation of glutamate-binding sites in rat brain, is excitatory on rat piriform cortex pyramidal cells studied in a perfused brain slice. Both the monosynaptic excitation of the pyramidal cells elicited by stimulation of the lateral olfactory tract and the response to N-acetylaspartylglutamate were blocked by DL-2-amino-4-phosphonobutyrate but not by other excitatory amino acid antagonists. Responses to glutamate and aspartate, previously considered to be candidates as the lateral olfactory tract transmitter, were unaffected by 2-amino-4-phosphonobutyrate. Three days after unilateral bulbectomy there was a significant decrease in concentrations of N-acetylaspartylglutamate as well as aspartate, N-acetylaspartate, and gamma-aminobutyrate in the pyriform cortex of the side from which the olfactory bulb had been removed. These results are consistent with the possibility that N-acetylaspartylglutamate is the endogenous transmitter of the lateral olfactory tract.

[Retention cyst of the prostate gland: report of a case].

A 55-year-old man was admitted to our hospital because of pollakisuria and dysuria. Rectal examination revealed a normal prostate and did not show fluctuation or tenderness. Cystography and cystoscopic examination revealed a lesion projecting into the bladder cavity. An echogram showed an irregular internal echo at the left lobe of the prostate, but prostatic biopsy revealed benign prostatic hypertrophy. Transvesical removal of the prostatic cyst was performed. The cyst was about 3 cm in diameter and filled with yellow fluid (5.8 ml). The fluid contained no sperm and its acid phosphatase and zinc levels were high. The cystic wall was lined by cubo-collumnar cells and partly by flattened epithelium.

[A case of preputial calculi].

A case of a 73-year-old patient with preputial calculi is presented. The patient visited our hospital complaining of consciousness loss. It was impossible to place a catheter because of complete phimosis. Palpatory examination suggested the presence of stones in preputial cavity. Dorsal incision was made. After pus discharge nine small stones were extirpated. Ammonium hydrogen urate (96%) and calcium phosphate (4%) were found in these stones with infrared spectroscopic analysis. Bacteriodes ureolytcus and Peptostreptcoccus asaccharolyticus were isolated from urine specimen.

[Six cases of spontaneous peripelvic extravasation from the renal pelvis].

The diagnosis and management of 6 patients with spontaneous non-traumatic urinary extravasation are reported. It is important to distinguish extravasation of the fornical backflow type from that owing to frank rupture of the diseased renal pelvis. In Japan, 26 cases of spontaneous peripelvic extravasation and 29 cases of spontaneous pelvic rupture have been reported. Most of them were caused by calculous ureteral obstruction. Many of the cases of spontaneous peripelvic extravasation could be managed conservatively, and many of the cases of spontaneous pelvic rupture often required surgical treatment due to the patient's clinical condition, the persistence of obstruction or extravasation, or the presence of complication of extravasation such as urinoma or abscess. All of our 6 cases were managed conservatively and had no complications.

Action of vasopressin on CA1 pyramidal neurons in rat hippocampal slices.

The effects of arginine-vasopressin (AVP) on the excitability of 47 pyramidal cells of the CA1 region of the hippocampus were determined by using intracellular recording techniques in a submerged slice preparation. Addition of 10(-6) M AVP to the bathing medium evoked an increase in spike discharge which was slow in onset and only gradually reversible. The discharge was accompanied by an increase in excitatory postsynaptic potentials without significant change of the resting input resistance. AVP-induced excitation was found in 81% of ventral and 29% of dorsal hippocampal CA1 pyramidal cells. In low Ca2+, high Mg2+ solution this excitatory action by AVP was blocked. Microiontophoretic application of AVP onto apical or basal dendrites or the cell body did not result in excitation. These observations suggest that the action of AVP on CA1 pyramidal cells is transsynaptic and is more pronounced in ventral than dorsal CA1.
