Patients who have exhausted all options for conventional dialysis access due to lack of available vessels may be candidates for placement of a synthetic axillofemoral vascular graft. The axillofemoral access is surgically constructed under general anesthesia. The synthetic graft is anastomosed, end to side, to the axillary artery, proximally, and distally to the common femoral vein. Surgical complications have been negligible and intravascular blood flow rates support efficient dialysis. A review of two cases demonstrates the positive outcomes and acceptability of this nonconventional approach for blood access in hemodialysis patients.

Findings of this study of the role and functions of nurses working with renal dialysis and transplant teams between 1915 and 1970 suggest that many of the activities of dialysis and transplant nurses were assistive and technical in nature. However, further analysis of the characteristics of direct nursing care demonstrates that early nephrology nurses incorporated family support, interpersonal communication, and patient teaching as essential components in order to assist patients and their families in coping with the stresses of receiving these new experimental treatments.
The American Association for Nephrology Nurses (AANN), the predecessor of the American Nephrology Nurses' Association (ANNA), was formed in April 1969, in Atlantic City, New Jersey, but the beginnings of the organization reach back to the early 1960s. This article will review the events leading to the formation of AANN and discuss sources of support for the effort.

The authors define a professional practice model (PPM) as a system (structure, process, and values) that supports registered nurse control over the delivery of nursing care and the environment in which care is delivered. A PPM has five subsystems: values, professional relationships, a patient care delivery model, a management approach, and compensation and rewards. Analysis of five PPMs described in the literature showed that the professional values addressed most often are nurse autonomy, nurse accountability, professional development, and emphasis on high-quality care. Teamwork, collaboration, and consultation consistently appeared as approaches to enhance professional relationships. Primary nursing and case management are the care delivery systems most often used in the models. Decentralizing decision making, expanding the scope and type of unit nurse manager responsibilities, and instituting structural changes to support professional practice were common activities in the management subsystem. Compensation and reward systems generally recognized professional achievement and contribution toward organizational goals. Recommendations for implementing and evaluating PPMs are offered.
Many doctoral programs in nursing now offer at least one course in qualitative research. Yet it remains a challenge to adequately prepare new qualitative researchers due to lack of faculty expertise and mentorship, student unfamiliarity with qualitative research when entering the doctoral program, and uncertainty as to appropriate teaching methods. The authors describe how they addressed these issues historically and in their current two-course qualitative research sequence. Accompanying reports of four studies conducted by students who completed the two-course sequence during academic year 1996 to 1997 provide evidence of the viability of the authors' approach and serve as the basis for recommendations to others who use a similar strategy for teaching qualitative research.

To assess the current status of health services research in nursing, 113 studies reported between 1980 and 1989 were reviewed. Identified gaps in this research included comprehensive cost-benefit and cost-effectiveness analyses, effects of technology on patient care, evaluations of information systems, evaluations of innovative care delivery approaches, and studies of ethical issues related to the use of scarce resources.

The authors present an orientation model derived from a naturalistic study of a nursing orientation program. This model includes the organizational aspects of orientation, as well as characteristics related to the person being oriented. Included in the model is an affective element of welcoming newcomers to a facility and an assessment of clinical competency. In addition, the model includes the educational and managerial requirements of the process and emphasizes the importance of the preceptor.
