Molecular nature and possible presence of a membranous glycan-phosphatidylinositol anchor of CA125 antigen.

The molecular nature and possible presence of a glycan-phosphatidylinositol anchor (GPI-anchor) in CA125 molecules was investigated. Serial lectin affinity chromatography and N- or O-glycanase treatment to reduce antigenicity showed that CA125 contained certain N- and O-glycosylated sugar chains in the molecule, like a glycoprotein. CA125 released from ovarian cancer tissues increased time-dependently following phosphatidylinositol-specific phospholipase C (PI-PLC) treatment, concomitant with the release of tissue-unspecific alkaline phosphatase. Western blotting of CA125 treated by PI-PLC showed a single band of 90 kD instead of the 162- and 76-kD bands of the native antigen. Further, ovarian cancer tissues subjected to PI-PLC treatment lost the immunohistochemical localization of CA125 with OC125 antibody. Consequently, it is strongly suggested that CA125 is a glycoprotein that has both N- and O-linked sugar chains and a membranous GPI-anchoring moiety, and further, that its 90-kD form is the antigen without the GPI-anchor.

[Primary multiple neurilemmoma of the diaphragm].

Primary tumor of the diaphragm is very rare. Furthermore, to our knowledge, no cases of multiple neurilemmoma of the diaphragm have appeared in the literature. A 61-year-old female was admitted to our hospital because of epigastric discomfort. Roentgenologic examination of the chest showed a tumor in the left leaf of the diaphragm. Laparotomy revealed two tumors, one measuring about 5 cm in diameter, and the other about 1 cm in diameter arising from the diaphragm. There was no adhesion to adjacent organs. The two tumors were resected with a part of the left diaphragm. Histological examination revealed benign neurilemmoma of the diaphragm. There has been follow-up on the patient for two and a half years without evidence of recurrence.

Use of pedicled omental flap in treatment of empyema.

Omental pedicle flaps were used in the treatment of patients with acute and chronic empyema with bronchopleural fistula. In 5 patients (group 1) with postoperative acute empyema owing to bronchial stump fistula, an omental covering was applied as a reinforcement to close the fistula. Six patients in group 2 with chronic tuberculous or Aspergillus empyema with multiple fistulas initially underwent open-window thoracostomy or cavernostomy and secondarily had omental transposition. In all patients, the right gastroepiploic vessels were used to provide the blood supply for the flap. Successful closure of the bronchial stump was obtained in all patients in group 1, but 2 of them died of recurrence of their underlying lung carcinoma within 1 year. Five of the 7 patients in group 2 had a favorable outcome, but 2 patients had partial recurrence after omental plombage. From our experience with these patients, we believe that the omental flap is effective for closing fistulas due to postoperative or chronic empyema but has only limited success in patients whose lungs are severely damaged by persistent infection.

Developmental changes in the antigenicity and sugar-chain heterogeneity of rabbit alkaline phosphatases.

1. Rabbit alkaline phosphatases (APs) clearly fused with the anti-human AP antibodies. In particular, fetal liver and kidney APs reacted slightly less with the anti-intestinal AP antibody as did adult enzymes, suggesting that intestinal AP-like isozyme is expressed at earlier stages of gestation in rabbit liver and kidney. 2. Immunohistochemical data indicated that intestinal AP-like isozyme in the kidney was mainly localized in the distal convoluted tubules and slightly in the proximal straight tubules, whereas liver/bone/kidney AP-like enzyme was found more in the glomeruli and interstitial capillary walls as a major component. 3. The sugar-chain heterogeneity of adult and fetal rabbit APs displayed organ-specificity as did of rat and human APs. Moreover, in fetal development, the expression of high-mannose type or hybrid type sugar chains precedes the expression of complex type sugar chains in fetal development.

Suppression of an epidermal growth factor receptor-hyperproducing tumor by an immunotoxin conjugate of gelonin and a monoclonal anti-epidermal growth factor receptor antibody.

An immunotoxin was made by conjugating a murine monoclonal antibody (B4G7) that recognizes the human epidermal growth factor (EGF) receptor with gelonin, a ribosome-inactivating protein. This B4G7-gelonin conjugate was shown to be specifically cytotoxic for EGF receptor-hyperproducing cells. The conjugate was tested in nude mice and shown to be capable of suppressing the growth of an EGF receptor-hyperproducing squamous carcinoma cell (A431) solid tumor. Nude mice bearing an A431 cell tumor that were given injections i.p. for 5 consecutive days with at least 10 micrograms of the conjugate showed significant suppression of tumor growth for about 7 days. On the other hand, an unconjugated mixture of B4G7 and gelonin showed no specific antitumor activity against the A431 cell tumor. The growth of an EGF receptor-deficient small cell lung cancer cell (H69) tumor was not suppressed by injection of the conjugate. No toxic effects were observed in histological examination of nontumorous tissues of mice treated with at least 250 micrograms of conjugate per mouse. These results suggest that the conjugate may be useful for targeting therapy to EGF receptor-hyperproducing squamous carcinoma.

Surgical treatment of pulmonary aspergilloma and Aspergillus empyema.

Twenty-four patients, 16 male and 8 female, underwent a total of 35 operations for pulmonary aspergillosis. Intrapulmonary aspergilloma was detected in 19, and Aspergillus empyema was present in 5. The major operative procedures performed were pneumonectomy in 2 patients, lobectomy in 8, segmentectomy in 1, cavernostomy in 4, and open-window thoracostomy in 5. The surgical results in 5 patients 70 years old or older were excellent. Empyema developed postoperatively in 2 patients who had undergone wedge resection of the lung or segmentectomy. Although resection involving the minimum extent possible is desirable in the treatment of intrapulmonary aspergilloma so as not to decrease lung function, it is dangerous to perform a limited procedure in the case of aspergilloma with an invasive character. In patients in poor general condition, cavernostomy followed by muscle flap plombage is recommended. For Aspergillus empyema, the primary procedure was open-window thoracostomy followed by plombage using chest wall muscle or omentum. We consider omental flap plombage to be superior to thoracoplasty in some respects for mycotic empyema, especially because it is a less extensive surgical procedure.

Stimulus specificity of peripherally evoked substance P release from the rabbit dorsal horn in situ.

Characteristics of in situ substance P release from the lumbar dorsal horn were investigated in decerebrated rabbits. Noxious mechanical stimuli produced by pinching the skin of a hind leg ipsilateral to the perfusion site remarkably and significantly increased the release of immunoreactive substance P, which was identified as substance P itself, using separation with high-performance liquid chromatography and radioimmunoassay. The noxious pinch did not affect the release of immunoreactive substance P, when applied to the contralateral hind leg. Both the basal and pinch-evoked release of immunoreactive substance P were largest in the dorsolateral part of the dorsal horn. The pinch-evoked release of immunoreactive substance P was abolished when the dorsal horn was perfused with a Ca2+-free medium containing 7 mM Mg2+ or with a medium with 10 microM tetrodotoxin added. The evoked release of immunoreactive substance P was also abolished following pretreatment of a stimulated region with the local anesthetic dibucaine, a procedure which inhibited the pinch-evoked aversive behavior in freely-moving rabbits. Among a variety of natural stimuli applied to the hind leg, noxious pinch and a subcutaneous injection of formaldehyde solution significantly evoked the release of immunoreactive substance P from the dorsal horn. The most intense heat or scalding stimulation increased the immunoreactive substance P release in two out of five experiments. However, other natural stimuli such as ice-cold, warm, noxious heat and innocuous mechanical stimuli produced no apparent changes in the release of immunoreactive substance P. These results suggest that among the noxious stimuli, only mechanical and inflammatory but not thermal stimuli lead to a release of substance P from the primary afferent terminals in the dorsal horn. The present findings suggest that, at least in rabbits, substance P-containing primary afferents have high-threshold mechanoreceptors. Substance P may participate in the transmission of information related to noxious mechanical and inflammatory stimulation from the periphery to the dorsal horn.

Recurrent pancreatitis in the third trimester in 2 consecutive pregnancies--its relationship to liver histology: a case report.

A 20-year-old woman who had recurrent pancreatitis in the third trimester in 2 consecutive pregnancies was reported. Her liver histology was indistinguishable from acute fatty liver of pregnancy, although she showed no clinical signs. The relationship between idiopathic acute pancreatitis during pregnancy and acute fatty liver of pregnancy was discussed.

Hepatocellular lipofuscin in pre-eclampsia.

Thirty liver specimens from 30 pre-eclamptic women with and without liver dysfunction were examined for the amount of lipofuscin pigment deposited. The amount of hepatic lipofuscin correlated positively with plasma urate concentrations. This finding may indicate an involvement of oxygen-free radicals and xanthine oxidase in the pathophysiology of pre-eclampsia.

Results of resection for bronchogenic carcinoma in patients over the age of 80.

Thirty three patients aged over 80 years underwent resection for bronchogenic carcinoma. The operations performed were: lobectomy (21), segmentectomy (4), wedge resection (2), pneumonectomy (3), carinal resection (1). In two patients no resection was feasible. Three patients died within two months of surgery. The cumulative five year survival rate was 55%, 79% for patients with stage I carcinoma and 31% for stage III. It is considered that resection has an acceptable outcome in patients over 80 years.

[Promoting effect of endogenous TSH on carcinogenesis of rat thyroid transplanted into the spleen].

In an attempt to clarify a modifying effect of TSH on thyroid tumorigenesis, we carried out intrasplenic thyroid autotransplantation in rats treated with a thyroid carcinogen, diisopropanolnitrosamine (DIPN). Experiments comprised 3 groups of Wistar male rats: Group I (effective no., 15), total thyroidectomy plus hemithyroid transplantation 2 weeks after subcutaneous injection of DIPN at a dose of 2.4g/kg BW; group II (effective no., 22), DIPN alone; group III (effective no., 18), transplantation alone. The thyroid lesions of all rats at week 28 were examined by histopathology and enzyme histochemistry for gamma-glutamyl transpeptidase (GGT). Periodical radioimmunoassay for serum T3, T4 and TSH revealed low levels of T3 & T4 for at least 10 weeks and increased level of TSH until week 18 after the transplantation. Altered foci (AF) occurred in 36% and 6% of rats in group I and group II, respectively, whereas neoplastic nodules (NN) were found in 53% of rats only in group I. Both AF and NN displayed a positive reaction for GGT which was considered to be a useful marker for preneoplastic and neoplastic thyroid lesions. The present data are indicative that increased serum level of TSH for a significant period may exert a promoting effect on thyroid tumorigenesis.

[Results of surgical treatment in patients aged 80 years and over].

Surgical treatments were performed in 31 patients over 80-year-old with lung cancer. Men were 25 and women were 6, the eldest patient was 92-year-old male. Pneumonectomy was carried out in 3, lobectomy in 19, carinal resection in 1, Segmentectomy in 4, and partial resection of lung in 2. Histologies of resected specimen in 31 included 11 epidermoid carcinoma, 15 adenocarcinoma and 5 other cell types. There was no patients died within 30 days postoperatively. The cumulative 5 year-survival rate was 53.9%.

Determination of cortisol in human plasma by capillary gas chromatography-mass spectrometry using [2H5] cortisol as an internal standard.

A capillary gas chromatographic-mass spectrometric method for the determination of cortisol in human plasma using cortisol M + 5 as an internal standard is described. For calculation of plasma cortisol, peak areas were measured by selected-ion monitoring of the characteristic fragment ions of the dimethoxime-tri (trimethylsilyl) derivatives of cortisol and cortisol M + 5 (m/z 605 and 610, respectively). The inter- and intra-assay coefficients of variation for plasma sample were 3.07 and 1.77%, respectively. The method needed no complex corrections for contributions and provides a sensitive and reliable technique with good accuracy, precision and reproducibility.

Determination of cortisol in human plasma by stable isotope dilution mass spectrometry.

Cortisol selectively labelled with 2H at the 19-methyl and the C-1 position (cortisol-d5) was synthesized and used as an internal standard in stable isotope dilution mass spectrometry in order to determine cortisol in human plasma. A capillary gas chromatographic/mass spectrometric method provided a sensitive and reliable technique with good accuracy, precision and reproducibility without complex corrections for contributions by using cortisol-d5 as an internal standard. For calculation of plasma cortisol, peak areas were measured by selected ion monitoring on the characteristic fragment ions of the dimethoxime tri(trimethylsilyl) derivatives of cortisol and cortisol-d5 (m/z 605 and 610, respectively). The sensitivity of the gas chromatographic/mass spectrometric assay was 1.02 ng per injection, with a signal-to-noise ratio of about 6. The inter- and intra-assay coefficients of variation for plasma sample were 3.07% and 1.77%, respectively.

Preeclampsia: a microvesicular fat disease of the liver?

To study the interrelationship between preeclampsia, the syndrome of hemolysis, elevated liver enzymes, and low platelet count, and acute fatty liver of pregnancy, 41 liver specimens from 41 preeclamptic women with and without liver dysfunction were examined for the amount of fat deposited in hepatocytes. All 41 specimens stained with oil red O on frozen sections showed a significant amount of microvesicular fat droplets in varying degrees. In contrast, only 11 of the 41 stained conventionally (with hematoxylin-eosin) showed significant fatty infiltration. The density of hepatocellular fat correlated positively with plasma urate concentration and negatively with the platelet count. These findings suggest that preeclampsia may be one of several microvesicular fatty diseases of the liver and that acute fatty liver of pregnancy may be the most severe form.
