Determination of financial competence in consultation-liaison psychiatry.

The aging of our population will require physicians to become more aware of the issues surrounding the determination of financial competence. This retrospective study involved the review of 46 medical or surgical patients in a university teaching hospital that were referred to the psychiatric consultation-liaison team for assessment of financial competency. Charts were reviewed for completeness of documentation using suggested criteria for determination of financial competence. The study found that while many criteria were frequently documented, documentation was incomplete for important criteria, such as calculating abilities, judgement, the patient's appreciation of the consequences of financial competence, presence of delusions with regard to finances and the patient's awareness of debt. Concerns are expressed regarding a physician's ability to recall details pertaining to cases before a tribunal without complete and careful documentation. The importance of establishing formal criteria for the determination of financial competence is also discussed.

Intensive early exposure to geriatric psychiatry in residency training: impact on career choice and practice.

The authors examined the impact of intensive early exposure to geriatric psychiatry on the career choice of trainees. All psychiatrists who had completed a six month inpatient rotation on a general hospital psychogeriatric unit as part of their first year of residency were interviewed three to 12 years later with a questionnaire designed to elicit practice patterns, prior interest in geriatrics, and the major influences of the rotation. Nine of 16 trainees had spent more than 50% of their time since graduation treating elderly patients. Those surveyed who were not practicing geriatrics spontaneously reported positive influences of the rotation on psychiatry training in general. The nature of geriatric psychiatry and the clinical issues, as well as the supervisors and educational activities were rated as the most important influences of the rotation. Early experience in psychogeriatrics seems valuable for subspecialty recruitment as well as general psychiatry training.

Behavioural disturbances in the demented elderly: phenomenology, pharmacotherapy and behavioural management.

Behavioural disturbances in the demented elderly cause a significant amount of distress both to the patients and their caregivers. This article first summarizes the phenomenology associated with these disturbances. It then deals with the pharmacological methods of reducing these disturbances. Finally, it deals with some of the more recent advances in combining the insights of behavioural modification with those of neuropsychology in finding non pharmacological methods of reducing problematic behaviours. It is stressed that a combination of the two approaches is most likely to be required, and most likely to be successful, in the individual case.

Precision of estimating the time since death by vitreous potassium--comparison of two different equations.

The precision of death time estimation by vitreous potassium using two different formulas [1-3] is compared on an independent random sample of 100 cases. The very flat slope in Sturner's equation is the reason for a systematic overestimation of the time since death with much wider 95%-limits of confidence compared to the results using an own equation with a steeper slope of vitreous potassium.

Drug-induced catatonia.

An 81 year old depressed female is described who developed catatonic-like behaviours while on the combination of the monoamine oxidase inhibitor phenelzine, and the neuroleptic haloperidol. Alternative etiologies, and the roles of the individual agents are discussed. It is suggested that whenever there is evidence of drug-induced catatonia, consideration should be given to stopping all medications.

Delusional double mourning: a complication of bereavement in dementia.

We describe two elderly patients with senile dementia who presented with the delusion that a parent had died within weeks of the actual death of a spouse. These fixed, systematized, and persistent beliefs do not correspond to the delusions of dementia or of the bereaved elderly that have been previously described. The phenomenology and implications of these "delusions of double mourning" are discussed.
