In an attempt to determine the source of cognitive impairment in 106 consecutively admitted patients at the Johns Hopkins Chronic Pain Treatment Center, EEG, the Wechsler Adult Intelligence Scale, Memory Quotient, and Bender Gestalt tests were administered. Patients receiving benzodiazepines alone demonstrated alterations in cognitive functioning and EEG evidence of a sedative effect. Patients receiving narcotics alone and a group of patients not receiving medication did not show signs of cognitive impairment. The effects of benzodiazepines on sleep and perception of chronic pain, in combination with the cortical changes that they produce, imply that these drugs should not be used in most patients with chronic pain.

Infectious mononucleosis is usually thought to be a benign disease with occasional neurologic sequelae. Depression, incoordination, a reduction in intellectual ability, and altered EEG patterns were found in two patients; one recovered and the other seemed to have permanent residual effects. The possibility of tranylcypromine as a treatment for the depression and appropriate counselling of patient and family are discussed.

Plasma aldosterone levels of seven untreated manic-depressive patients in the manic phase were compared to five untreated schizoaffective patients in the excited phase and 18 controls. Aldosterone was significantly elevated in the manic patients, and elevated in the schizoaffective patients. After 14 days of lithium carbonate therapy, both psychiatric groups showed a significant reduction in plasma aldosterone and a marked clinical improvement. These data suggest that elevated aldosterone is associated with the manic phase of manic-depressive psychosis.

This study followed 120 chronic pain patients referred to a multidisciplinary pain center. The referral diagnosis for many patients, such as "chronic pain," "psychogenic pain," or "lumbar strain," was frequently found to be incomplete or inaccurate (40%) following a multidisciplinary evaluation that used appropriate diagnostic studies, including magnetic resonance imaging, computed tomography, nerve blocks, and qualitative flowmeter. Significant abnormalities were discovered in 76% of the diagnostic tests. An organic origin for pain was found in 98% of these patients. The patients were discharged with objective verification of diagnoses including facet disease, nerve entrapment, temporomandibular joint disease, thoracic outlet syndrome, and herniated discs.
