A prospective randomized study on the effect of doxycycline alone, tinidazole alone and the two antimicrobials in two different combinations as prophylactic antimicrobials on elective colorectal surgery is presented (Table II). The number of patients included in the study was 265. Of the four different treatment groups, the one which consisted of a single preoperative dose of tinidazole 2 g and doxycycline 200 mg, seemed to give the most favourable result. In this treatment group (n = 70), wound sepsis was recorded in 7.2% and perineal sepsis in 34.8% of the cases. Only one serious, but non-fatal intra-abdominal complication (1.4%) was observed in this group. The trial did not include a placebo group.
Geographical variation in clinical presentation may be a key obstacle to accurate diagnosis of patients with acute abdominal pain. This paper studies two series of patients presenting to hospital with an "acute abdomen"; 552 patients from Leeds, England and 398 patients from Akershus, Norway. The causative disease patterns and detailed clinical presentation are compared and contrasted (where appropriate) with a larger series of 6,097 patients under study by the World Organisation of Gastroenterology. Finally, the report outlines the results of implementing automated (computer-aided) diagnostic systems both in Leeds and Akershus, and the effects of adopting a structured case history form for data-recording when the patient is first seen.

A case of tracheal stenosis secondary to tracheostomy and respirator treatment is presented. The stenosis caused acute respiratory failure, which made immediate surgical intervention mandatory. It is stressed that usually the tracheal lumen is severely reduced when the stenosis causes alarming symptoms. Rational treatment, which must be instituted immediately, consists of resection of the stenotic area and end-to-end anastomosis.
