Cutaneous and ocular side-effects of oral photochemotherapy: results of an 8-year follow-up study.

To determine the long-term cutaneous side-effects of oral photochemotherapy (PUVA), we examined 95 patients, 59 with psoriasis and 36 with mycosis fungoides (MF). These comprised 80% and 69% respectively of the patients with these disorders treated with PUVA in our department from 1977 to 1985. Two psoriatic patients had squamous carcinomas, both of whom had received high cumulative UVA doses and also methotrexate concurrently with PUVA. Six patients with MF had actinic keratoses. The mean age of these patients (69 years) was significantly greater than the mean age of the patients without actinic keratoses (54 years), but there was no significant difference in their cumulative UVA doses. No patients developed basal cell carcinomas or malignant melanoma. 'PUVA lentigines' were found in 46% of the patients. They were most frequent in patients currently being treated and in those who had received high cumulative UVA doses, but persisted for up to 7 years after discontinuing therapy. Seventy-one patients had yearly ophthalmological examinations, or a single examination at least 3 months after commencing PUVA. This examination included retinal function tests to detect any subclinical visual impairment. Five of these patients had cataract prior to PUVA therapy, and were significantly older (mean age 71 years) than those without cataract (mean age 53 years). Three patients (mean age 61 years) developed new lens opacities whilst receiving PUVA. However, none of these patients was considered to have cataract as none had impairment of visual acuity due to lens opacity. No patients without lens opacity developed evidence of subclinical visual impairment.

Neutrophil leukocyte morphology, cigarette smoking, and palmoplantar pustulosis.

Neutrophil leukocyte morphology was examined in whole blood films from 20 patients with palmoplantar pustulosis (PPP) and 32 healthy controls. In the PPP patients, there was a significant increase in the number of neutrophils having polarized morphology or membrane ruffling; however, there was no significant difference in neutrophil morphology between cigarette smokers and nonsmokers, suggesting that the epidemiologic link between smoking and PPP is not explained by increased polarization of peripheral blood neutrophils.

Malignant melanoma of the head and neck in Scotland: an eight-year analysis of trends in prevalence, distribution and prognosis.

Four hundred and eighteen cases of primary cutaneous malignant melanoma of the head and neck were registered with the Scottish Melanoma Group between 1979 and 1985. Distribution of histogenetic types of melanoma was lentigo maligna melanoma 50 per cent, superficial spreading melanoma 24 per cent, nodular melanoma 20 per cent, unclassifiable 6 per cent. Annual registrations of all types increased over the seven-year period but were less for lentigo maligna melanoma than for the other types. Patients with lentigo maligna melanoma were older, and those with superficial spreading melanoma younger, than the overall average. These two features support the hypothesis that the former may be more closely related to cumulative sunlight exposure than are other histogenetic types of melanoma. The sex ratio (F:M) was 1.7:1 for lentigo maligna melanoma, 1.5:1 for superficial spreading melanoma, but 1:1 for nodular melanoma. All types of melanoma were seen most commonly on the cheek. The proportion of lentigo maligna melanoma on the cheek was significantly greater than nodular melanoma or superficial spreading melanoma. Superficial spreading melanoma was significantly increased in frequency on the neck and scalp, and nodular melanoma on the ear. Melanomas of the scalp were significantly more frequent in male patients than female. The anatomical site distribution of nodular melanoma showed considerable differences between the sexes: nodular melanoma of the ear or neck occurred more commonly in male patients and nodular melanoma of the cheek was significantly more frequent in female patients. Five-year mortality data for head and neck melanoma derived from patients registered between 1979 and 1981 revealed that mortality was greater in male than in female patients, but there was no significant difference in prognosis between the three histogenetic types of melanoma when these were stratified according to tumour thickness.

An annular erythema of infancy.

An unusual annular and arciform erythema in an infant is reported. Skin lesions appeared when the child was 4 days old, and multiple lesions have been present during the subsequent two years. Individual lesions gradually enlarge over a period of a few weeks, and they resolve spontaneously without any residual cutaneous abnormality. This annular erythema is clinically and histologically distinct from previously described annular erythemas of infancy, with which it is compared.

The prognostic value of blood glucose and glycosylated haemoglobin estimation in patients with stroke.

In a prospective study of 109 patients admitted to hospital with a provisional diagnosis of acute stroke, 87 were found to have acute hemiplegic stroke lasting more than 24 hours, and did not have any other co-existing life-threatening disorder. In 81 of these patients, blood glucose and glycosylated haemoglobin (HbA1) were measured at the time of admission. Thirteen of these 81 patients (16%) were hyperglycaemic--in 5 cases, normal HbA1 was found in conjunction with hyperglycaemia suggesting that this represented a 'stress' response. There was no significant difference in age or in blood glucose level between those who died as a result of stroke and those who survived. However, hyperglycaemia with normal HbA1 was demonstrated in 4 of 26 patients who died compared to only one of 55 survivors (P less than 0.02), and all 3 patients with blood glucose greater than 10 mmol/l in conjunction with normal HbA1 died as a result of stroke. Biochemical evidence of 'stress' hyperglycaemia in patients with acute stroke suggests a poor prognosis.

Atypical lichen planus associated with ulcerative colitis.

A case is reported of a patient known to have ulcerative colitis who developed atypical lichen planus. This did not resolve when the patient discontinued sulphasalazine treatment but became more extensive within 48 h of an exacerbation of colitis. This case supports the view that there may be an association between ulcerative colitis and lichen planus.

Lowered ultraviolet minimal erythema dose in hemiplegia.

In view of recent reports of increased tanning in hemiplegic limbs, we have investigated ultraviolet (u.v.) minimal erythema dose (MED) in hemiplegia using the bilateral comparison technique. Seven of 10 patients had a lower MED in the hemiplegic arm compared to the normal side, the mean reduction being 16% (range 0-33%, P = 0.003). No patients had a higher MED in the hemiplegic arm. We review the literature regarding other non-neurological features of hemiplegia, in particular asymmetry of temperature, oedema, and finger clubbing, and we propose a vasomotor or trophic aetiology for these findings.

Crossed-leg callosities.

We report five patients in whom localised callosities were found on the dorsum of the feet. This was bilateral in four cases and was due to repetitive minor trauma associated with the sitting posture adopted by the patients. Three of the patients also had psoriasis vulgaris which was clinically dissimilar to the lesions we describe.
