[In vitro glucose consumption in severe hyperleukocytosis. A cause of factitious hypoglycemia].

Following the observation, in a patient with leukemia, of a factitious hypoglycemia that was related to in vitro glucose consumption by leukocytes, a study of glucose fall in vitro in the blood of 17 patients with leukemia of various types and with various grades of leukocytosis was undertaken. Results show that in vitro glucose consumption is linked to the intensity of leukocytes concentration, and is independant of the type of leukocyte. Factitious hypoglycemia may thus frequently occur in patients with high blood leukocyte count. This phenomenon, that was previously described, is again of interest to day because many laboratories do not yet use any more glycolysis inhibitors that partially inhibit it. Indeed, these inhibitors interfere with measures of other biological parameters that are simultaneously determined in autoanalysers.

[Comparative study of 2 prognostic classification systems for myeloma and examination of the correlation between initial bone marrow plasmacytosis and prognosis. A propos of a homogeneous population of 50 patients followed from diagnosis to death].

We report the outcome of the statistical analysis of 50 myeloma patients, making a comparison of the prognosis value of the staging systems proposed by Durie and Salmon and, more recently by Merlini, Waldenström and Jayakar. Moreover, we studied the relationship between initial percentage of bone marrow plasma cells and prognosis. All patients had a complete follow-up and received the same treatment. We excluded our cases of non excreting and solitary myeloma. The staging system proposed by Durie and Salmon still has the best prognosis significance. The system proposed by Merlini, Waldenström and Jayakar does not allow a staging of the patients in rigourous conditions. The absence of a relationship between initial bone marrow plasmocytosis and prognosis corroborates the poor reliability of this staging system.

[Candida albicans spondylitis. Review of the literature apropos of a case with study of bone penetration of 5-fluorocytosine].

In spite of the development in cases of deep mycoses, cases of osteoarthritis with Candida remain rare (only thirty seven cases reported). Among these, ten observations of spondylodiscitis have been collected and the authors report a new case where Candida albicans was isolated in the discovertebral focus through bone puncture. This spondylodiscitis occurring in a leukemic patient receiving antibiotics, corticosteroids, cytostatics drugs and radiotherapy, was treated successfully by an association of Amphotericin B-5-Fluorocytosine. The authors take this opportunity to report the main characteristics of the ten previously described cases specifying the rules for the uses of fungicidal drugs in the treatment of deep mycoses and for the first time, they study simultaneously seric concentration, penetration into healthy bone and into discovertebral focus of the 5-Fluorocytosine. They also justify their preference for the association of Amphotericin B-5-Fluorocytosine as a "first line" treatment after confirming the sensitivity of the strain to 5-Fluorcytosine.

[Drug-induced benign intracranial hypertension. Apropos of a case with amphotericin B. Review of the literature].

Benign intracranial hypertension (BICH) is a rare adverse event. We report the case of a 31-year-old female drug addict who had been seropositive for HIV since 1987. She had stage IV C1 AIDS, and was receiving intravenous amphotericin B for generalized cryptococcosis with no neuromeningeal involvement. She developed BICH that regressed when the antifungal drug was withdrawn and treatment for cerebral edema was started. BICH is a clinical entity involving intracranial hypertension with no focal neurological signs or detectable intracranial lesion. The manifestations include headache, transitory or permanent visual disturbances (diplopia, loss of visual acuity) and the perception of intracranial noise. The cerebrospinal fluid is under increased pressure but the composition is normal. The eye fundus examination shows papillary edema, and the neuroradiological workup is normal. BICH can only be diagnosed once an expansive intracranial process, neuromeningeal infection, and non-communicative hydrocephalus have been ruled out. In the majority of cases, no etiology is found. Such cases of idiopathic BICH usually occur in overweight young women, although drugs can be implicated. Amphotericin B has not previously been held responsible for BICH. On the basis of this observation, we present a review of the literature.
