Intranasal or subcutaneous co-administration of recombinant cholera toxin B subunit stimulates only a slight or no level of the specific IgE response in mice to tetanus toxoid.

Whether recombinant cholera toxin B subunit (rCTB) co-administered intranasally or subcutaneously with aluminium-non-adsorbed tetanus toxoid (nTT) can induce the production of tetanus toxoid (TT)-specific IgE antibodies in mice was investigated compared with aluminium-adsorbed tetanus toxoid (aTT) administered intranasally or subcutaneously. Mice immunized intranasally or subcutaneously with nTT together with rCTB showed a high level of TT-specific serum IgG antibody response and no or a slight level of TT-specific serum IgE antibody response. On the other hand, in mice vaccinated intranasally or subcutaneously with aTT alone, higher levels of TT-specific IgG and IgE antibodies were induced in comparison with intranasal or subcutaneous inoculation of nTT together with rCTB. These results suggest that intranasal or subcutaneous co-administration of rCTB with nTT is better than intranasal or subcutaneous administration of aTT to avoid IgE-mediated allergic reactions.

A DNA probe specific to Streptococcus sobrinus.

Three DNA fragments (SSB-1, -2 and -3) in the dextranase gene (dex) of Streptococcus sobrinus were amplified by polymerase chain reaction and used as DNA probes. The probes were examined for the specificity and the sensitivity of hybridization with DNA of oral streptococcal species. While probes SSB-1 and SSB-2 were specific to both S. sobrinus and Streptococcus downei, SSB-3 was specific only to S. sobrinus. SSB-3 was able to detect 5 ng of chromosomal DNA purified from S. sobrinus NIDR6715 and DNA extracted from 1 x 10(5) cells of the strain. In addition, SSB-3 could differentiate clinical isolates of S. sobrinus from Streptococcus mutans. These results suggest that SSB-3 is an effective DNA-probe to detect and to identify S. sobrinus.

Lung cancer in patients under 50 years old.

A long-term retrospective study was carried out on 790 cases of lung cancer to determine if the clinicopathologic characteristics and survival rates of lung cancer patients under the age of 50 differ from those of patients 50 years of age or older at diagnosis by analyzing data on patients registered at Tochigi Cancer Center Hospital. Of the 790 patients, 77 (9.7%) were under the age of 50 at diagnosis. The percentage of women in the younger patient group was significantly higher than that in the older patient group (39.0% vs. 27.5%; P = 0.034). Tumor histology revealed a significant preponderance of adenocarcinomas (60 patients, 77.9%) and a paucity of squamous cell carcinomas (8 patients, 10.4%) in the younger age group (P<0.001). The preponderance of adenocarcinoma was significant in both males and females (male: P = 0.004, female: P = 0.004). Smoking rates and rate of detection by cancer screening did not differ between the two age groups. Because of the paucity of smokers among the younger female patients, causes of lung cancer other than smoking should be sought in younger patients. No difference was found in the stage of the disease at presentation, treatment methods and survival rates between the two age groups. It is suggested that the prognosis for patients with lung cancer under the age of 50 is not significantly worse than for those aged 50 years or older, as has been shown by several investigators.

[An outbreak of parainfluenza 3 virus infection in the elderly in a ward].

In the D ward of Nagoyashi-Koseiin geriatric hospital (36-beds), upper respiratory illnesses were recognized in all the inpatients between July and August in 1995, and we studied 7 elderly subjects with parainfluenza 3 infection diagnosed by serology and viral culture. The outbreak of upper respiratory illnesses occurred in the ward during the 17 days from July 21 through August 6, 1996. Fifteen of the 18 elderly persons with upper respiratory illnesses were tested by serology; parainfluenza 3 infection was identified in 7. One of the 7 patients, parainfluenza 3 virus was isolated. Seven elderly subjects with parainfluenza 3 infection were 2 males and 5 females and five of them (71.4%) were bedridden. The most common complaint was fever and coughing in 7/7 (100%), followed by sputum in 5/7 (71.4%), wheezing in 4/7 (42.9%). The pyrexial period in the parainfluenza-infected group ranged from 1 to 4 days (average 3.1 days), and was significantly shorter than that of the influenza group. The maximum recorded temperature in the parainfluenza-infected group ranged from 37.0 to 39.2 degrees C (average 38.1 degrees C), and was significantly lower than that of the influenza group. Two of the 7 patients with parainfluenza 3 virus infection had pneumonia, but nobody died, and all 7 patients recovered without sequele. It is possible that parainfluenza 3 virus infection among elderly subjects cause secondary bacterial infection, so we think that prevention of nosocomial parainfluenza infection should be a high priority in the case of outbreak of such an infection in a ward.

Screening of blood donors for human parvovirus B19 and characterization of the results.

BACKGROUND AND OBJECTIVES: Human parvovirus B19 (B19 virus) can be transmitted through blood transfusion and plasma-derived products. In a previous report, we utilized the simple hemagglutination method based on the interaction between the B19 virus and P antigen on human erythrocytes in order to screen the blood donors. We called this method receptor-mediated hemagglutination (RHA) [Lancet 1995;346:1237-1238]. In this paper, we report on a large-scale screening of the B19 virus by RHA and discuss the results. MATERIALS AND METHODS: Donor sera from September 1995 to March 1997 and seroconversion panels were enrolled. Donor sera were examined by RHA for large-scale screening. The positive sera in the first screening were then further investigated by the RHA inhibition test, countercurrent immunoelectrophoresis (CIE), an enzyme-linked immunosorbent assay, and polymerase chain reaction (PCR). We also evaluated the infectivity and neutralizing activity of various kinds of sera by the erythroid colony forming unit (CFU-e) assay. To examine the detection limits of the B19 virus by RHA, B19-viremic sera were purified by sucrose gradient ultracentrifugation. RESULTS: Among 257,710 sera specimens, 293 sera (0.11%) gave a positive reaction in the first screening using RHA. Out of these 293 sera specimens, 31 were positive for PCR, of which 28 were also RHA inhibition-positive, and 25 of the 28 CIE-positive. In the CFU-e injury assay, all the RHA inhibition (+) sera showed a decrease in the number of erythroid colonies. The RHA inhibition (-) PCR (+) B19 antibody (+) sera did not affect the erythroid colony formation and protected CFU-e from injury by the B19 virus. By measuring the amount of purified B19 protein and its RHA titer, the detection limit of the B19 virus by RHA was calculated to the 0.37+/-0.03 ng/ml. CONCLUSION: These results suggest that the RHA(+) RHA inhibition (+) sera were infectious in vitro. The combination of RHA and the RHA inhibition test is considered to be useful for the large-scale screening of infectious B19 virus in blood donors with high specificity.

Sex differences in susceptibility of ICR mice to oral infection with Corynebacterium kutscheri.

Sex difference in susceptibility to oral infection with Corynebacterium (C.) kutscheri was experimentally studied in ICR mice. Immature (4-week-old) and adult (14-week-old) mice were inoculated with two infecting doses of C. kutscheri, and necropsied for bacteriological and serological survey 4 weeks after the bacterial infection. No macroscopic lesions at necropsy were demonstrated, except for one adult male given 10(9) bacteria. In immature mice, C. Kutscheri isolated from the oral cavity and cecum with FNC agar, were recovered in only 40.0% of female mice but in 90.0% of male mice given 10(6) bacteria (p < 0.05), and in only 55.6% of female mice but in 80.0% male mice given 10(8) bacteria. In adult mice given 10(9) bacteria, the organism were recovered in only 45.5% of female mice but in 90.9% of male mice (p < 0.05), furthermore, the mean number of organisms in the cecum of male mice harboring the organism was significantly higher than that in females (p < 0.01). Castration caused an increase in host resistance in adult male mice. These results indicated that ICR male mice were more susceptible than females, in terms of bacterial colonization in the cecum and the oral cavity, to oral infection with C. kutscheri.

Morphometric comparison of the human optic nerve fiber with various other human nerve fibers.

The morphometric characteristics of nerve fibers of the human optic nerve in the chiasmatic region were measured with the combination of an image analyzer and a computer, using the Luxol fast blue-periodic acid-Schiff-hematoxylin discriminative staining method. The mean axonal transverse area of the human optic nerve fibers was 0.644 +/- 0.361 micron 2. Comparison of the size of the axon of the human optic nerve fiber with that of various other human nerves showed optic nerve fibers were definitely thinner than the other nerve fibers, and were surrounded by a thinner myelin sheath. Optic nerves may be more liable to mechanical damage at surgery that previously believed.

Myelinated nerve fiber analysis of the human greater splanchnic nerve.

The purpose of the present study is to analyze the human greater splanchnic nerve in relation to aging. We adopted a new staining method which makes it possible to discriminate various structures of the nervous tissue. We examined 25 human greater splanchnic nerves from cadavers for anatomy dissection. We measured the number, area and perimeter of axons. The results reveal that: (1) there is no correlation between age and the number of axons; (2) the mean area and perimeter of axons increase with age, but not the total area and perimeter. We compared these results with those for the lesser splanchnic nerve. These morphological changes in the greater splanchnic nerve may indicate a kind of compensation through axon hypertrophy for hypofunctions in abdominal organ control.

Morphometric nerve fiber analysis and aging process of the human vestibular nerve.

Myelinated and unmyelinated nerve fibers of the human vestibular nerve were analyzed with the use of a new staining method that makes it possible to discriminate various structures of the nervous tissue. An image-analyzing digitizer, a microscope with a drawing tube and a personal computer for storing data and performing statistical analyses were employed in this study. We measured the axonal numbers and transverse areas of myelinated vestibular nerve fibers in 20 cadavers, and of unmyelinated fibers in 14 cadavers. The average number of myelinated axons and unmyelinated axons were 20,318 and 2,782, and the average transverse area of their axons were 3.46 and 0.49 micron 2, respectively. The transverse areas of myelinated axons decreased with age, although the numbers of their axons did not change. However, in the case of unmyelinated axons, both the transverse areas and the numbers did not change.

Sexual dimorphism of the human spinal cord in the aging process.

There have been few morphometric studies on age-related changes in the human spinal cord. The purpose of the present study was to determine the existence of sexual dimorphism of the spinal cord between males and females during the aging process. Spinal cords were removed from cadaver specimens, 26 males and 22 females for anatomic practice, the age at death ranged from 41 to 97 years for males (average, 71.5 years) and from 59 to 92 years for females (average, 76.6 years). Spinal cord segments were embedded in celloidin after secondary fixation and dehydration. Sections were stained with the Luxol fast blue-periodic acid-Schiff-hematoxylin and Klüver-Barrera methods. Morphometric analysis was performed with an electronic optical planimeter and a computer. Each section was enlarged 13.5 times to take a picture. The areas of the transverse section white matter and gray matter of the spinal cord at segments C5 and L3 were measured. Although there was no correlation between the total transverse area of the spinal cord and age either in males or females, we noticed that the area of the gray matter decreased faster in males than in females; while the area of the white matter decreased faster in females than in males. The area ratio of the white matter to the whole segment area of the spinal cord (W/T) at level C5 is larger in males than that in females. Our results suggest that there could be a difference between males and females in changes in the white and gray matters of the spinal cord due to aging.

Radiotherapy for large hepatocellular carcinoma combined with transcatheter arterial embolization and percutaneous ethanol injection therapy.

This study was performed to determine the effect of radiotherapy (RT) combined with transcatheter arterial embolization (TAE) and percutaneous ethanol injection (PEI) on large HCC. Between 1988 and 1996, 102 patients with unresectable, biopsy proven HCC underwent uniform pretreatment assessment followed by TAE and PEI. Of the 102 patients, 68 (67%) had more than 2 lesions in the liver, and the largest tumor sizes in each patient ranged from 3 to 8 cm in diameter. Immediately after TAE and PEI, external beam RT (36 to 70 Gy) was administered to the largest tumors only in 44 patients. The cause-specific 5-year survival rate for all patients was 39.9%. The 3-year survival rate of the RT group was better than that of the no RT group (81.1% vs. 54.6%). The cumulative local control rates of the largest treated tumors were 53.2% in the RT group and 32.7% in the no RT, respectively (p=0.006). When the survival rate was compared between patients with and without local control in the RT group, that of patients with local control was significantly better than that with local recurrences (p=0.048). No deaths or major treatment related complications occurred. RT combined with TAE and PEI did not clearly show improvement of the survival, however, it effectively controlled large HCC, and demonstrated minimal toxicity. This treatment may represent therapeutic option for some patients with unresectable large HCC.

[Morphometric development and the variability of neurons of the human auditory system: ventral cochlear nucleus and superior medial olivary nucleus].

The development of the human ventral cochlear and medial superior olivary nuclei was studied. We made serial sections of the brain in 10 fetuses at 16-40 weeks of gestation (WG), an infant at 2 months of age and an adult of 63 years using an electronic planimeter with a computer. Although the shape of neurons of the two nuclei was different, our morphometric analysis showed that the development of both of them accelerates between 18 and 21 WG in terms of the columnar length and volume, neuronal size and circularity ratio and the amount of Nissl bodies increases gradually.

High-monounsaturated fat diet-induced obesity and diabetes in C57BL/6J mice.

A high-monounsaturated fat diet has been proposed as a palatable alternative to a high-carbohydrate diet in diabetic patients, but it is unknown whether a higher intake of monounsaturated fat induces obesity and diabetes, as usually observed with other types of fat. To answer this question, C57BL/6J mice were divided into three groups: the first group was given a high-carbohydrate diet, and the other two groups were given a high-monounsaturated fat diet (60% of total energy) as olive oil or synthetic triolein for 4 months. It has been previously reported that the C57BL/6J mouse has a genetic predisposition for intraabdominal obesity and non-insulin-dependent diabetes mellitus (NIDDM) by high-polyunsaturated fat (n-6) feeding. Although there were no significant differences in energy intake and fat absorption among these three groups, compared with the high-carbohydrate diet, both high-monounsaturated fat diets produced hyperglycemia, obesity, and triglyceride accumulation in the liver and skeletal muscle. These data indicate that the recently recommended high-monounsaturated fat diet might induce obesity and diabetes.

Systemic and mucosal immune responses of mice to aluminium-adsorbed or aluminium-non-adsorbed tetanus toxoid administered intranasally with recombinant cholera toxin B subunit.

For the purpose of changing the immunization procedure of tetanus toxoid from intramuscular or subcutaneous injection, which has been in practice for a long time, to intranasal administration, we examined systemic and mucosal immune responses of mice to aluminium-adsorbed tetanus toxoid (aTT) and aluminium-non-adsorbed tetanus toxoid (nTT) inoculated intranasally with recombinant cholera toxin B subunit (rCTB). Intranasal immunization with aTT induced, at a concentration of 0.5 Lf, high levels of TT-specific serum IgG antibody titres and moderate levels of TT-specific serum IgA antibody titres in the presence and absence of rCTB. Induction of high or moderate levels of mucosal TT-specific IgA antibody responses was observed with and without rCTB in the lung, the nasal cavity, the small and large intestines and the vagina. Generally speaking, the co-administration of aTT and rCTB showed higher mucosal TT-specific IgA antibody titres when compared with the administration of aTT alone. In case of intranasal administration of nTT, the dose of 5 Lf was necessary and stimulated, only in the presence of rCTB (10 micrograms), high levels of tetanus toxoid (TT)-specific serum IgG antibody responses in all mice examined and moderate or slight levels of TT-specific IgA antibody responses in the nasal, pulmonary and small and large intestinal lavages of a few mice. All mice intranasally immunized with aTT alone or nTT and rCTB escaped onset of tetanus. This is the first report concerned with the mucosal adjuvant activity of an aluminium compound. Judging from these results, intranasal administration of aTT with and without rCTB or nTT with rCTB appears to be a very useful means for a vaccination against tetanus with respect to ease, safety, certainty, low cost and no need for an injection needle.

An erratic parasitism found in the lungs of sheep during experimental infection with Fasciola gigantica (Japanese strain).

An erratic parasitism was observed in the lungs of sheep experimentally infected with Fasciola gigantica at autopsy 14 weeks after inoculation. Macroscopically, several hyperaemia of 7.5-12.5 mm in diameter were found on the surface of the lungs. Juvenile flukes detected in the lungs were much smaller than those in the liver of the same sheep. A slight inflammatory reaction was observed in the lungs and it is likely that the flukes had migrated in the pleural cavity for some time. Occasionally, a mixed thrombus with many eosinophils was found in the blood vessels adjacent to the bronchia, although no cough was observed clinically. This suggests that diagnosis of juvenile fluke is difficult not only by parasitological but also serological methods.

Intervening sequences in 16S rRNA genes of Campylobacter sp.: diversity of nucleotide sequences and uniformity of location.

We found and sequenced intervening sequences (IVSs) in the PCR-amplicons of 16S rRNA genes of 3 strains of Campylobacter rectus, 2 strains of C. curvus and 2 strains of C. sputorum. The lengths of the IVSs were 140 to 233 bp. The IVSs of C. rectus were identical and had a sequence homology of 55 to 79% against those of C. curvus and C. helveticus. The IVSs of C. sputorum were 97.9-100% homologous but poorly homologous to the other IVSs. In spite of the diversities of the lengths and the nucleotide sequences, all of the IVSs were located at the same position in the 16S rRNA genes.

Detection of dextranase-producing gram-negative oral bacteria.

Thirty-one strains of 23 gram-negative oral bacterial species were examined for dextran-degrading activity on agar plates containing blue dextran. One strain each of Capnocytophaga ochracea, Capnocytophaga sputigena, Prevotella loescheii, Prevotella melaninogenica and Prevotella oralis had detectable dextranase activity. The culture supernatants of P. melaninogenica and P. oralis cells contained dextranases of multiple sizes, but those of the other three species had a single size of enzyme. A 56-kDa dextranase was purified from the culture supernatant of P. oralis and the antiserum against the enzyme was prepared with a rabbit. The Ouchterlony test showed that the antibody reacted with the supernatants of both P. melaninogenica and P. oralis but not with the others. Dot-blot hybridization using the dextranase gene of Streptococcus mutans as a probe revealed that there was no significantly homologous sequence in the chromosomal DNA of the five species.
