Congenital aneurysm of the left coronary sinus and left main coronary artery with fistulous communication to the right atrium in pregnancy.

We describe a unique case of a left coronary arteriovenous fistula arising from a left sinus of Valsalva aneurysm in a pregnant woman. The relevant diagnostic contributions of two-dimensional echocardiography, color flow Doppler, magnetic resonance imaging, and angiography are discussed. The hemodynamic manifestations of this anomaly in pregnancy and the eventual surgical correction are reviewed.

Dry-grind process for fuel ethanol by continuous fermentation and stripping.

Conversion of a high-solids saccharified corn mash to ethanol by continuous fermentation and stripping was successfully demonstrated in a pilot plant consuming 25 kg of corn per day. A mathematical model based on previous pilot plant results accurately predicts the specific growth rate obtained from these latest results. This model was incorporated into a simulation of a complete dry-grind corn-to-ethanol plant, and the cost of ethanol production was compared with that of a conventional process. The results indicate a savings of $0.03 per gallon of ethanol produced by the stripping process. The savings with stripping result from the capacity to ferment a more concentrated corn mash so there is less water to remove downstream.

Continuous fermentation and stripping of ethanol.

Recycling the contents of a continuous fermentor through a stripping column is proposed as a means of reducing product inhibition and lowering the cost of fuel ethanol production. A 2-L fermentor and 10-cm packed column were continuously operated for 150 days without contamination. Some fouling of the packing with attached yeast cells was observed which partially blocked the column. Cell yield was lower than in a simple continuous fermentor. Complete conversion of 200 g/L glucose feed and 90% conversion of 600 g/L glucose feed were achieved. Data were analyzed by computerized process simulation. Cost analysis indicated that, with heat recovery to reduce heating and cooling costs, the continuous fermentor/stripper is possibly a lower-cost alternative to conventional fermentation and distillation.

Computed tomography of mediastinal hematoma secondary to unusual esophageal laceration: a Boerhaave variant.

The Boerhaave syndrome is classically considered a panmural perforation of the distal esophagus secondary to forced vomiting or retching. A variant of the syndrome is described in which only the outer longitudinal muscle layer of the esophagus was torn, and in this case most of the usual radiological findings were absent. The clinical, radiological, and surgical findings are discussed.

The benefits of exercise in geriatric women.

Women 65 years of age and older are the fastest growing segment of the population. As women age, physical inactivity is the leading cause of morbidity and mortality due to coronary heart disease, diabetes mellitus, and hypertension. There is also a loss of both physiologic and psychological capabilities that contributes to an overall reduction in function and independent living. Regular physical activity in geriatric women has been shown to reduce cardiovascular morbidity and all-cause mortality. Further research needs to be done to evaluate the role of regular physical activity in the management of diabetes and hyperlipidemia.

Update on exercise cardiology.

This review focuses on innovative directions and recent studies in cardiovascular exercise science. Studies that address the enhancement of the diagnostic and prognostic value of exercise testing, by virtue of electrocardiographic analysis and the integration of exercise tolerance and clinical findings, are reviewed. The roles of exercise electrocardiographic testing in chest pain centers and the epidemiologic and physiologic effects of exercise training are discussed. The impact of exercise training on the angiographic and clinical regression of coronary artery disease and gender differences in exercise testing and training are among the concluding topics.

Various modalities of local administration of bacterial immunostimulants in transplantable rat tumours and in primitive methylcholanthrene mouse tumours.

The frequent use of intra-lesional injection of bacterial immunostimulants is hampered by apparent rarity of susceptible tumours, absence of therapeutic effect on large tumours, lack of variety of experimental models, eventual traumatism which is feared in case of intra-lesional, and injection in visceral cancers. (1) Methylcholanthrene induced primitive tumours in mice are more frequently susceptible when the carcinogen induction dosage is low (0.01 mg). (2) Using transplantable rat tumours, one susceptible and one resistant to intra-tumoral BCG or Corynebacterium parvum therapy, we have shown that both are resistant to systemic administration of immunostimulants. For the susceptible tumour, subcutaneous peritumoral multiple injections have the same efficacy as intra-tumoral injection in curing small tumours and ipsilateral distant tumours, when the rats receive a double graft of the same tumour. Superficial multifocal intratumoral injections can cure more voluminous susceptible tumours. The association of peritumoral and intra-tumoral injections rendered susceptible the usually resistant tumours.
