[Efficacy and tolerability of flunoxaprofen in the treatment of rheumatoid arthritis. A cross-over clinical study using naproxen].

The efficacy and safety of the new non-steroidal antiinflammatory drug flunoxaprofen were compared with those of naproxen in a cross over clinical study in patients with classical or definite rheumatoid arthritis (RA). Twenty female out-patients in the active phase of the disease were randomly assigned to one of the two groups studied; one group (A: 10 patients, mean age 51 years) received flunoxaprofen 400 mg/day p.o. for 30 days, followed by a 7-day wash-out period before starting the second treatment, with naproxen 500 mg/day p.o. for 30 days. Another group (B: 10 patients, mean age 58 years) received naproxen before flunoxaprofen and followed the same schedule of group A. The results showed that flunoxaprofen and naproxen have essentially equivalent therapeutical effects in controlling painful and functional symptoms of RA: both treatments resulted in a significant relief of spontaneous diurnal and nocturnal pain, pain on active and passive motion, morning stiffness, and in a significant improvement of grip strength and Richtie's index. None of the two drugs modified biochemical parameters of inflammation (ESR, CPR) or the laboratory variables measured to assess the tolerability of flunoxaprofen (hepatorenal function tests; haematological parameters). Flunoxaprofen was found to be very well tolerated: this feature together with the good therapeutic efficacy makes flunoxaprofen a very safe and useful tool in the management of severe chronic disease such as RA.

[Efficacy of sucralfate in the prevention of stomach diseases induced by non-steroidal anti-inflammatory drugs].

In 20 patients suffering from osteoarthritis and treated with non steroidal anti-inflammatory drugs (NSAIDs) for one month sucralfate has been added for further two months. Endoscopic examination before/after this treatment confirmed the cytoprotective property of sucralfate, with normalization of the gastric mucosa and subsidence of subjective symptomatology.

[Evaluation of the clinico-therapeutic response to oral gold salts in patients with rheumatoid arthritis].

The authors investigated the possible correlation between blood concentrations and clinical outcome in 38 auranofin-treated patients with rheumatoid arthritis, diagnosed according to ARA criteria. The results showed that the whole blood gold concentrations did not correlate with the clinical parameters investigated and some laboratory findings. The authors discuss this lack of correlation, suggesting that a clear genetic disposition can interfere with the clinical response, affecting the metabolism and kinetics of the drug.

The efficacy and safety of auranofin in the treatment of juvenile rheumatoid arthritis. A long-term open study.

The safety and efficacy of auranofin in the long-term treatment of children with juvenile rheumatoid arthritis was investigated in an open study of 14 patients. Twelve patients completed at least 12 months of treatment, and 7 patients completed 36 months of treatment. Classic parameters of disease activity showed improvement over baseline values after 6 months of treatment, and laboratory indices remained stable or improved throughout the study. Auranofin was well tolerated; the frequency of adverse effects was lower in these patients than has been previously reported in either adults or children whose arthritis has been treated with injectable gold.

Evaluation of the renal injury from gold salts and nonsteroidal anti-inflammatory drugs in patients with rheumatoid arthritis.

The urinary excretion of proteins, N-acetyl-3-glucosaminidase and leucine aminopeptidase, was measured in 50 patients with rheumatoid arthritis (RA), of whom 20 subjects were on nonsteroidal anti-inflammatory drugs (NSAIDs) and 30 on gold salts. No pathological changes of the above-mentioned urinary parameters have been observed in the group of RA patients, in comparison with control subjects and patients suffering from osteoarthritis treated with NSAIDs.
