The role of anti-pig antibody in pig-to-baboon cardiac xenotransplant rejection.

The role of naturally produced antibody in discordant xenograft rejection is still uncertain. Twelve orthotopic pig-to-baboon heart transplants (HTx) were performed. In 2 baboons, no antibody adsorption (AbA) was performed. In 5 baboons, AbA with a pig lung was performed during circulatory arrest. In 5 baboons, AbA and blood exsanguination at the beginning of cardiopulmonary bypass (CPB) were performed. Baboons were divided into 2 groups; group 1 (n = 4) died within 24 hr of HTx and group 2 (n = 8) survived more than 24 hr. Mean survival period was 9.8 +/- 3.0 hr in group 1 and 151 +/- 33 hr in group 2. Baboon anti-pig antibody (Ab) was measured before CPB, before circulatory arrest, during AbA, at the end of CPB, and daily after HTx. Anti-RBC Ab was measured by the titration method at temperatures of 4 degrees C and 37 degrees C (RAb-4 and RAb-37). Anti-endothelial cell Ab (EAb) and anti-white blood cell Ab (WAb) titers were measured with ELISA. RAb titration > or = 1/4 and EAB and WAb > or = 1/256 were determined to be seropositive (S(+)). S(+) rate of RAb-37 at the end of CPB (endCPB) in group 2 was significantly higher than that in group 1 (8/8 vs. 1/4; P < 0.05). The seronegative (S(-)) rates of RBC-4 and EAb (endCPB) in group 2 were higher than those in group 1 (7/8 vs. 1/4 and 6/8 vs. 1/4, respectively), but not significantly. There was no difference in S(-) rate of WAb (endCPB) between group 1 and group 2. More than 4-fold decrease in RAb-4 and RAb-37 by AbA with a pig lung was observed in 5 and 7 of 8 baboons, while EAb and WAb did not change by AbA. In all of group 2, RAb-4 reverted to S(+) within 3 days after HTx. One baboon had no rejection episode and died of infection 16 days after HTx (baboon 16); however, it also became S(+) for RAb-4 a day after HTx until death. In 4 of group 2, RAb-37 became S(+) 1 or 2 days before death by rejection. Baboon 16, however, became S(+) for RAb-37 7 days after HTx and S(-) again 9 days after HTx until death. EAb became S(+) in all of group 2, but 5 of them survived more than 5 days after seroconversion.(ABSTRACT TRUNCATED AT 400 WORDS)

Budd-Chiari syndrome and Epstein-Barr virus (EBV) associated plasmacytoma in a patient with chronic active EBV infection.

A 42-year-old Japanese man with chronic active Epstein-Barr virus (EBV) infection initially responded to treatment with interleukin-2 (IL-2). Six months later he developed thrombosis in the hepatic veins, and Budd-Chiari syndrome associated with severe hepatic damage was diagnosed. He also developed a solitary EBV-positive plasmacytoma in the right femur. Since these rare complications occurred after long-term IL-2 therapy, the possibility that long-term IL-2 therapy might cause Budd-Chiari syndrome and liver damage as well as EBV-associated plasmacytoma is discussed.

Changes in left ventricular performance after global ischemia: assessing LV pressure-volume relationship.

To determine the effect of ischemia and reperfusion on left ventricular systolic function, we studied the ischemia-induced rightward shift of the ventricular pressure-volume relationship. Eight mongrel dogs were intubated, and their hearts were exposed through a thoracotomy. A conductance catheter and micromanometer were used to obtain instantaneous left ventricular pressure-volume data. The dogs were subjected to 20 minutes of normothermic global myocardial ischemia, followed by 80 minutes of reperfusion under total cardiopulmonary bypass. Data were acquired during transient (10- to 12-second) periods of acute volume loading before ischemia and at 20-minute intervals during reperfusion as bypass was continued. The relationship between stroke work and end-diastolic volume (ie, preload recruitable stroke work [PRSW]) and the end-systolic pressure-volume relationship were highly linear throughout the study (mean r = 0.954 to 0.983 for PRSW; mean r = 0.954 to 0.984 for end-systolic pressure-volume relationship). Ischemia produced changes in the PRSW: (1) the slope decreased significantly at 20 minutes and 40 minutes of reperfusion then returned to preischemic levels at 60 minutes and 80 minutes, and (2) the x-intercept increased significantly up to 60 minutes. The preload recruitable work area (the area under the linear regression line of PRSW) reflected changes in both slope and x-intercept of PRSW and was significantly decreased throughout the 80 minutes of reperfusion despite gradual recovery. The slope and the x-intercept of the end-systolic pressure-volume relationship did not change after ischemia.(ABSTRACT TRUNCATED AT 250 WORDS)

Study of efficacies of leukocyte-depleted terminal blood cardioplegia in 24-hour preserved hearts.

To evaluate the effect of leukocyte-depleted terminal blood cardioplegia on prolonged preservation, 41 canine hearts were stored in modified Collins' solution and transplanted heterotopically. Hearts were transplanted soon after harvesting in group 1 and after 24-hour preservation in groups 2, 3, and 4. Blood cardioplegia was applied just before aortic unclamping in groups 3 and 4; group 3 received simple blood cardioplegia and group 4 received leukocyte-depleted cardioplegia. The percentage of the preload recruitable stroke work and diastolic compliance after transplantation compared with the preharvesting value in group 4 did not differ from those in group 1, but the percentage of the preload recruitable stroke work in groups 2 and 3 was significantly lower than that in groups 1 and 4. The percentage of diastolic compliance in groups 2 and 3 was significantly higher than that in groups 1 and 4. Coronary blood flow 40 minutes after aortic unclamping in group 4 did not differ from that in group 1, but was significantly higher than the blood flows in groups 2 and 3. Significant production of malondialdehyde was detected during terminal blood cardioplegia and 10 minutes after aortic unclamping in groups 2 and 3, but never in groups 1 and 4. After leukocyte-depleted terminal cardioplegia, the myocardial adenosine triphosphate content increased to the preharvesting value in group 4. Our results suggest that leukocyte-depleted terminal blood cardioplegia may be effective in replenishing the energy-depleted myocardium and reducing reperfusion injury, resulting in adequate cardiac function.

Monoclonal antibodies recognizing 2-oxo acid dehydrogenase components in granular structures in neurons.

Monoclonal antibodies (MAbs) were raised against the hippocampal homogenate of young rats and classified into three types by immunohistochemical analysis: (1) MAbs specific for a granular structure observed within neurons, (2) MAbs specific for neuronal cell surface and cell body, and (3) MAbs specific for both neurons and astroglial cells. One MAb (2D11-7) specifically reacted with granular structures observed in neurons. A specific protein antigen was purified from rat homogenate by immunoadsorbent assay with MAb 2D11-7. Amino acid sequencing followed by lysyl endopeptidase digestion of the proteins in the eluate demonstrated that the antigens recognized by MAb 2D11-7 were E2 components of the 2-oxoglutarate dehydrogenase complex and pyruvate dehydrogenase complex. The cell specificity and age dependency of these proteins are also discussed.

An autopsy case of extraskeletal Ewing's sarcoma followed up for 24 years.

A long-term (24 years) follow-up case of extraskeletal Ewing's sarcoma is reported. The light microscopic examination showed features hardly indistinguishable from Ewing's sarcoma of the bone, that is, the tumor cells were diffusely arranged and uniform in size and shape, and possessed glycogen in the cytoplasm. Homer-Wright rosettes were found only in the autopsy material. An immunohistochemical study using a neural marker (neuron-specific enolase) demonstrated positive staining in most tumor cells. An ultrastructural study revealed intracytoplasmic glycogen particles and incomplete neural characters as follows: the cytoplasmic processes resembled neural processes without neurosecretory granules, microtubules or neurofilaments. These findings suggest that this case finally acquired an incomplete neural character with repeated recurrence. This tumor was diagnosed extraskeletal Ewing's sarcoma, but in future it may be categorized as primitive neuroectodermal tumor (PNET).

A case of Fanconi syndrome whose origin was suspected to be cystinosis.

We report a case of 6 year old girl with Fanconi syndrome the origin of which was suspected to be cystinosis. Pathological findings of a renal biopsy showed needle-like materials in the epithelial cells but the cystine content of the white blood cells was normal. Although excessive urinary loss of growth hormone was detected, the height and weight of the patient was normal. Urinary loss of growth hormone did not cause growth retardation in this case.

Fibronectin synthesis bioactivity in human breast milk.

To evaluate the effect of milk feeding on fibronectin in the perinatal period, fibronectin synthesis bioactivity (FnSA) in human breast milk was examined. FnSA was measured by bioassay using the cell-lined fibroblast. For the characterization of the FnSA, the milk samples were chromatographed. In gel chromatography (Sephadex G-100), three fractions of FnSA were detected. Two fractions overlapped with those of immunoreactive epidermal growth factor (ir-EGF). The other was present in the low-molecular-weight portion. In the high-performance liquid chromatography, two of four fractions with FnSA presented ir-EGF. One of two fractions with FnSA in heparin-binding affinity chromatography almost coincided with the portions of basic brain fibroblast growth factor (ir-bFGF). From these findings, we speculated that human milk has the bioactivity of fibronectin synthesis and it is composed of EGF, FGF, and unknown low-molecular-weight factors. Although the clinical meanings of FnSA have not been clarified, our data indicate a new characteristic of human milk.

A human T-cell lymphotropic virus type I carrier with temporal arteritis terminating in acute myelogenous leukemia.

A human T-cell lymphotropic virus type I (HTLV-I) carrier with temporal arteritis (TA) in whom acute myelogenous leukemia (AML) developed 1 year after successful treatment of the autoimmune disease is described. This case suggested that the induction of immunodeficiency by infection with HTLV-I may be related to the development of the autoimmune disease and malignancy.

Risk factors for graft failure associated with pulmonary hypertension after pediatric heart transplantation.

Postoperative pulmonary hypertension can be a major cause of early death after heart transplantation in children. To identify predictive risk factors of pulmonary hypertension after heart transplantation, we performed a retrospective analysis of our 194 infant and pediatric recipients who underwent heart transplantation between 1987 and 1992. Because the response of pulmonary vasculature may change during growth, the patients were divided into two groups: age less than 1 year in group I (n = 152) and 1 year or older in group C (n = 43). The following risk factors were evaluated: cardiomyopathy, congenital heart disease and hypoplastic left heart syndrome, pretransplant pulmonary hypertension, history of operation, oversized donor (donor/recipient weight ratio greater than 2), donor's history of cardiopulmonary resuscitation, and prolonged graft ischemic time (graft ischemic time 360 minutes or longer). Though there was no significant difference between group I and group C in overall early mortality including early graft loss (19 of 152 versus 5 of 42), the mortality rate from pulmonary hypertension in group I was significantly lower than that in group C (2 of 152 versus 4 of 42; p < 0.05). The mortality rate from pulmonary hypertension in patients with congenital heart disease in group I was significantly lower than that in group C (0 of 44 versus 4 of 24; p < 0.05). In group I, there was no significant difference in the early mortality rate or the mortality rate from pulmonary hypertension from any factors studied. The mortality rate from pulmonary hypertension in association with prolonged graft ischemic time in group C was significantly higher than when no prolonged graft ischemic time was present in group C and with either prolonged graft ischemic time or no prolonged graft ischemic time in group I (4 of 16 versus 0 of 26, 0 of 37, and 2 of 115). In conclusion, older patients had a higher mortality rate from pulmonary hypertension after heart transplantation, especially in patients with congenital heart disease who received a graft preserved more than 6 hours. This study demonstrates another benefit of early heart transplantation in infancy, that is, prevention of death from pulmonary hypertension.

Donor stromal cells support hematopoiesis in CFU-S colonies.

To examine the effect of hematopoietic stromal cells on bone marrow transplantation (BMT), CF-1 cells, which were previously established from splenic stroma of a mouse that was administered recombinant human granulocyte colony-stimulating factor (rhG-CSF), were transplanted with bone marrow cells. The stromal cells were transfected with the plasmid DNA of beta-galactosidase (beta G-CF-1) cells for studying immunohistochemistry. Using immunohistochemistry, the spleen and bone marrow of the recipient mice were examined for beta-galactosidase on the 8th and 12th day after BMT. In the spleen, beta G-CF-1 cells were observed within and in the regions immediately surrounding the colony-forming unit-spleen (CFU-S) on the 8th and 12th day after BMT. In certain regions, beta G-CF-1 cells existed in or surrounded the CFU-S colonies that consisted of the undifferentiated hematopoietic cells, while in other regions the granulopoiesis increased, especially in correspondence with the existence of beta G-CF-1 cells. In the bone marrow cavity, beta G-CF-1 cells were also recognized, though hematopoiesis was lower than the normal bone marrow. These data demonstrate that CF-1 cells, that is, donor stromal cells, can be transplanted to the hematopoietic organs and can then support hematopoiesis.

[Aortic valve replacement in a kidney transplant recipient].

A 62-year-old man with chronic renal failure, who had been treated with hemodialysis, received a kidney transplant from his son in December 1989. He kept good renal function without any episode of graft rejection under the immunosuppressive therapy with Cyclosporine A (CyA), Predonine (PRD), and Mizoribine. In 1990 he started to have exertional dyspnea, and was diagnosed to have aortic valve regurgitation. He underwent aortic valve replacement in September, 1991. He was treated with continuous infusion of CyA and bolus injection of methylprednisolone from the start of the operation, and returned under the preoperative immunosuppressive therapy after oral intake became possible. On the third post-operative day, the level of serum creatinine elevated. Although we couldn't detect significant changes in subpopulations of peripheral blood lymphocytes, steroid pulse therapy was performed as the possibility of acute rejection was underiable. Serum creatinine returned to normal level in a few days and the postoperative course after that was uneventful. He was discharged on the 30th post-operative day.
