Growth-promoting effect of retinoic acid in transplantable pituitary tumor of rat.

MtT/F84 grew well in Fischer rats (F344), but tumor growth was promoted in hyperestrogenized rats. Effects of dietary retinoic acid (RA) on tumor growth, estrogen receptor (ER) and serum growth hormone (GH) level were examined. Tumor latency became shortened, and tumor take and weight were promoted by all-trans RA at dosages of 50 and 200 mg/kg basal diet, but not dose-dependently. ER level was elevated in tumor of RA-treated rats, whereas the retinoic acid-binding protein level remained unchanged. RA also elevated incorporation of 5-bromo-2'-deoxyuridine, a thymidine analogue, into DNA of tumor cells. Average serum GH level was increased in tumor-bearing rats treated with RA and was well correlated with tumor weight. RA may directly affect ER level and enhance estrogenic action, resulting in promotion of tumor growth, or it may act independently for tumor growth and elevation of serum GH level.

Local ofloxacin delivery using a controlled-release insert (PT-01) in the human periodontal pocket.

PT-01, a controlled-release insert, was developed for topical chemotherapy in periodontal disease. It is a soluble insert that consists of fast-release and sustained-release parts containing ofloxacin (OFLX) as an antibacterial agent. In this study, the release profile of OFLX from PT-01 was investigated in vitro. Twelve adult volunteers were administered OFLX as PT-01 or as an aqueous solution into their periodontal pockets, OFLX concentrations in gingival crevicular fluid (GCF) were evaluated from the viewpoint of pharmacokinetics. The in vitro release profile of OFLX from PT-01 showed a biphasic pattern. The release rate of OFLX was relatively rapid in the early phase and slow thereafter. When OFLX aqueous solution was administered into periodontal pockets, the OFLX level in GCF rapidly decreased to be about 1/100 after 30 minutes. When PT-01 was inserted into the pockets, the OFLX level in GCF immediately reached a peak (about 12 mg/ml), and gradually decreased until the 3rd day, and maintained a constant level above 2 micrograms/ml, the effective minimum antibacterial concentration for periodontopathic microorganisms, from the 3rd to 7th day after insertion. No side-effects were observed in the volunteers who received the PT-01 insert. The above results suggest that PT-01 is a suitable pharmaceutical preparation for periodontal chemotherapy.

Establishment of a series of pituitary clonal cell lines differing in morphology, hormone secretion, and response to estrogen.

Four kinds of cultured clonal cell line were established from estrogen-induced mammotropic pituitary tumor. These clones showed differences of hormone secretion, morphology, and response to estrogen. One clone tentatively designated MtT/E, consisted of spindle-shaped epithelial cells and showed the strongest adhesion to the culture dish, but did not secrete any pituitary hormone. The second cell line, designated MtT/S, secreted only GH, showed very weak contact with the culture dish, and proliferated almost anchorage independently. the MtT/S cells were mainly spherical and formed floating or weakly adherent clusters. Some of them had very long dendrite- or neurite-like cell processes. Electron microscopic examination of the MtT/S cells showed a normal somatotrope-like appearance, i.e. the presence in the cytoplasm of many GH-containing secretory granules, well developed rough endoplasmic reticulum, and Golgi apparatus. Furthermore, these cells secreted GH in response to stimulation with GRF. The third cell line with an ovoid cell appearance tentatively designated MtT/SM, secreted both PRL and GH, and showed anchorage-dependent proliferation. The fourth cell line designated MtT/Se secreted only a small amount of GH. Among these newly established cell lines, MtT/Se was the smallest in size and showed estrogen-dependent proliferation. The many small secretory-like granules present in the cytoplasm of MtT/Se cells showed no immunocytochemically positive reaction for anterior pituitary hormone. MtT/S and MtT/SM cell lines were also sensitive to estrogen.

[The influence of miosis on the central visual field].

We studied the effects of miosis on mean sensitivity in cases of glaucoma and normal subjects. The visual field in 30 degrees was measured by an automated perimeter Octopus. The subjects consisted of 13 cases (24 eyes) with open angle glaucoma and 10 normal subjects (10 eyes). Glaucomas had a 1.8dB reduction of mean sensitivity with a decrease in pupil size from greater than 3mm to less than 2mm with 3% or 4% pilocarpine, regardless of age and mean sensitivity. Normal subjects showed a depression of 1.6-1.8dB with a decrease in pupil size from more than 3mm to less than 2mm with 3% pilocarpine. There were no significant changes of mean sensitivity in pupil size over 3mm in normal. There was no difference in the reduction of mean sensitivity with miosis between cases of glaucoma and normal subjects. The mean sensitivity measured under conditions of pupil size over 3mm before glaucoma surgery could be quantitatively compared with that of the pupil after operation.

[MRI lesions of the optic nerves in optic neuritis].

Magnetic resonance imaging (MRI) was performed in 14 patients with optic neuritis. Three patients suffered from multiple sclerosis but the etiologies of the remaining 11 cases could not be identified. They were bilateral in 6, and unilateral in 8. The MR images were compared with the symptomatic lesions of optic neuritis and pattern reversal VECP. The STIR mode (short time inversion recovery), was employed for the MRI in the orbit and T2-weighted mode in the brain. In 11 eyes with hyperemia of the optic disc, 7 eyes showed a high signal in the optic nerve with the MRI, and 9 eyes showed an abnormal pattern VECP. Seven eyes with normal disc and two eyes with a pale disc showed a high signal in the optic nerve with MRI, those 9 eyes had abnormal pattern VECP. The high signal in the optic nerve was not related to visual acuity or visual field abnormalities of patients. However, the degree of the high signal of the optic nerve lesion in MRI was associated with the clinical course and prognosis of the optic neuritis. The degree of the high signal of the optic nerve lesion decreased with the recovery of visual acuity in optic neuritis.

Necrotizing B-cell lymphoma. A case report with a review of the literature.

We describe a case of massively necrotizing B-cell lymphoma, involving both axillary and cervical lymph nodes. Histologically, no vascular lesions were noted, and the necrosis was manifested by death of the tumor cells themselves. These cells were surrounded by macrophages, simulating the features of tuberculous lymphadenitis. Immunostaining was a useful method of defining the origin not only of viable cells, but also of necrotic cells in the lymphoma.

[A case of adrenal myelolipoma with colonic cancer and gallstones].

A 47-year-old female, referred to Kushiro Rohsai Hospital because of upper abdominal pain on January 10, 1986, was found to have a gallbladder stone on X-ray examination of her abdomen. On admission, the occult blood found in her stool was strongly positive and a barium enema and a colonoscopy showed an elevated lesion in the caecum. Subsequently, a diagnosis of a colonic cancer was made after a biopsy. Sonography and CT scans revealed a gallbladder stone and a right suprarenal mass 4 cm in diameter. At surgery, this gallbladder stone was found, as was a Borrman II type tumor in the caecum. The surface of an adrenal mass was hard and smooth and a cut of this surface showed a yellowish gray color. Microscopic examination revealed adrenal cortical tissue beneath the capsule with a mixture of hematopoietic and adipose tissue. These findings were compatible with myelolipoma. Postoperatively, a bone marrow aspiration was performed, but no definite abnormality was detected.

Morphological changes in acute cerebral ischemia after occlusion and reperfusion in the rat.

Blood-brain barrier permeability was investigated in acute focal ischemia after MCA occlusion and reperfusion. Four kind of tracers were used: sodium fluorescein and ionic lanthanum as a small molecular tracer, and Evans blue and HRP as a macromolecular tracer. BBB permeability to the tracers was observed macroscopically and studied by electron microscopy. The MCA was dissected free and occluded by ligation with 10-0 monofilament nylon suture. In the reperfusion model, the nylon suture was cut and the occluded MCA was reopened. Diffuse fluorescein staining was noted in the ipsilateral cortex; however, there was no extravasation of Evans blue or HRP in the occlusion model. Ionic lanthanum was demonstrated in the interendothelial space, basement membrane, and extracellular space. In the reperfusion model, intense fluorescein staining and hemorrhagic infarction were observed. Remarkable extravasation of Evans blue and HRP was also revealed in the ischemic lesion. HRP was demonstrated in the basement membrane and around the neuropils. Ultrastructural findings suggested that small molecules such as sodium fluorescein and ionic lanthanum may pass through the entire interendothelial cleft into the extracellular space before leakage of the macromolecules in the acute ischemic stage. Between 30 to 60 min after MCA occlusion, cerebral edema may begin with an escape of water and ions through the tight junctions. Reperfusion of MCA in the acute stage of ischemia may lead to abnormal vascular permeability to macromolecules as a manifestation of severe damage to the BBB.

[Variation of sensitivity in the central visual field by the range of measurement].

We compared two programs with different measurement field ranges in terms of sensitivity for common coordinates using an automated perimeter Octopus. The combination of programs consisted of programs 31 and 61, 31 and PSQ, 61 and 11, 61 and PSQ. The center of program 61 was (0, 0). PSQ was made by the sargon program and a program which measured the outer 16 points of the 61. The mean sensitivity of the common coordinates in 11 was significantly better than that in 61, which was better than that in 31, and that in PSQ better than that in 31 and in 61. The sensitivity was better in programs which tested a smaller size of measurement in the central visual field.

[A metastatic Merkel cell carcinoma successfully treated with chemotherapy].

A 68-year-old man referred to the hospital because of a swelling of his bilateral cervical, left supraclavicular, and left axillary lymph nodes, and an ulcerated tumor in the right palatine tonsil. Three years previously, he had undergone a local excision in the left forearm of a skin tumor that had been pathologically diagnosed as a Merkel cell tumor. CT scan demonstrated an abdominal mass and his serum neurospecific-enolase (NSE) was found to be 20.3 ng/ml. Following biopsies of the lymph nodes and the tumor in the right tonsil, a metastatic Merkel cell carcinoma was diagnosed. Systemic chemotherapy thus was begun with doses of cyclophosphamide, doxorubicin and vincristine. After three cycles were completed, tumor were no longer present and his NSE returned to normal. Three months after the discontinuance of chemotherapy, he remains disease-free.

Effects of sex difference, gonadectomy and estradiol on N-ethyl-N-nitrosourea-induced trigeminal nerve tumors in rats.

The occurrence of trigeminal nerve tumors (TNTs) induced by neonatal administration of N-ethyl-N-nitrosourea (ENU) in WF x LE F1 (F1) rats was studied with special reference to sex difference, effect of gonadectomy and estradiol (E2) administration. Experimental groups 1-6 were treated with 40 mg ENU/kg of body weight neonatally. They consisted of male, female, castrated male, ovariectomized female, E2 pellet (0.1 mg, s.c.) supplemented and gonadectomized male and female rats respectively. Rats of groups 7-12 served as the respective controls without ENU. All the rats were killed at 8 months of age. Levels of serum E2 and E2 receptor (ER) of the TNTs were also examined. It was noted that the incidence of TNT was higher in males (79%) than in females (48%, P less than 0.05) and did not change by castration in males (91%) but increased in ovariectomized female rats (74%, P less than 0.05). Administration of E2 followed by gonadectomy inhibited the occurrence of TNTs in male rats (59%) but not in female rats (60%). No TNT was observed in any control groups. Kidney tumors were the second most frequent tumors next to nervous system tumors in the present experiment. The incidence of kidney tumors was much higher in females (38%) than in males (4%, P less than 0.05) and decreased by ovariectomy, whereas it increased in male rats by E2 administration. ER levels of TNTs and trigeminal nerve tissue were less than 1 fmol/mg protein. These results suggest that in rats treated with ENU neonatally, E2 has an inhibitory effect on the induction of TNTs but may not be regulated through ER. E2 also shows a promoting effect on kidney tumorigenesis.

The risk factors for ischemic heart disease in Tibetan highlanders.

The risk factors for ischemic heart disease (IHD) in 35 Tibetan highlanders were investigated and compared with those in 30 age- and sex-matched healthy Japanese controls. Although Tibetans had remarkably high hematocrit values, and a decrease of eicosapentaenoic acid in both serum total lipids and serum phospholipid (PL) possibly due to their diet, they were considered to have a low incidence of IHD from our door-to-door study. These positive risk factors are likely counteracted by other negative risk factors as follows; Tibetans rarely exhibited systolic hypertension, and had lower levels of serum cholesterol and serum apolipoprotein (apo) B, and apo B/apo A-I ratio. In addition, Tibetan highlanders showed a decreased level of palmitic acid and an increased level of linoleic acid in serum PL which may protect against atherosclerosis.

Neonatal brain abscess with intracystic hemorrhage--case report.

The authors describe the case of an 11-day-old boy with high fever and signs of meningeal irritation in whom computed tomography demonstrated a large brain abscess with intracystic hemorrhage in the right temporal lobe. Cerebrospinal fluid analysis indicated purulent meningitis caused by Escherichia coli. After aspiration of the abscess contents, the entire ventricular system gradually enlarged. Despite repeated ventricular drainage and ventriculoperitoneal shunting, the lateral horn of the left lateral ventricle remained dilated. The isolation of the lateral ventricle may have resulted from septation due to the inflammatory reaction. This fluid was also shunted to the peritoneal cavity.

Induction of cataract in methylnitrosourea treated Fischer (F344) rats.

Cataracts were observed in female F344 rats who intravenously received methylnitrosourea (MNU), a potent carcinogen for multiple organs, in one dose of 50 mg/kg body weight. Induction of cataract 40 weeks after MNU treatment was 41% whereas no cataract was observed in control rats. The aggregation of beta H-crystallin fraction by MNU was studied in vitro. HPLC pattern of beta H-crystallin changed when lens protein was incubated for 24 hrs with MNU. HPLC patterns indicated that MNU induced high molecular weight aggregates of beta H-crystallin. This study conveys some indication about the direct interaction of MNU with lens protein in cataract formation.
