Multifocal idiopathic fibrosis presenting as facial pain and trismus.

An unusual case of idiopathic sclerosing fibrosis is described in which there was involvement of the face, neck, mediastinum and lungs with sparing of the thyroid and retroperitoneal tissues. In contrast to previous cases, facial pain and limitation of jaw movements were the presenting complaints and major causes of disability. Prednisolone treatment failed to arrest the disease.

Seasonal variation and time trends in childhood asthma in England and Wales 1975-81.

In England and Wales hospital admissions for childhood asthma almost trebled over the period 1975-81. This may have reflected a true increase in the incidence of acute asthma, a swing from primary to hospital care, or both. The trend was not due to a change in diagnostic fashion. Monthly admissions showed a pronounced seasonal variation with fewest admissions in winter, rising in spring and early summer to peak in the autumn. A deep admission trough was present in August. The monthly admission profile was very similar throughout England and Wales, suggesting that major "trigger" factors were responsible.

Colonic release of 5-amino salicylic acid from an oral preparation in active ulcerative colitis.

5-amino salicylic acid coated with acrylic based resin was given orally to eight patients with active ulcerative colitis. Abdominal X-rays confirmed that the preparation released its contents in the terminal ileum and proximal colon. Differences between individuals were marked; the plasma 'concentration-time profile' reflected variations in the time taken for tablets to reach the terminal ileum.

Authors and publishers: the mutual selection process.

Writing a book is a full-time job--a demanding, frustrating, and lonely occupation. Most nurse authors append this task to another full-time job in teaching or practice and, often, to family responsibilities as well. To fulfill all these commitments and emerge physically and mentally healthy requires a special kind of person--one who is organized, disciplined, and tenacious. Yet having written and been published affords a satisfaction unsurpassed by few other experiences. Evidence of infection by viruses in small British field rodents.

Four populations of small wild British rodents were studied by capture--re-capture methods over a period of three years. Samples of blood were taken from these animals and tested for antibodies to nine viruses. Animals were removed from another 11 sites around the UK, and immunosuppressed. Samples of tissue from these animals were tested for the presence of viruses by passage in laboratory mice and serum samples from some of them were tested for antibody to the nine viruses. Indications were found of the possible influence of epizootic outbreaks of certain diseases on animal populations.

Psychological stress, ego defenses, and cortisol production in children hospitalized for elective surgery.

This study was designed to investigate the relationship between the effectiveness of coping mechanisms and physiological indicators of distress in children faced with the experience of hospitalization and surgery. Twenty-five children between the ages of 7 and 11 were studied in the out-patient department, 2 weeks before surgery, and again during their hospital stay. Effectiveness of defenses and defense style was measured by a clinical interview and by the Rorschach test. Cortisol production rates were measured by the analysis of 24-hour urine collections at home and again in the hospital. Ward adjustment was also rated by a ward questionnaire. The results indicated no relationship between defense effectiveness and cortisol production rates in the out-patient department and an inverse relationship between cortisol production and defense effectiveness under the stress of hospitalization. Defense style was found to correlate with coping under stress. Four different groups of children emerged, suggesting four different types of reaction to the hospital experience.
