[Hepatic abscesses].

The authors analyse 16 cases of hepatic abscesses recorded in a 10-year interval. The difficulties in assessing the etiology and diagnosis, as well as in indicating an effective treatment are discussed. The clinical aspects and investigation methods, among which scintigraphy, arteriography and especially echotomography are extremely useful in making the diagnosis and in localizing the collection, are reviewed. The bacteriological examination revealed the presence of various organisms in 14 patients, the examined specimen of pus being negative in the remainder of 2 cases. The treatment was surgical in most of the cases (15) consisting in an abdominal approach or posterolateral with rib resection (1 case) and drainage. In 3 cases the intervention was extended to cholelithiasis which was the underlying cause of the abscess. The ultrasound-guided transparietohepatic drainage was performed in 2 cases. No death was recorded.

[Problems of therapeutic management of cancer of the superior pole of the stomach].

This cancer is rare (17-30%) and has the worst prognosis. In the interval 1970-1987 to the I-st Surgical Clinic of Iaşi were admitted and treated 648 patients with gastric cancers of which 98 (15.2%) with adenocarcinomas of the cardiac end of the stomach. In most patients the diagnosis was late: stage II and III-40 patients; stage IV-58 patients. Given the extension of the tumoral process, the exeresis was possible but in 36 cases (36.7%), as compared to 26-70% mentioned in the literature. The therapeutical management of these cases and the obtained results are presented.

Adenocarcinoma↗

[Chronic myeloid leukemia and periurethral adenoma].

Dysectatic disturbances in leukemia patients can be determined by leukemic infiltration of the prosthate or by the evolution of a co-existing peri-urethral adenoma. Indications and results of physiotherapy and surgery are evaluated in connection with an observation of peri-urethral adenoma in a patient with chronic myeloid leukemia.

[Mechanical anastomoses in esophagogastric surgery for malignant tumors].

The authors present their experience in employing for the first time the mechanical circular suture with I.L.S. stapler in the surgery of 10 malignant esophagogastric tumors sited in C (4 cases) and M areas (6 cases). The disease was in stage II in 2 patients and in stage III in the remainder of 4. Three upper polar esophagogastrectomies (Akiyama technique) and 7 total gastrectomies were performed in old patients (mean age 63 years) at high surgical risk. Neither death nor fistulas were recorded. The average hospital stay was 14 days. A series of 17 patients (mean age 59 years), at medium surgical risk, with total gastrectomy for gastric carcinoma and classical suture served as controls. Five fistulas, two deaths and a two times longer hospital stay were recorded. Although the high cost of staplers prohibits the routine use of this technique, the authors demonstrate the great advantages resulting from mechanical suture in esophagogastric surgery.

[The treatment of rectal cancer--the optimizing possibilities].

The authors studies 280 rectal cancers treated in the first surgical clinic of Iaşi in the period 1981-1994. Although surgery is the main treatment of these tumors, it cannot be and it must be not used alone. Surgery must be associated with radio- and chemotherapy, because is the only way possible to increase the number of resections for rectal tumors and the survival rate. For the 263 tumors operated the resectability was 73, 5%: Milles's operation 101 (36, 7%), Dixon's operation 81 (30, 79%), other operation 91 (33, 11%). We consider that Dixon's operation is the best choice for the tumors in the stages A and B of Dukes classifications. Preoperative radiotherapy can influence the tumor volume (downstaging) and decrease local recurrences to about 10-15%. Surgery alone must be practiced only in emergencies. Postoperative chemotherapy is mandatory and it reduces local recurrences and delays metastasis evolution.

[Malignant tumors of the digestive tract below the diaphragm in young people].

We have retrospectively studied 1243 malignancies of the infradiaphragmatic tract treated in the First Surgical Clinic between 1978-1994 (678 gastric, 300 rectal and 256 colonic malignancies). Out of these 39 cases have been encountered in young patients (14 gastric, 13 colon and 12 rectal malignancies). The following particularities have been noted in this group: hereditary predisposition, more advanced stage at diagnosis, predominance of non-Hodgkin lymphomas and poorly differentiated forms. Young age does not significantly influence prognostic, with the control group. In young people the most important prognostic factors are tumor stage, cellular grading and tumor DNA index as determined by flow cytometry.
