Vision disturbances after operative hysteroscopy.

Vision disturbance after transurethral resection of the prostate using glycine (1.5% aminoacetic acid) as the irrigating fluid is well known (TURP syndrome). Not so commonly seen or reported is transient blindness after operative hysteroscopy with glycine. It occurred in a 38-year-old woman who underwent hysteroscopic myomectomy and endometrial resection in which the distention medium was glycine. (J Am Assoc Gynecol Laparosc 6(2):213-215, 1999)

Maternal transmission of translocation 2;21 associated with Down's syndrome.

An unusual 2;21 translocation associated with Down's syndrome is reported. The proband was a 3 year old boy, clinically diagnosed as having Down's syndrome and with a family history of Down's syndrome. Maternal age at the time of study was 28 years. Out of the four sibs with Down's syndrome three had died. Two pregnancies ended in first trimester miscarriage. The proband was found to have trisomy 21 associated with a 2;21 translocation inherited from his mother.

Diagnostic and therapeutic hysteroscopy in the management of abnormal uterine bleeding.

Three hundred seventy cases of abnormal uterine bleeding that occurred from January 1985 to June 1989 were studied. Diagnostic and operative hysteroscopy was performed as an outpatient procedure. Included in the study were all patients with dysfunctional uterine bleeding that did not respond to progestogens for three months, patients with severe menometrorrhagia and those with postmenopausal bleeding. Patients with pregnancy-associated bleeding, obvious uterine enlargement, polycystic ovary disease and abnormal cervical smears were excluded. The ages of the patients ranged from 22 to 82 years. The uterine cavity was found to be normal in 33.51%. Polyps were found in 21.53% of cases, submucous myomata in 11.35%, endometrial hyperplasia in 22.97%, endometrial atrophy in 1.62%, synechiae in 5.67% and adenocarcinoma in 1.35%. Targeted biopsies, excision of polyps, removal of myomata (with scissors and resectoscope) and curettage were performed as necessary.

Laparoscopy for resolving Müllerian abnormalities.

One hundred thirty-five patients with various müllerian abnormalities underwent laparoscopy. At a glance the precise malformation was diagnosed correctly: 44 patients revealed a complete absence of the müllerian system; 35 were found to have a transverse ridge across the pelvis, the lateral ends of which were well developed; 33 patients had rudimentary uteri; 7, a median müllerian nodule; 5 belonged to the group with the testicular feminization syndrome; 4 were classified as having a bicornuate uterus; 3 had unicornuate uteri; and 3, septate uteri. One rare variety of müllerian abnormality is also described. Laparoscopy was found to be invaluable in the diagnosis of müllerian abnormalities.

The scope and limitations of laparoscopic ovarian biopsy in the diagnosis of secondary amenorrhea.

Laparoscopically directed ovarian biopsy has been evaluated in 39 cases of secondary amenorrhea. The definite diagnosis was achieved in 29 cases. In 10 cases, ovarian histopathology alone was insufficient, and the estimations of gonadotropins were essential for the final diagnosis. The scope and limitations of this procedure for the differential diagnosis of premature ovarian failure and hypothalamic-pituitary lesions are discussed.

Premature ovarian failure.

Twelve cases of premature ovarian failure (POF) are described. The clinical, hormonal, histopathologic and cytogenetic studies of each patient have been presented. The scope and the limitations of laparoscopic ovarian biopsy are discussed, as is the importance of radioimmunoassay of follicle-stimulating and luteinizing hormones (FSH/LH) in comparison to ovarian biopsy for the diagnosis of premature ovarian failure. A plea is made for the early diagnosis of POF so that these young women can be spared the expensive and time-consuming treatment for the associated problem of infertility.
