[Acute intestinal occlusion caused by pseudo-neoplastic, endometriosic rectosigmoid stenosis].

Bilateral hysteroadnexectomy and rectosigmoid resection were successfully employed in a case of acute intestinal occlusion due to endometriotic rectosigmoid stenosis in a 36-yr-old woman. The endometriotic character of the stenosis was only made clear histopathologically. The clinical features of intestinal endometriosis are examined. Its lack of pathognomonic symptoms makes diagnosis difficult and may result in confusion with cancer, even intraoperatively. Ex tempore histological examination is thus fundamental in clinching diagnosis and hence the extent to which intestinal resection is required.

[Value and limitations of intravenous cholangiography in the diagnosis of the main bile duct diseases. Studies in 209 cases controlled surgically].

The value and limits of i.v. cholangiography were appraised in the light of the surgical picture in 209 patients. Exact preoperative diagnosis was possible in many cases, while no more than dilatation of the main bile duct was demonstrable in others. False negatives mainly concerned lithiasis of the common duct without jaundice, whereas false positive lithiasis was primarily observed in papilitis and cephalopancreatitis.

[Hepatic scintigraphy and laparoscopy in the diagnosis of the metastatic liver neoplasms. Comparative studies in 112 cases].

Personal experience of hepatic scintigraphy and laparoscopy in the diagnosis of secondary tumors of the liver is reported. The usefulness of both the techniques, associated, is proved by the small number of cases (2,98%) where metastases remained undiagnosed before surgery. Considering only the results of scanning, a larger number of "false positive" is noted, while those of laparoscopy are more often "false negative".

[Acute postoperative necrotizing enteritis. Apropos of one case after total hysterectomy with bilateral annexectomy].

The authors report a case of acute necrotising enteritis occurring on the 7th day after total hysterectomy with bilateral removal of the adnexa for fibroids in a 45 year-old woman. This is the first case in the literature to occur after hysterectomy. The clinical picture included the onset of intestinal obstruction with severe general toxic manifestations. The diagnosis was made at operation and, in spite of intestinal resection, the patient died on the 6th day, with liver and renal failure, anuria and irreversible collapse. The blood and stool cultures were negative, but the histological appearance of the lesions, without any apparent suppuration, were in favour of a vascular etiology. Necrosis of the mucosa and inflammatory oedema of the sub-mucosa, were accompanied by vascular changes suggesting thrombosis and congestion.

[Investigation on fats of Virola surinamenis; I. Glyceride and acid composition, and chemical nature of various unsaponificable components].

The A.A. report a work done on the influence of hypophysectomy on fluoride metabolism by the bones of rats. The results obtained indicate that hypophysectomy enhances F concentration in bones and teeth. The F retained is function of the growth of the animals and depends on the type of mineralization of the colagen matrix. All these effects seem related to the thyroid hypofunction subsequent to the pituitary removal.

[The clinical picture of pseudomyxoma peritonei].

The authors describe two cases of pseudomyxoma peritonei, one of which with a lengthy clinical course and the other with a rapidly fatal outcome. The clinical picture in pseudomyxoma peritonei is associated with a variety of benign or malignant mucus-secreting tumours, mainly originating in the appendix or ovaries. Survival is generally long owing to the lack of visceral invasion or metastases. An accurate prognostic assessment can only be achieved on the basis of identification of the primary tumours and its degree of malignancy.

Emergency laparoscopy in multiple trauma patients: experience with 106 cases.

Emergency laparoscopy was successfully used in 106 multiple-trauma and often unconscious patients with blunt abdominal traumas. In 40 cases a negative result avoided any subsequent exploratory laparotomy; two false negative were registered, while a diagnosis of retroperitoneal hematoma was possible in 6 cases.

[The Roux-en-Y jejunal loop in extrahepatic bilio-digestive anastomoses. Technic, indications and early complications in 101 cases].

Personal experience with Roux's Y loop in 101 cases in which bilio-jejunal anastomosis was performed for mechanical icterus of malignant (66 cases) and benign (35 cases) origin is described. It is felt that this operation does not present particular technical difficulties. End-to-end anastomosis is preferable. While bilio-duodenal and bilio-jejunal anastomoses are both sound practice, Roux's Y loop offers greater possibilities of application.
