Mastodynia due to fibrocystic disease of the breast controlled with thyroid hormone.

Nineteen patients were evaluated for breast pain and nodularity associated with fibrocystic disease. Rapid pain relief occurred in 73 of patients, with total relief in 47 percent after daily treatment with 0.1 mg of levothyroxine. Softening of breast tissue and decreased nodularity occurred within 3 months in many patients. Three patients had elevated levels of serum prolactin before treatment, with dramatic pain relief and normalization of prolactin levels after treatment. Further trials of levothyroxine in patients with mastodynia due to fibrocystic disease appear justified.

Autoantigens in breast cancer tissue.

Two antigens in specimens of human breast cancer have been identified as autoantigens. One antigen appears to be an IgG Fc fragment; the other, an IgG Fab fragment. Antibodies in sera of cancer patients directed against these antigens have also been identified. These autoantibodies occur in patients with breast cancer as well as other cancers, and rarely in patients with non-cancerous disease or in people screened in a detection center for breast diseases. The breast cancer patient found to have antibody directed against the IgG Fab antigen falls into a group that has improved two-year survival statistics over groups of patients who lack this antibody.

Chylous ascites following abdominal aortic aneurysmectomy. Management with total parenteral hyperalimentation.

Chylous ascites may follow operative injury to retroperitoneal lymphatics. When possible, early reoperation has been advised. This report describes a patient with chylous ascites following emergency abdominal aortic aneurysmectomy. Because the patient was not a candidate for reoperation, total parenteral hyperalimentation was employed in management. This approach resulted in a successful outcome.

Aspects of fibrocystic disease of the breast. Treatment with danazol.

The differing aspects of fibrocystic disease are presented. The various histological components making up the breast account for the variegated presentation of symptoms and findings. Pain, tenderness and nodularity represent the greatest problem with the former two prompting the patient to seek relief. The latter symptom, nodularity, causes concern for patient and physician as such changes must be distinguished from those of carcinoma of the breast. Guidelines for and consideration of premalignant benign changes are briefly presented. Data from a clinical trial of danazol for the patients with fibrocystic disease are presented. The drug was highly effective in affording relief from pain and tenderness. Some diminution in nodularity was also seen. The impact of this drug on the problems associated with fibrocystic disease of the breast is discussed.

A transthoracic devascularization procedure for control of bleeding esophageal varices: preliminary report.

A procedure for control of variceal bleeding has been described which involves "devascularization" of the distal one third of the esophagus and upper one half of the stomach combined with a Nissen fundoplication performed through a left lateral thoracotomy and an incision in the left diaphragm. Transection of the truncal vagi is required to effectively de vascularize the distal esophagus and, thus, pyloromyotomy or pyloroplasty is added. Six patients who were moderately high risk candidates underwent this procedure for control for persistent hemorrhage; five of these patients were operated on within 48 hours of admission. There was one postoperative mortality. The five survivors have not rebled, and there has been no evidence of encephalopathy or progressive liver deterioration over a mean follow-up of thirteen months.

Serum antibody in patients with breast disease: Correlation with histopathology.

Antibody to a breast cancer antigen was detected by immunodiffusion or complement fixation in at least one serum sample in 46% of 84 patients with a diagnosis of carcinoma, 34% of 96 patients with fibrocystic disease and 25% of 44 patients with fibroadenoma. A single serum sample obtained from screenees of the Detection Center for Breast Diseases was tested by immunodiffusion only and antibody was found in 3 of 206 screenees (1.5%). Eleven of 13 patients with breast cancer metastatic to lymph nodes and no detectable serum antibody either had recurrence or were dead within 12 months of mastectomy. Fifteen of 18 patients with breast cancer metastatic to lymph nodes and with detectable serum antibody were alive and free of disease for up to 24 months. Histologic slides from patients with a diagnosis of fibrocystic disease or fibroadenoma were reviewed for the presence of ductal epithelial hyperplasia. Ductal epithelial hyperplasia was present in a similar per cent of patients with no detectable serum antibody as those with antibody detected in the serum. Severe sinus histiocytosis of the axillary lymph nodes was present in 45% of those cancer patients with serum antibody and only 11% of those cancer patients with no detectable serum antibody.

Growth delay effect of combined interstitial hyperthermia and brachytherapy in a rat solid tumor model.

The rat mammary AC33 solid tumor model was used to investigate the efficacy of interstitial hyperthermia and/or brachytherapy. Subcutaneous flank tumors were heated with an interstitial microwave (915 MHz) antenna to a temperature of 43 +/- 0.5 degrees C for 45 min for two treatments, three days apart, and/or implanted with Ir-192 seeds for three days (-25 Gy tumor dose). Following treatments, tumors were measured 2 to 3 times per week. Hyperthermia alone produced a modest delay in tumor volume regrowth, while brachytherapy was substantially more effective. The combination produced a improvement in tumor regrowth delay compared to brachytherapy alone.
