[Adenomatoid tumor of the epididymis: cytologic diagnosis by fine-needle aspiration (FNA)].

Adenomatoid tumor of the epididymis diagnosed using fine needle aspiration (FNA). The adenomatoid tumor of the male genital system is a relatively uncommon, well recognized neoplasm with benign histologic appearance, and mostly asymptomatic clinical features. We describe a case of adenomatoid tumor of the epididymis diagnosed before surgery by fine needle aspiration cytology (FNA). This diagnostic procedure has allowed a rapid, reliable, and conclusive surgical approach.

Appropriateness in anti-nuclear antibody testing: from clinical request to strategic laboratory practice.

As a result of rapid changes in laboratory technology, clinical behaviour and patients' expectations, along with limited economic resources, there is a greater requirement for an appropriate use of autoantibody testing. We examine the various aspects and the most controversial points of the diagnostic procedure in systemic autoimmune rheumatic diseases, and make recommendations for the most efficient approach to autoantibody testing, based on selected publications and the relevant literature. Appropriateness is a complex task that can be achieved only by combining the efforts from the laboratory and the clinic, thereby using scientific knowledge, inter-disciplinary consultation and expert clinical investigation.

Familial occurrence of multiple myeloma and monoclonal gammopathy of undetermined significance in 5 siblings.

The etiology of monoclonal gammopathies remains unclear but familial occurrence of immunopathies seems to favor a possible hereditary background. In support of this view we report on a family in which five siblings had a monoclonal gammopathy. When MGUS was diagnosed in two of them, a family study was carried out: one sister died from multiple myeloma and four out of the seven living siblings were discovered to have a MGUS. The immunogenetic study (HLA) showed no direct correlation between haplotypes and the presence of the monoclonal protein. Nevertheless possible environmental exposure to viruses, chemicals and radiation was apparently excluded. This new report on familial involvement, the second ever reported for number of affected subjects, strongly supports the theory of genetic predisposition in the development of plasma cell dyscrasias.

Spindle cell and cartilaginous metaplasia in a breast carcinoma with osteoclastlike stromal cells. A difficult fine needle aspiration diagnosis.

The cytologic picture in fine needle aspirates from a unique type of breast tumor, with stromal proliferation of osteoclastlike giant cells, cartilaginous metaplasia and metaplastic spindle-shaped carcinomatous cells, is described. In this case, an erroneous cytologic diagnosis of fat necrosis associated with a rich component of reactive fibroblasts was made. This false-negative report, which was mainly due to lack of an obvious carcinomatous component in the aspirated material, is discussed with emphasis placed on the need to exclude spindle cell metaplasia in a breast carcinoma whose aspirates are characterized by a rich component of spindle-shaped cells.
