Descriptive analysis of a series of operations for prostatic adenomas in inhabitants of Lyon, France, in 1988. Urological College of Lyon.

A retrospective, population-based study was conducted in Lyon, France, to elucidate the benefits and risks of a treatment for prostatic adenoma (benign prostatic hyperplasia). Case records were reviewed for all prostatectomies performed on patients in Lyon in 1988 for benign prostatic hyperplasia. Data were obtained from all records of public and private hospitals. Of 408 procedures, 312 involved endourethral resection (transurethral resection) and 96 open surgery. The mean weight of resected tissue was 20.3 +/- 0.9 g after endourethral resection and 71.7 +/- 6.4 g after open surgery. About 20% of the resections took place in a university hospital center, 43% in a not-for-profit private hospital, and 37% in a private clinic; there were eight deaths in the first 3 postoperative months, and 11 patients required hospitalization for urologic complications. Length of hospital stay uniquely correlated with age and type of surgery.

[The value of cytological grading of prostatic adenocarcinoma. Comparison of the clinical state, the Gleason score and grade in 69 cases].

During a two-year period, 69 positive prostatic fine-needle cyto-aspirates were selected for a comparison between the cytological grade and the clinical data, and also with the Gleason score and grade obtained with needle-biopsy. The cytological grade is independent of stage, except for grade III that indicates an advanced tumor (T3-T4). On the other hand, the cytological grade provides valuable indications on the histopathological differenciation according to Gleason, with no grade II inferior to Gleason score 5, and no grade III inferior to Gleason 7. 90% of cytological grades III are Gleason grade 4. According to recent studies, we believe that the cytological grade I is related to invasive or in-situ carcinoma, but also to high-grade intra-ductal dysplasia. Thus, it is not surprising to find no correlation between grade I and the clinical status, nor between grade I and the Gleason score and grade, though grade I (or DIC 3) has a good diagnostic value.

Calcitonin-like immunoreactivity of amyloid fibrils in medullary thyroid carcinomas. An immunoelectron microscope study.

Using 3 polyclonal antisera directed against synthetic human calcitonin, we investigated at the electron microscope level the intra-or-extra-cellular fibrillar/filamentous aggregates found in 4 amyloid-rich medullary thyroid carcinomas (MTC) and in a number of other endocrine polypeptide tumours with or without demonstrable amyloid deposition. The antisera were applied by the immunogold procedure on ultrathin sections of glutaraldehyde-fixed, usually osmium-postfixed, tissues. In MTC cases, a strong labelling was present over two types of aggregates: one composed of rigid, criss-crossing fibrils 7-10 nm in diameter, suggestive of amyloid, and the other consisting of loosely arranged fibrils, 4-7 nm in width, often wavy or poorly defined. In both cases, the labelling was closely associated with that part of the sectioned fibril exposed to the antiserum. Amorphous material was sometimes present adjacent to the later aggregates, but did not bind the calcitonin antibodies. In contrast, no labelling occurred over the amyloid deposits found in two non-calcitonin-producing endocrine tumours of the pancreas, nor over the cytoskeletal filaments stored in various endocrine polypeptide tumours. The specific value of the labelling for calcitonin-like immunoreactivity was assessed by control tests, such as absorption of the antiserum by excess calcitonin and comparative use of normal serum and antisera directed against human IgG and P component. No immunoreactivity of the MTC amyloid fibrils was found using antibodies directed against katacalcin and human prealbumin. We conclude that in tumour tissues conventionally processed for electron microscopy, MTC amyloid fibrils of varying morphology can be selectively and specifically labelled for calcitonin-like immunoreactivity.

Aerobic responses to low level exercise testing following an acute myocardial infarction.

Thirty-four patients who had recently sustained an acute myocardial infarction performed low-level exercise testing with analysis of expired gas 7.1 +/- 2.6 days after the event. They were classified as finishers (F) and nonfinishers (NF) of the low-level protocol. The ejection fraction in the NF was 39 +/- 14% vs 56 +/- 17% in the F (p less than 0.01), and the NF had 2.6 +/- 0.8 vessels stenosed vs 1.8 +/- 0.9 vessels stenosed in the F (p less than .05). Ten normal subjects also performed the exercise test. At the same workload, patients with recent myocardial infarction had significantly lower oxygen consumption (NF less than F), significantly higher minute ventilation (NF greater than F), ventilatory equivalent for oxygen (NF greater than F), and higher respiratory exchange ratio (NF greater than F) than did normal subjects. The heart rate responses were higher in the post infarction patients than in normal subjects. The oxygen pulse was significantly lower in the cardiac patients compared to normals. These findings suggest that during the early recovery phase from an acute myocardial infarction, patients, particularly the NF, utilize less oxygen at submaximal work loads than do normal subjects. This suggests that in these patients part of the energy requirements for exercise are met anaerobically. This could be due to abnormal extraction of oxygen by the working muscles or as a result of poor delivery of oxygen due to abnormal left ventricular function.

[Immunohistochemistry of thyroglobulin in the diagnosis of clear cell carcinomas of the cervical region].

6 thyroid gland and 3 cervical lymph node clear-cells carcinomas are investigated by means of indirect immunofluorescence detection of thyroglobulin. Immunofluorescence was positive only in 1 case, which was a primitive thyroid carcinoma. In 7 other cases, its negativity pointed at the metastatic nature of the lesion, from a clear cell carcinoma of kidney (in 4 cases, nephrectomy was very remote, and in 3 cases, the renal lesion was discovered after our examination of the thyroid tumour). One observation showed negative staining reaction, and was a propagation of a parathyroid carcinoma to the thyroid gland. It is concluded that thyroglobulin can be used as an immunohistochemical marker to establish whether a clear cell cervical tumour originates from thyroid gland, or is a metastatic neoplasm.

[Immunohistochemistry of thyroglobulin by indirect immunofluorescence in thyroid cancers].

The authors have developed an indirect immunofluorescence technique for histochemical detection of thyroglobulin and have tested it on 66 tissue sections. Fluorescence reflecting the presence of thyroglobulin was elicited in 57 samples of differentiated thyroid tissue, including healthy or hyperfunctional tissue and primary or metastatic papillary or follicular carcinomas. Thyroglobulin was found to be distributed heterogeneously between different areas and different cells. It may occupy the whole cell, the apex, the colloid substance and sometimes extracellular spaces. No fluorescence was present in non-thyroid tissues. This technique could be applied mainly to the diagnosis of thyroid carcinomas, where 25 metastases have been examined by this method, and to clear cell carcinomas. In these two cases, thyroglobulin appeared to be a good marker of tumoral tissue.

Pharmacokinetics of contrast media: experimental results in dog and man with CT implications.

The pharmacokinetics of contrast media such as sodium iocarmate, sodium ioxithalamate, metrizamide, sodium/meglumine diatrizoate, and sodium ioxaglate (a recently introduced medium: a hexaiodinated monoacid dimer) was compared in 25 dogs. A biphasic phenomenon was observed due to the rapid diffusion of contrast media from plasma into tissue (interstitium) followed by slow urinary excretion. A bicompartmental analysis was performed in dogs for each agent, showing that the tissue distribution of ioxithalamate is higher than that of other media. Sodium/meglumine ioxithalamate, sodium iocarmate, and sodium/meglumine ioxaglate were also compared in 20 human subjects. The diffusion and excretion phases observed in man appear to be slower than in the dog. Significant variations of iodine plasma concentration from one patient to another were recorded for the same medium and at the same interval after injection. Significant differences were observed between ioxithalamate and iocarmate or ioxaglate plasma concentrations due to the greater tissue diffusibility of ioxithalamate. The mechanisms affecting contrast media diffusibility are discussed: osmolarity, liposolubility, protein binding, and molecular size. Variations in contrast medium concentration in plasma noted in different patients with the same medium are explained by variations in tissue distribution, contrast medium volume, patient age, and patient hydration. Some computed tomographic (CT) implications of these pharmacokinetic studies are discussed. The need for highly diffusible media in routine CT and for less diffusible media in CT angiography is emphasized.

Host-Pathogen Interactions: IX. Quantitative Assays of Elicitor Activity and Characterization of the Elicitor Present in the Extracellular Medium of Cultures of Phytophthora megasperma var. sojae.

Resistance of soybean (Glycine max L.) seedlings to Phytophthora megasperma var. sojae (Pms) is in part due to the accumulation in infected tissue of a compound which is toxic to Pms. The accumulation of this compound, a phytoalexin called glyceollin, is triggered by infection, but it can also be triggered by molecules, "elicitors," present in cultures of Pms. The ability of the Pms elicitor to stimulate phytoalexin accumulation in soybean tissues has been used as the basis for biological assays of elicitor activity. Two bioassays were developed and characterized in this study of the Pms elicitor. These bioassays use the cotyledons and the hypocotyls of soybean seedlings. The cotyledon assay was used to characterize the extracellular Pms elicitor. This elicitor was isolated from Pms cultures and purified by ion exchange and molecular sieving chromatography. The extracellular Pms elicitor was determined to be a predominantly 3-linked glucan, which is similar in composition and structure to a polysaccharide component of Pms mycelial walls.

[Differentiated thyroid carcinoma in children and adolescents: therapeutic strategy according to clinic presentation].

AIM OF THE STUDY: The aim of this retrospective study was to propose a therapeutic strategy according to clinic and pathologic presentations, in differentiated thyroid carcinoma in patients less than 20 years of age. PATIENTS AND METHODS: From 1995 to 2002, 74 patients less than 20 years of age were operated on for a differentiated thyroid carcinoma. Patients were divided in two groups according to the presence of lymph node (LN) detected before operation (19 "with LN" and 55 "without LN"). RESULTS: Surgery consisted of total thyroidectomy and lymph node dissection in the group "with LN". In the second group "without LN", total thyroidectomy was performed in 19 patients (associated with lymph node dissection in seven) and loboisthmectomy in 29 patients. Post operative radio iodine ablation was performed in 25 patients (16 "with LN" and nine "without LN"). During the follow up (median 71 months), 9/19 patients "with LN" underwent surgery for lymph node recurrence. Four patients of the "without LN" group were reoperated for recurrence in the controlateral thyroid lobe after initial loboisthmectomy. Initial presence of lymph nodes was a risk factor for reoperation. Survival without reoperation at 5 and 10 years was 58% and 38% for the "with LN" group and 96% and 91% for the "without LN" group respectively (P < 0.001). At the time of analysis, all patients were alive. Ninety percent of patients were in remission (68% of "with LN" and 98% of "without LN" patients). CONCLUSION: The risk of reoperation for lymph node recurrence during first post operative year is important in the "with LN" group. Therapeutic strategy consists of total thyroidectomy, complete lymph node dissection, radio iodine administration and l-thyroxine treatment. In "without LN" patients risk of lymph node recurrence is low. Treatment consist of total thyroidectomy, lymph node dissection and radio iodine administration are not systematic.

DXA body composition: theory and practice.

Dual energy X-ray absorptiometry (DXA) was used to measure human body composition on a high speed fan beam scanner. High and low energy attenuation pairs, produced by the various combinations of fat mass and fat-free mass in the human body, were compared to attenuation values produced by standard materials (aluminum and acrylic). These standards were measured in various combinations to construct calibration curves for fat and fat-free mass. Primary calibration of the aluminum/acrylic combinations was achieved by direct comparison to the dual energy attenuation produced by stearic acid and pure water. Whole body examinations were accomplished using three 45 s longitudinal passes of the fan beam. These passes were acquired and assembled to create a giant, isocentric fan beam with a single center of focus. In vivo precision was 0.009 g/cm2 for BMD and 425 g for fat mass and fat-free mass (s.d.).
