Sonographic diagnosis of periappendicular abscess in pregnancy.

The diagnosis of appendicitis in pregnancy is complex because of altered maternal physiology and anatomy, and definitive therapy is often delayed. Ultrasonic screening is important in either diagnosing or excluding many pathologic conditions that complicate pregnancy and present a clinical picture similar to that of appendicitis. Reported here is a case in which a periappendicular abscess due to a perforated appendix was diagnosed by sonographic screening.

Prophylactic single-dose co-trimoxazole for prevention of urinary tract infection after abdominal hysterectomy.

The efficacy of a preoperative single-dose of co-trimoxazole in reducing postoperative urinary tract infection and febrile morbidity after abdominal hysterectomy was evaluated in a randomized placebo-controlled study of 90 patients undergoing surgery. Among the co-trimoxazole patients, 6.2% developed urinary tract infection compared to 31% in the placebo group (p less than 0.001) and 12.5% febrile morbidity compared to 38% in the placebo patients (p less than 0.025). No adverse side effects of co-trimoxazole were observed and this regimen seems both safe and effective.

Fetal supraventricular tachycardia: prenatal diagnosis and pharmacological reversal of associated hydrops fetalis.

Intrauterine fetal supraventricular tachycardia is a rare condition often associated with the syndrome of nonimmune hydrops fetalis. When the fetus is preterm it is vital to treat the arrhythmia with maternally administered drugs and different regimens have been reported in the literature. We report 1 case of this arrhythmia successfully treated in utero with maternally administered digoxin resulting in complete reversal of the hydrops fetalis and the birth of a healthy baby. A review of the relevant literature is brought forth.

[Prediction of ovulation by echographic monitoring and the appearance of the double outline of the follicle].

Using ultrasound examination of the ovary routinely has given us an opportunity to find a sign that is always present when ovulation is about to occur. It consists in the appearance of a double contour inside the wall of the follicle, the outline of the wall losing its sharpness (due to the separation of the cellular mass of the granulosa). In 55 cases studied by ultrasound and laparoscopy there were 34 proven ovulations. The sign was present in 29 of these cases (85%). In 21 cases when ovulation did not take place the sign was present in only 2 cases (9.5%). This important sign therefore can be used in every case where it is necessary to work out the exact moment of ovulation for artificial insemination or in vitro fertilisation.

Single-dose mezlocillin prophylaxis in emergency cesarean section.

A single 5-gm dose of mezlocillin or a placebo was administered intravenously 30 minutes before surgery to patients undergoing emergency cesarean section. The assignment of drug or placebo was randomized. Postoperative morbidity occurred in 62.5% of patients receiving placebo and in 18.4% of those receiving mezlocillin (P less than 0.001). The incidences of febrile morbidity, endometritis, and urinary tract infection were significantly lower in the group given mezlocillin. Other benefits of antibiotic prophylaxis included a shorter hospital stay and no serious infections in the group given mezlocillin.

Cellular and humoral suppressor activity induced by concanavalin A-stimulated human fetal liver cells.

Incubation of human fetal liver cells (HFLC), obtained from 8-12-week-old fetuses, with mitogenic doses of Concanavalin A (Con A) resulted in generation of a population that could suppress alloantigen-induced lymphoproliferation. The supernatant media in which these cells were activated suppressed DNA synthesis, both in mitogen and alloantigen induced peripheral blood lymphocyte cultures. Suppression was achieved only when the Con A-activated cells or the supernatants were added during the early stages of sensitization. Treatment of the cells with mitomycin C after Con A activation did not alter their ability to mediate suppression. This study indicates the presence of suppressor or presuppressor cells in the liver in the early period of fetal life. If the suppressor activity of HFLC in vitro is also induceable in vivo, it may contribute to the fetal defense mechanism against maternal rejection.

Successful control of pheochromocytoma in pregnancy. Case report.

A patient found to have a pheochromocytoma in the 16th wk of pregnancy was successfully managed by medical treatment with phenoxybenzamine hydrochloride and propranolol. In the 38th wk of pregnancy she was electively delivered by cesarean section, and the tumor was simultaneously removed.

The successful treatment of a case of primary sterility resulting from Fredrickson's Type V hyperlipemia and panhypopituitarism.

Successful treatment of primary sterility in a woman having the rare association of panhypopituitarism with Fredrickson's Type V hyperlipemia is described. Replacement therapy with l-thyroxine, prednisone and a low fat diet cleared the patient's blood of the excessive chylomicrons and very low density lipoproteins. Ovulation was induced with human gonadotrophins, and triplets (two normal girls and a boy) were born.

Suppressive effect of 1,2-dibromo-3-chloropropane on human spermatogenesis.

Azoospermia was diagnosed in six factory workers who had been chronically exposed to 1,2-dibromo-3-chloropropane. Infertility was the presenting symptom in two patients and a decrease in libido or impotence characterized the others. Hormone studies revealed elevated plasma follicle-stimulating hormone levels and normal plasma luteinizing hormone and testosterone concentrations. Testicular biopsy showed selective atrophy of the germinal epithelium, intact Sertoli cells, and a normal appearance of a relatively increased number of Leydig cells.

Fracture and chemical composition of the deposit formed on the Lippes loop after prolonged use.

In order to confirm or refute the theory that there is the need to replace IUD's every few years due to their breaking, 26 Lippes Loops were tested. The sediment found on the devices was composed mainly of calcium carbonate. After removal of the sedimentation, it was seen that there was some corrosion of the body of the devices which caused a decrease in the IUD thickness. Pressure tests showed that an IUD which has been in use for a long period of time is more rigid, has less ability to deform and breaks under less pressure. It seems that the current medical opinion stating that IUD's should be replaced every few years, is valid.

Significance of hyperprolactinemia in 70 women with amenorrhea.

The clinical, radiological and hormonal findings in 70 amenorrheic patients are described. Hyperprolactinemia was found in 18 patients (25.7%) who were divided into three groups: nine patients with functional secondary amenorrhea, six patients with radiological evidence of pituitary tumor, and three patients with primary amenorrhea. One of the latter group had primary hypothyroidism. Plasma gonadotropin concentrations were normal or low in most patients, with the exception of five women with ovarian failure in whom the levels were significantly raised. Galactorrhea was diagnosed in 23 patients (32.8%), and in 16 of them plasma prolactin levels were elevated. Recent experience has shown that reduction of prolactin levels to normal by removal of prolactin-secreting tumors or by treatment with bromocriptine results in the restoration of normal menstrual cycles and fertility. The hormonal and radiological identification and characterization of hyperprolactinemic amenorrhea are therefore of prime importance.
